LOCAL SCHOLARSHIP APPLICATION INSTRUCTIONS General Instructions Complete all questions neatly and provide all information requested, especially signatures. Incomplete applications and/ or incorrect application form will disqualify applicants for scholarship consideration. Pay close attention to deadlines and eligible criteria. Copies of local scholarship criteria and application forms can be downloaded by the following instructions: 1. Go to http://www.shawanoschools.com/ 2. Click on Schools and Shawano High 3. Click Student Services on left menu 4. Click on SCHS Community Scholarship Booklet 5. Save applications to word document Completed forms are to be turned into the SCHS Student Service Office by February 13th for review by the Shawano Community High School Scholarship Committee. Once the applications are received by the Student Service Office, any information contained therein will be made available to the scholarship sponsors or faculty committees for their review and evaluation. Any required statement of need, financial disclosure, student essay, G.P.A., class rank, or school activity is the responsibility of the applicant and should be provided by that applicant with the initial application. All applications must be given to Mrs. Parker or Mrs. Marquardt in Student Services by Feb 13 th, 3:20 pm. Contact your school counselor if you have any questions regarding local scholarships. NOTE: A separate application form must be completed for each scholarship. Most scholarship organizations require the SCHS Local Scholarship Application. However, some scholarships require a different scholarship application. The following scholarships require a different application: FBLA, FFA, J. Ainsworth Agricultural (use FFA),Wolf Rivers Builders Association, Shawano Rotary Emereth Brunner, Shawano Area AARP, and Dodge Family Scholarship Foundation, Inc. - Medical (Sandy Dodge Application). Must have a service affiliate-must be the daughter, son, wife or widow of a veteran. A granddaughter or greatgranddaughter may apply if she is an American Legion Auxiliary member. 2. Good Citizenship, good sportsmanship, and good moral conduct shall be important considerations. 6. The award shall be distributed equally among male and female students. FROM THE PINERIES TO THE PRESENT From the Pineries to the Present Amount $100.00 Number 1 Qualifications 1. Graduating senior. 2. Applicant will fill out the SCHS Local Application form. In addition, a 200 word minimum essay explaining the history, people, communities, cultural heritage, historic sites, natural environment and recreational opportunities of the Pineries to the Present area (Shawano Co) must be submitted or show evidence of participating in the Pineries to the Present activities. 3. This award will be available to a student who will pursue at least one year of additional training or education beyond high school. will be made by the Pineries to the Present Recipient will receive award from Pineries to the Present with proof of enrollment. 14

FUTURE BUSINESS LEADERS OF AMERICA SCHOLARSHIP Future Business Leaders of America Amount $100.00 Number 2 Qualifications 1. Must be a FBLA member in good standing. 2. Must be a senior. 3. Need to be an active member: participate in regional conferences or be an FBLA officer. Students will complete the FBLA Scholarship Application. Baumann Memorial Automotive Scholarship Kenneth F. Baumann was born and raised in Shawano, Wisconsin, graduating from Shawano High School with the Class of 1962. He owned and operated Ken s Auto Repair where he did auto and truck repairs and auto body work. The Baumann Memorial Automotive Scholarship was established to not only honor Kenneth s memory and life work, but also to honor his deceased father, Arthur, who was a tractor and truck mechanic for Farmers Implement, his deceased brother, Rodney, who was an auto technician for Lasch Motors, his brother, Harold, who was a diesel truck mechanic for Catencamp, and his son, Kendall, a Master Auto Technician for Bergstrom Mercedes. Donald L. Moede Memorial Scholarship This scholarship was established in loving memory of Donald L. Moede by his brother, John. Donald graduated from Shawano High School and attended UW River Falls. After completing his freshman year, Donald worked that summer to earn money for his sophomore year. At age 19, he was involved in a swimming accident that left him a quadriplegic and he spent two years in hospitals, enduring several surgeries. Donald was determined to complete an accounting correspondence course, all done without the use of computers which were not available at that time. Donald managed a H&R Block office for many years prior to his death in 1999. Eligibility: Graduating seniors from Shawano High School who are planning to obtain post secondary education in the accounting field at an accredited college, university or technical college. SHAWANO EDUCATION ASSOCIATION SCHOLARSHIP The Shawano Education Association established this scholarship in the 1982-83 school year with revisions to the qualifications in the 2007-08 school year. Amount $300 Number 1 Qualifications Application & a. The applicant(s) shall be a graduating senior from Shawano Community High School. b. The applicant shall either be a child of a Shawano Education Association Member (current or retired) or pursuing a post-secondary program in education. c. Only applicants who have maintained a 3.0 or better GPA (on a 4.0 scale) throughout their years of enrollment at Shawano Community High School will be considered. d. The applicant(s) must be of good moral character and exemplify good citizenship throughout their high school years. Applicants should complete the SCHS Local scholarship application form. The selection committee shall include at least one Shawano Education Association member. Collection of the Award The scholarship recipient must provide evidence of having maintained a 3.0 GPA for the first semester of post secondary education, and evidence of enrollment for the next semester. If the recipient does not meet these criteria, the scholarship may be awarded to a runner-up previously selected by the SCHS scholarship committee. Upon submission of evidence to the Shawano Education Association treasurer, payment will be made to the student. SHAWANO MASONIC LODGE SCHOLARSHIPS No. 170 F & AM SHAWANO MASONIC LODGE Amount $1,000.00 Number 1 Qualifications 1. The recipient must be a graduating senior from a Wisconsin High School, displays a commendable academic record and the potential to successfully continue in a post secondary college or university. This includes a junior college or technical college when the full credit load is acceptable to a degree granting institution. The attending college may be private or public and the course of study must lead toward an associate or baccalaureate degree. 2. The academic course of study selected should be the choice of the student and not a factor in the selection process. It is deemed that the arts, letters, sciences, education, agriculture, business, etc., are all an integral part of the American way of life. 3. The recipient(s) will have demonstrated leadership ability and service as evidence by school, community and church/synagogue involvement. 4. The recipient must initiate schooling within 6 months of receiving the award (in name only) and will receive the actual funds upon receipt of proof of matriculation to college. If the recipient does not enroll in college, the scholarship money should be returned to the Wisconsin Masonic Foundation. 5. The Shawano Masonic Lodge would prefer that its scholarship be awarded to a student meeting all criteria, but who does not otherwise receive scholarships from other sources. 37
