Hawke s Bay District Health Board Position Profile / Terms & Conditions Position holder (title) Cancer Pathway Coordinator Reports to (title) Nurse Manager Oncology and Medical Subspecialties Department / Service Oncology Services Villa 6 Purpose of the position Coordinate proactive management and optimisation of the process through the cancer patient pathway from referral with high suspicion of cancer (HSCan) to first treatment. To ensure and prioritise a focus on patient safety and quality relating to processes to enable equitable access to services within the HBDHB Cancer Services. Assist with the development of strategies to streamline the cancer pathway reducing duplication and waste, and defining a consistent approach to cancer pathway management.. The role is the communication and network single point of contact between HBDHB clinical teams managing patients with HSCan to capture the FCT data. Supports the delivery of organisational KPI s including relevant MOH target. To support the co-ordination of MDM administration, including agendas, VC enablement, relevant health record documentation and capture of MDM outputs. FCT Governance committee and Corporate Pathway group Patients and whanau Mid Central DHB cancer services clinical and admin teams (MOSAIQ) Private organisations and other DHB diagnostic services Tertiary treatment centres. GPs Cancer stream coordinators in other DHBs Regional Cancer network.

Dimensions Expenditure & budget / forecast for which accountable Challenges & Problem solving NA The role responsibilities include review of relevant clinical information (hard copy and electronic), working with the cancer nurse coordinators (CNC), CNS, and MDM coordinator to identify those patients who are to be included in the FCT programme. Completing the data collection for the monthly reporting to the organisation and the MoH. To enable the CNC and CNS roles to focus on the patient and to support the Consultant to mitigate any delays on the patient s pathway to cancer diagnosis and treatment Maintains the FCT statistical database in FCT module in ECA. Works with MDM chairs and stakeholders to set agendas in a timely manner, assemble the necessary clinical documentation, and capture the MDM outputs Works with IT to facilitate the functioning of the VC units and hardware Discusses with radiology, pathology; and oncology the resource requirements to facilitate their attendance at MDMs.. Need to have a comprehensive understanding of PMS in both MCDHB (Mosaiq) and HBDHB (ECA) Establish a network of contacts across external DHBs and regional cancer centres to facilitate information and data capture. Key Accountabilities SERVICE DELIVERY Administrative systems are established and maintained to ensure the delivery of information and the smooth running of the FCT programme.. Establish comprehensive understanding and application of the FCT data definitions to support regular review and monitoing of the patient journey. Recommend actions and strategies to facilitate patient journey through clinical Services in HB and assist in the development and implementation of alternative apporaches to timely quality patient care in the relevant services Work with CNCs and CNS to to identify delays, and inefficicent processes, including onward referrals in the most timely manner. Faciliatte MDM coordination with system and process functions. Support the MDM chairs and stakeholders to enable a robust and conmprehensive discussin and outcome for patients. Establish robust system and process for monitoring FCT pathways of individual patients linking with the HB clinical teams. Participate in the development and monitoring of the clinical tracking process.with the speciality teams. To undertake any other duties as agreed within Health Services as future needs may demand. Collation of service stats from the FCT module to enable review and audit. Weekly case management reporting/meeting with FCT team of patients on FCT with HSCan Monthly reporting of FCT patients pathways and delay codes. Monthly reporting of FCT targets for MOH. Capture and reporting of MDM outcomes in clinical records and ECA. Monthly reporting of MDM attendances. Annual review of key reports. COMMUNICATION NETWORK SPOC for HBDHB FCT reporting and communication.including transfer of information. To communicate effectively in both written and oral form with all team members, patients and the clinical teams. appropriately. Adheres to the Hawke s Bay District Health Board Privacy/Confidentiality Code Link with Bookers and admin personnel to maximise timely clinic scheduling and minimal DNA Patient data is processed accurately, matching codes and fields for activtiy description and capture. To participate in the ongoing implementation of cancer pathways, protocols, policies and future cancer startegy. To escalate potential breaches in a timely manner to maxmisie the potential of all cancer patients reaching first treatment within 62 days of referral. Case management forums are efficient. MDM agendas are timely MDM outcomes are reported to stakeholders as required.. Routine reports are comprehensive and meet timelines. Information pathways to ECA FCT data collection are monitored for validity and functionality. Communication with CNC and CNS enables the cancer patient reporting and minimses delays.

INFORMATION SYSTEMS Competent to work across ECA and to gain understanding of the DHB PMSs. Accurate and complete data is entered into the patient management systems in a timely manner.. Referrals are regularly audited and missing referrals are traced. Adherence to HBDHB Privacy/Confidentiality Code/Patient Code of Rights All relevant clinical informtion is scanned into the PMS. E.g MDM outcomes, faxed referrals, external diagnostic reports.. All clinical information available for case management of HSCan patients. Monthly reports reflect patients journeys from ref to diagnosis to treatment and the delays that may have occurred. Key Competencies CUSTOMER SERVICE Open and responsive to customer needs.
