Who we are Silver Chain Group is a not-for-profit organisation delivering community health and care services across Australia. We have a proud history of serving Australians for over 110 years in Western Australia and more than 120 years as RDNS in South Australia. We currently deliver our services in Western Australia, Queensland, New South Wales, Victoria and South Australia. We are the largest provider of non-government community-based palliative care services in Australia with over 30 years of specialised palliative experience. There is consistently strong evidence for the positive outcomes of our service, inclusive of the increased occurrence of patients dying in the place of their choice, reduced length of stay in hospitals, and improved palliative support services in the community. Whilst clinical governance lies with Silver Chain, our team of registered nurses and doctors work closely with medical specialists, general practitioners, health professionals and local palliative care services to ensure the highest quality of care and service. 2 3

Silver Chain Community Palliative Care in Western Sydney In New South Wales, we already support the Last Days of Life service provided by eight Local Health Districts. From July 2017, the community of Western Sydney will have access to our comprehensive in-home palliative care service. This service is based on our widely successful Perth model, which is recognised nationally as a best practice model of communitybased palliative care. Our new service will enhance the existing palliative care services within Western Sydney by increasing community based services for patients and their families who choose to be supported at home. Our service will be free of charge to all people with a Medicare card. Pharmaceutical costs may be incurred in addition to this service. 4 5

What services are available Our palliative care team includes: A palliative care medical specialist General Practitioners A Nurse Practitioner Registered Nurse Case Coordinators A Hospital Liaison Nurse Registered Nurses for home visits and phone support (24/7) Assistants in Nursing for personal care and respite (4 hours maximum) Social workers/counsellors A spiritual care worker A volunteer coordinator Access to other Allied Health staff (physiotherapist, occupational therapist, speech therapist and dietetics) Conditions covered by our Community Palliative Care Service Our service is focussed on, but not confined to the last three months of life. It is available to people with advanced, life-limiting diseases such as cancer, chronic obstructive pulmonary disease and congestive heart failure; and degenerative conditions such as Huntington s disease and dementia. Patient Eligibility Patients are eligible for the service if they: Live within the Western Sydney LHD boundaries Have an advanced, progressing, life-limiting condition Are over 18 years of age, or a patient transitioning from child to adult health services Have agreed to referral to the service To discuss your patient s eligibility for our service, please call our Contact Centre on 1300 758 566 to speak with one of our senior registered nurses. How to refer Any doctor can refer a patient to this service using an official Palliative Care referral form available on our website (www.silverchain.org.au). Local Health District staff can refer directly via EMR. If required, clients and family are able to selfrefer via a phone call to our Contact Centre on 1300 758 566. Patients may access our service episodically; care may be paused and resumed as their care needs change. We will refer to other service providers for ongoing service if required before discharging from our care. 6 7
