Bringing more world class care to our patients Presented by Brian Bunkers, M.D., President and CEO, Deb Richards, Nurse Administrator, David Berg, Associate Administrator When you or a loved one find yourself in need of health care today, you re now more likely than ever to receive the care you need close to home. Throughout 2015, Mayo Clinic Health System staff have continued their deliberate effort to bring more world class care to our patients. We ve welcomed new experts in new fields, expanded access to existing services and invested in the newest medical technologies, all with the goal of delivering you the best care close to home. Our medical centers in Faribault and Owatonna welcomed not only more primary care providers throughout the year but also specialists in several areas including occupational medicine, neurology, physical medicine and rehabilitation, sleep medicine

and cardiology. These provider additions helped us expand access to existing services such as cardiology and allowed us to bring new services closer to home. Mayo Clinic Health System in Faribault s Express Care clinic opened in June and offers immediate care for sore throats, ear infections and other routine illnesses. Why are we focusing so much on bringing more care to our patients? We live in a time of tremendous change in national, regional and local health care. Individuals, employers and our government are working to contain health care costs that simply are not sustainable. Mayo Clinic Health System is not immune to this change. We are working hard to deliver care in a way that manages costs and allows you to spend less time and money traveling. Most of the time, a local health care team, including a nurse, nurse practitioner/physician assistant and doctor in the primary care setting can take care of your needs. When you need specialized care, we re working to make sure we can meet those needs in the community. For the most highly specialized care, we share an electronic health record and other systems with Rochester to ensure any necessary transfer is seamless with the care you ve received at home. All of these transformations from how you access your lab results to where you might see a specialist are designed to provide you convenient access to world-class health care. Our patient satisfaction and quality scores show in this time of tumultuous change in health care, we aren t just surviving we are succeeding in making Faribault, Owatonna and surrounding communities thriving, healthy places to live. Be seen sooner by a member of your physician s care team. Look up your results in Patient Online Services. Send a message to your provider or look up your bill on Patient Online Services. We ve also sought to improve the way we operate in order to keep costs down for patients and to provide consistent, efficient, convenient care across all our sites. Now instead of waiting for an opening with your physician, you can be seen sooner by a member of your physician s care team. Instead of waiting by the phone for test results, you can review your results in Patient Online Services, which also offers prescription refill ordering. All our sites have transitioned to Mayo Clinic Health System s electronic health record, connecting the local teams with the rest of the system. In the next couple of years, all of Mayo Clinic will change to a single electronic health record. Other technologies, like telemedicine where patients can visit with a subspecialist in Rochester using advanced videoconferencing have helped us bring more Mayo knowledge locally. Josh Berndt, P.A. David Ivance, M.D. Small town, big results: New Richland patient benefits from local specialties Two years ago, Eleanor Schlaak of New Richland, Minn., was progressing through physical therapy after a back surgery when Joshua Berndt, Physical Therapist, Mayo Clinic Health System in Waseca, mentioned that she might need care for her knee as well. We started incorporating a few exercises into the therapy for my back, says Schlaak. Next thing I know, he s handing me an appointment card to meet with the surgeon. Schlaak saw David Ivance, M.D., Orthopedic Surgery, at Mayo Clinic Health System in Owatonna, who recommended a conservative approach at first. But late in 2015, Dr. Ivance suggested to Schlaak that the time for a replacement had arrived. She had no cartilage

left; the knee was bone on bone, says Dr. Ivance. Surgery was really the only option that would get her active again. Dr. Ivance performed a knee replacement on Schlaak in early 2016 at Mayo Clinic Health System in Owatonna. It was tough at first, says Schlaak of her post-operative period, but Dr. Ivance is not just a great doctor, he is a caring person, and the staff at the hospital treated me like a queen. After her discharge from the hospital, Schlaak s care came full circle when she again began physical therapy with Josh Berndt at Mayo Clinic Health System in Waseca. Every aspect of Schlaak s care from the therapy appointments that highlighted her knee issues and her initial consultation with Dr. Ivance, to her steroid injections, surgery and therapy for her knee was provided locally. At her six week follow up appointment with Dr. Ivance, Schlaak says her knee was just the starting point for a great conversation. He showed me everything about my knee, but we mostly talked about our families. He s not just a doctor; he wants to get to know who you are. Dr. Ivance finds personal relationships to be the most rewarding part of his job providing care locally. We live in a smaller community, and it s likely that I will see my patients over the course of many years. You get to know the pieces that make up their lives, not just as patients but as people; you get to know their families, find out your kids know their kids it s very enjoyable to help improve quality of life for people in the community. And according to Schlaak, word has gotten around in this small town about Dr. Ivance s excellence as a surgeon and his kind demeanor. Whether it s with a friend in town or at the appointment desk at Mayo Clinic Health System in Owatonna, people talk with me all the time about how happy they are with their surgery results and what a very wonderful man he is. Really, I ve had wonderful people helping me all along the way. Employee Health and Wellness How we re partnering with employers Employee Health and Wellness at Mayo Clinic Health System in Faribault partners with businesses in the Faribault and Owatonna area to deliver exceptional, coordinated services in the areas of occupational health, injury prevention and worksite wellness. A collaborative team led by Kodjo Bossou, M.D., Preventive Medicine, and Jill Fauchald, Employee Health and Wellness coordinator, works with skilled nursing and rehabilitation staff to address the specific health and safety needs of area employers. What sets us apart is that we understand employer needs. The needs of the patient always come first, and in occupational medicine, we understand that employers have needs too, says Fauchald. Our Occupational Medicine experts care for patients and are extremely knowledgeable about business needs like surveillance, regulatory compliance and worker s compensation. We become your

