Date: December 2014 Job Title : Health Care Assistant Department : Location : WDHB Reporting To : Charge Nurse Manger Direct Reports : Nil Functional Relationships with : Internal District Nurses HCAs Locality Assistants External Patients and their significant others Purpose : To work as a member of the interdisciplinary team to support the district nurse on afternoon duty. KEY TASKS Provide support and a sense of security for the district nurse in the evening Provide support for the district nurse in the delivery of safe care to clients in their homes Develop a working knowledge of standard precautions and use them at all times EXPECTED OUTCOMES District nurses are accompanied during client visits between 6.30 and 9.30 pm. Challenging incidents are responded to appropriately. The district nurse is assisted in locating client houses after dark. Follows district nurses instructions in assisting with basic cares to clients. Demonstrates skills and understanding in the provision of basic client care: hygiene, comfort, pressure area, mouth care. Safe lifting and transferring techniques are used to protect self, district nurse and clients. Is familiar with Infection Control policies and them as a basis for client care. Training and development needs are identified. Maintain work practice within WDHB policy framework Undertake other reasonable tasks as required by the district nurse to support the smooth operation of the locality Understands and works within the Privacy Act, Patient Codes of Rights and Responsibilities, and Informed Consent Completes all mandatory training as required All requested tasks are performed according to instructions.
