JOB PURPOSE To contribute to the work of the Community Children s Nursing Service (CCN-S) within NHS Lothian in providing the highest standard of personalised nursing care to patients and their families in partnership with all members of the multi-disciplinary team. The post holder is responsible for the ongoing assessment of care needs and the development, implementation and evaluation of programmes of care under the supervision of senior staff nurse and the management direction of the Community Children s nurse. The post holder has responsibility and accountability for maintaining both clinical and staff governance. 3. DIMENSIONS The post holder implements care to patients with acute and chronic health care needs as directed by the Community Children s Nurse, across the NHS Lothian area. The post holder will not be directly responsible for any budgets but must be aware of the need to work within them. The Lothian University Hospitals Division (LUHD) delivers services within the environment of a University Teaching hospital setting.

4. ORGANISATIONAL POSITION Community Childrens Nurse Team Leader Community Childrens Nurse Band 6/Band 7 This Post Health Care Support worker 5. ROLE OF DEPARTMENT Community Children s Nursing Department provides a high quality, equitable service to children, young people and their families throughout the City of Edinburgh, East Lothian, Midlothian, West Lothian and where appropriate South East Scotland. Services provided are: Community Children s Nursing Service (CCN-S), Stoma Nurse Service, Urology Nurse service, Palliative Care Nurse Service, Discharge Planning Service, Outreach Service, Respite residential service, Complex needs schools and the Drop in centre at RHSC. The CCN service provides a high standard of specialised paediatric nursing care to children and young people with a broad spectrum of nursing needs. This service facilitates a safe and effective discharge from hospital to the community setting and empowers patients and their families to access health care services appropriately.

6. KEY RESULT AREAS Professional 1. Practice at all times within the Nursing and Midwifery Council Code of Professional Conduct. 2. Develop the role by using evidence-based practice and continuously improve own knowledge, following PREP guidelines. encompassing all aspects of Community Childrens nursing practice 3. Implement/conduct clinical evidence based practice in accordance with national SIGN guidelines and clinical standards. 4. Act as an exemplary professional role model for Community Children s nursing. Managerial 5. Support the Senior Staff Nurse and CCN Case Manager to ensure continuing high standards of care by prioritising delegated workload in response to the patient and their families needs. 6. Be aware of differing systems and resources within the geographical area of the NHS Lothian. 7. Recognise importance of resolving complaints timeously and effectively at local level and escalate as appropriate. 8. Participate in the formulation of team objectives relating to the specialist field of Community Children s Nursing in order to develop the service in response to the changing needs of the community. 9. Accept responsibility for active membership on councils, committees and work groups in order to facilitate evidence-based practice. 10. Maintain department stock levels, through ordering of stores, ensuring efficient and effective use of resources. Clinical 11. Assist the CCN Case Manager with the management of a caseload for a defined geographical area. 12. Act as the named nurse for allocated patients within the caseload by providing ongoing assessment, planning, implementation and evaluation of care in partnership with the patient, their family and both the multi-disciplinary and multi-agency professionals. 13. Work within, and monitor, standards of care within the defined policies, procedures, standards and protocols of the Department, Directorate, and Division to ensure adherence to, and delivery of, a high quality service. 14. Comply with the policies and procedures relating to Child Protection and Protecting Vulnerable Adults. 15. Maintain effective communications with patients, relatives, and members of the multi-disciplinary / multi-agency team to ensure that appropriate information is shared and patient needs are met. 16. Perform specialist technical nursing procedures ensuring a high standard of nursing care. 17. Adopt safe working practice in the community, identify risks and adhere to the Lone Working Policy. 18. Identify clinical risks for incident reporting ensuring care is taken to safeguard patients at all times. 19. Ensure current practice is evidence-based utilising standards of care, research and audit within the specialist field of Community Children s Nursing. 20. EQUIPMENT AND MACHINERY The post holder is expected to have a working knowledge of and the ability to use all equipment used in the area however may not have daily clinical involvement. To ensure all equipment is stored safely and maintained to a high standard ready for use as required. SYSTEMS The CCN-S provides monthly written statistics to enable efficient workforce planning and population of databases Maintains written patient records Inputs and retrieves information from patient information system and Special Needs System TRAK and Community TRAK Accesses internet and intranet for professional and clinical information Participate in departmental audits Accesses and utilises IT systems for Home Enteral Feeding Supply Delivery, Continence Supply delivery, Community Equipment Provision 8. ASSIGNMENT AND REVIEW OF WORK The post holder s work is generated from the services within the Division, Primary Care, Education, Social Work and voluntary agencies, and from legislation, and directives set by the Scottish Executive Health department. The desired outcome is to benefit the quality of service to the patient and their families. The CCN service operates an open referral system and receives referrals directly from: parents, hospital professionals, primary health care professionals, social work and education.

The post holder s caseload is delegated by the CCN in response to the patient, their family and professional needs and can be categorised as planned case management or crisis intervention. The post holder s daily workload is a combination of pre-planned patient contact organised by the post holder in relation to the caseload needs; and unplanned new referrals to the service to be arranged in response to the patient s needs. The Post Holder will have a Professional Personal Development Plan, which is reviewed annually by the delegated line manager. 9. DECISIONS AND JUDGEMENTS The post holder will work within their own clinical and professional limitations. The post holder is expected to make clinical and professional decisions and assess the current care needs of individual patients taking remedial action as required, daily. The post holder is expected to take remedial action and seek support in highly complex, rapidly changing and unfamiliar situations including child protection issues and crisis management. The post holder is expected to provide clinical and professional advice to the multi-disciplinary and multi-agency teams. 10. MOST CHALLENGING/DIFFICULT PARTS OF THE JOB Lone working creates clinical isolation and risk to personal safety on a daily basis. Working in a diverse geographical area balancing the demands of individualised patient care and service requirements. This encompasses a variety of challenges: time management, risk assessment and driving Exposure to complex and challenging situations: visiting families in their home, cultural diversity, spiritual diversity, deprived and disadvantaged communities, child protection, violence and aggression. 11. COMMUNICATIONS AND RELATIONSHIPS The post holder will communicate efficiently and effectively with patients, their families, all health care professionals across acute and primary care, education, social work, voluntary agencies and professional groups, aiming to provide a high standard of patient care and a first class CCN service.
