KEY TASKS EXPECTED OUTCOMES Clinical assessment and counseling to Maori Tangata Whaiora Manage a clinical caseload Appointments for Tangata Whaiora are scheduled for assessments and follow up sessions. Whaiora are regularly reviewed and appropriate care plans/discharge plans are updated and completed Triage Manage referrals and follow standard process and procedures Facilitate group programmes for Maori Tangata Whaiora Provide group programmes within a Tikanga Maori framework that will promote the concept of healing in relation to alcohol and drug misuse. Facilitation of group programmes that ensure effective delivery of harm reduction philosophy. Review programmes to ensure continuous quality improvement. Project Work Involvement in community working parties and initiatives pertaining to Maori Tangata Whaiora alcohol and drug issues. To contribute to the ongoing development of Te Atea Marino within Counties-Manukau District Health Board catchment area. Commitment to the principles within Te Tiriti o Waitangi To recognize Individual Responsibility for Workplace Health and Safety under the Health and Safety in Employment Act 1992 Participation in ongoing reviews of Clinical Pathways. Pro-actively seek input/feedback from Tangata Whaiora in regards to service delivery and improvements. Seek opportunities to promote healthy choices for Maori Tangata Whaiora around the use of alcohol and drugs. Demonstrated practical application of the principles within Te Tiriti o Waitangi. DHB s Health and Safety policies are read and understood and relevant procedures applied to their own work activities.
