JOB REQUIREMENTS Key Result Area 1 First Contact & Engagement Respond to contact/referral from people affected by multiple sclerosis from internal and external sources Provide information and advice that will build capability /health literacy for the individual to live their life and to navigate any next steps as independent as possible. Explore and guide the individual to reflect on options, choices and decisions, how they want to live their life and how current situations fit for them within the circumstance and life. Be able to provide detailed symptom management and advice, e.g. benefits and entitlements i.e. Caps or NDIS Where appropriate and agreed with the person affected by multiple sclerosis, support the referral to relevant services. All contact is responded to within agreed service levels. To achieve 4 or above in a net promoter score index. Assessment, planning and referral templates are used. 80% of direct service delivery is achieved. Page 2 November 2017

Respond to contact from health practitioners seeking clinical advice. Key Result Area 2 Relationship Management Work with external partners and suppliers to ensure maximum value for funding achieved. i.e. consulting urologist Develop strong rapport with people affected by multiple sclerosis in order to gain their trust and allow the provision of assistance. Develop strong and collaborative working relationships with relevant referrers and agencies in order to promote the open sharing of information conducive to positive outcomes. Feedback reporting mechanisms reflect a positive outcome is achieved. A relevant network of support, internally and externally, is established and appropriately maintained to the benefit of the customer. The number of networks established that demonstrate participant outcomes. Develop strong and collaborative working relationships with key external stakeholders including Funding body other key peak organisations and service provider s i.e. Continence foundation of Australia. Work with internal staff to ensure the smooth running of the urology clinic. Key Result Area 3 Administration Create and update individualized case files on relevant operating system for all customers in line with MS protocols. Complete all internal and external client referral requirements relating to each intervention. Budget is achieved. Complete a range of internal and external reports relating to clients including for case allocation, case management statistics, and feedback summaries and quarterly & yearly outcomes reports for funding bodies Complete a range of other administrative duties for the efficient running of the service including statistics reports, referral letters, goals plans etc. Create and update educational material suitable for presentations and training of internal and external partners. Quarterly and Yearly reports to Funding Bodies is completed and approved. Education is provided to a range of partners in a series of delivery methods. C. D. Selection Criteria Essential: A Degree in Nursing, registered Nurse, holds current registration with Australian Health Professional Regulation Agency (AHPRA) Graduate Certificate in Continence Promotion & Management Minimum 2-3 years experience in providing clinical services, support and advice to people with chronic neurological conditions, including MS. Proven track record for delivering solutions valued by clients. Ability to develop effective working relationships and network Demonstrated ability to achieve results through strong influence and consultative skills. Pragmatic, assertive, organised and demonstrates sound judgment. Resilience and proven ability to adapt style and approach to suit varying situations. High levels of computer literacy in using a range of Office computer packages & Salesforce (training will be provided for Salesforce) Desirable Current Drivers Licence Member of the Continence Foundation of Australia Page 5 November 2017
