Job Purpose To facilitate the co-ordination of clinical adverse event management systems and processes across NHS Borders and have the operational lead role for Clinical Adverse Event Management within the Clinical Governance and Quality Team. Supporting the development of a just culture in NHS Borders using a human factors approach. To manage the screening, interpretation, and analysis of coding of Adverse Clinical Event reports on Datix, and the collation of correspondence and communications related to Clinical Risks. To provide specialist advice and guidance in the management of adverse event reviews. To facilitate and support clinical staff and managers to develop proactive approaches to managing clinical risk and creating a positive patient safety culture. To provide specialised on the job training, advice and guidance on the completion of clinical adverse event reporting templates on Datix. To ensure that clinical adverse event recording, monitoring and review systems are in place and that lessons learned from the analysis of adverse events inform service review and development. Scope and Range This is an NHS Borders wide post. NHS Borders provides the full range of hospital based services such as acute, community and day hospitals, and covers a population of 109,700. In addition, NHS Borders is responsible for the delivery of the full range of Primary Care and Mental Health Services and is jointly responsible with Scottish Borders Council for Learning Disability Services. Clinical services within NHS Borders are organised under four Clinical Boards, Borders General Hospital, Borders Primary and Community Services, Mental Health and Learning Disabilities. The post holder is a member of the Clinical Governance and Quality Team, which functions across NHS Borders. There is no budgetary responsibility. Main Duties/Responsibilities Management of Clinical Adverse Event Reports Detailed inspection, interpretation and analysis of all the information reported on the Clinical Adverse event System, Datix, informing judgement about the appropriate action and level of follow-up required for each individual adverse event. Responsible for the production of quarterly and annual reports, and ad-hoc statistical data as required for meetings and departments, as well as response to freedom of information requests. Contribute to the development, implementation and review of the clinical adverse event strategy and policy, addressing national drivers and local needs. Undertake, with the Clinical Governance and Quality Facilitator Patient Safety, thematic review of adverse event data to inform the focus of improvement activities and the patient safety programme. Promote the principles of a Just culture in the management of adverse events ensuring appropriate support is deployed for patients, families and staff during the adverse event review process. Liaison with patients, families, staff and external governing bodies to provide support and guidance throughout adverse event review. Reviews and reports Alert line manager and Head of Quality and Clinical Governance as appropriate about any high profile adverse events and communicate with the Director of Nursing and Midwifery, Medical Director, Associate Medical Director for Clinical Governance and Quality and senior managers as appropriate regarding major and extreme adverse events. Oversee the arrangement of and facilitate adverse event review processes making sure these are carried out within appropriate timescales. Compile, and produce documents and reports from complex information using Root Cause Analysis methodology and tools, collating information gathered from written communication with staff and from health records - >40% of this post entails interpretation of frequently complex information. Support internal processes where there are adverse event management implications e.g. Fatal Accident Inquiries, complaints and claims. Oversee appropriate responses to outcomes and learning from adverse event reviews and investigations. Present regular updates on Clinical Risk activity and investigations to the Clinical Governance Groups and Clinical Board Level Groups, as required. Training Facilitate the development and maintenance of staff s ability and clear understanding of grading of incidents through provision of specific training in the use of the Risk Matrix. Coach other staff in the use of a human factors approach and root cause analysis techniques in the review of adverse events. Responsible for ensuring that the Clinical Risk internet microsite is maintained. Other Influences departmental policies within own and other work areas and advises on any changes necessary as a result of new guidelines or legislation. Contribute to the self-assessment processes leading to external peer review by bodies such as Healthcare Improvement Scotland. Ensure Data Protection legislation is adhered to. Decisions and Judgements The post holder will have a high level of autonomy in the management of their workload and be expected to anticipate problems taking preventative/corrective action as appropriate. The post holder will be required to assess and prioritise the workload as appropriate, according to the quantity and severity of adverse events arriving on a day-to-day basis, planning own workload around unpredictable and conflicting demands e.g. dealing with queries from staff at all levels and from different areas, managing ongoing reviews and new adverse events arriving. There is a requirement to process complex information, in relation to clinical incident reporting and review. Making decisions as to the severity of adverse events based on the information given. The post holder is responsible for deciding the level of follow up required for each adverse event and to take actions accordingly and work within appropriate timescales. The post holder advises managers and clinicians on decision making regarding communications of adverse events with other areas/managers re collaborative actions and feedback. The post holder will be required to recognise/decide when Senior Managers, Line Managers and the Head of Quality and Clinical Governance should be alerted about significant adverse events and when to escalate to significant adverse event review. Identifies where trends in statistical data require escalation to the next level of review or to Senior Management level. Will be required to assess and guide staff undertaking reviews in the use of the NPSA decision tree to ensure appropriate pathways are following in the management of adverse events. The post holder is required to provide specialist advice to and have key relationships with frontline healthcare professionals and work with a wide variety of disciplines, agencies and organisations, having an understanding of their roles and terminology. The post holder requires highly developed communication and interpersonal skills. They will be required to advise/persuade individuals and groups to gain co-operation where there may be resistance/barriers to change or to acceptance the information. The post holder will require positive negotiation and motivation skills to influence change within and out with area of direct responsibility. They will be required to be supportive to staff during highly stressful situations. 9. Physical Demands of the Job The post requires prolonged periods of intense concentration, a high degree of accuracy and the ability to retain and communicate complex and sensitive information. Frequent/daily computer use for prolonged periods of time.