business partner that ensures you have a healthy and productive workforce. Providers have special expertise in Occupational Medicine as well as wellness programs and activities for your employees. Occupational health services focus on promoting employee health and productivity with: Work injury treatment and management Pre-placement physical examinations Regulatory and surveillance examinations Drug and alcohol testing Medical Review Officer services DOT examinations Fit-for-duty examinations Respiratory protection Workplace exposures Wellness programs and services offer businesses on-location services for their employees like: Onsite biometric health screenings Health Education Wellness programs Preventive Care ergonomic assessments, flu shot clinics, Integrative Healing Arts Workshops on self-management and prevention of chronic disease Travel Health Tobacco Cessation individual and group counseling The Employee Health and Wellness team conducts regular employer tours to ensure a greater understanding of the work environment and especially the work performed. Patients are often surprised when they tell me about the particular machine or area in which they work and realize I have actually seen that machine and been onsite, says Dr. Bossou. Community Board Member Perspective Having MCHS in our community means consistent access to high quality health care professionals. It means our local workforce has the chance to be healthier and more productive, which is good for our people and our employers. And more recently, it means that because of the increase in specialists available in Owatonna and Faribault, people in our community don t have to travel to get their health care needs met. Mayo Clinic Health System: Faribault Upgrade Mayo Clinic Health System in Faribault is getting an upgrade. The existing 12,200 square foot clinic building at 300 State Avenue is expanding to add an additional 15,250 square feet of patient care space. Construction began in October 2015 and the facility is well on its way to completion, which will allow more consolidated clinical services and more efficient operations in an integrated medical campus. Employees will move into the new building in late 2016 or early 2017. Along with the new addition, crews are remodeling the existing clinic space, and the renovated and expanded medical campus will provide space for high-tech imaging, laboratory services, and team-based care. The clinic will also expand specialty and surgical services within the Faribault community. I think the first impression people will have of this building better aligns with the quality of care we have been providing in this area for a long time, says Mark Brown, Operations Administrator. It s a convenient location with easy access to hospital services, and we hope it will inspire hope and healing for our patients and staff.

Making a difference... Steele County Free Fair: More than 80 Mayo Clinic Health System employees supported this year s booth at the Steele County Fair, which helped fairgoers register for Patient Online Services and learn about the new care team model. As a not-for-profit organization, Mayo Clinic Health System is committed to the communities it serves. The organization reinvests in its communities and gives back through financial assistance such as charity care and participation in community building activities. The clinics also donated money and items to support many efforts and events in partnership with local schools, businesses, and many community non-profit organizations. Community Walking Program: The community walking program promotes long-term health benefits through physical activity. Mayo Clinic Health System partners with Allina Hospital, Steele County Public Health, and Owatonna Parks and Recreation, and the program is open to the community.

Have a Safe Summer Program: Founded in 2007, the Have a Safe Summer program is a community partnership with local safety agencies including police and fire departments, along with bike safety and summer youth programs to educate youth on making healthy and safe choices. The program hosts an annual Safety Fair at the Steele County Fairgrounds, and the landing of Mayo One, a medical transport helicopter, attracts many community members. A Mayo Clinic Health System audiologist and dietitian were on-site to discuss hearing and technology and healthy eating choices. Relay for Life: Mayo Clinic Health System is a primary sponsor of the walk, and more than 50 staff walked for the cause in 2015. Sports Screenings: Mayo Clinic Health System offers preparticipation athletic screening for athletes in seventh through 12th grades in Steele County. Staff has volunteered their time to provide free screenings in Owatonna, Faribault, Medford and Blooming Prairie for over 15 years. Women s Health Day: The 13th annual Women s Health Day at the Owatonna Country Club attracted almost 200 guests to learn about disease prevention and health improvement and to shop local vendors. Diane Wallner, M.D., Family Medicine, and Ashleigh Noble, Internal Medicine, who works as a registered dietitian, shared educational messages on emotional wellness and managing stress.

Mayo Clinic selected Epic to replace its three EHRs in use today. With staff across the Mayo Clinic institution working together on a common system, you can expect accelerated innovation, enhanced services and a better patient experience.
