Date: November 2017 We are recruiting a Nurse Practitioner due to retirement to join our successful, innovative and happy practice. The post holder will be primarily responsible for the delivery of nurse prescribing care to the practice population. Successful candidates will be NMC registered, a Nurse Prescriber and possess excellent clinical, communication and IT skills. The practice itself is located in close proximity to Daventry Town centre in a modern building adjoining Danetre Hospital (a community hospital which opened in September 2006) offering a wide range of services and delivering a high standard of care. Also attached to the Practice operating independently is Boots Chemist and a Children s Nursery. The site has free car parking. The ideal candidate will possess experience & knowledge of treating minor illness, general nursing services, chronic disease management, family planning, and childhood immunisation; however training will also be available to the right candidate. Any increase over the last ten years has been due mainly to; natural change (more births than deaths); building development within Daventry itself and more recently migration from the rest of the UK and overseas. The boundaries of the Practice show we are large in terms of geographical size, with the majority of the land classified as rural. The practice itself is located in close proximity to Daventry Town centre in a modern building adjoining Danetre Hospital (a community hospital which opened in September 2006) offering a wide range of services and delivering a high standard of care. Also attached to the Practice operating independently is Boots Chemist. The practice currently has 4 GP Partners, 3 salaried GP s, and 3 x Advanced Nurse Practitioners. Resources assigned to the Clinical Team are: 2 x Diabetic Nurses: 2 x Practice Nurse: 3 x Health Care Assistant: 1 x Research Nurse: and 1 x Prescribing Technician time. Attached staff include; Health Visitors, Midwife and District Nurses. ADDITIONAL INFORMATION The post holder holds a position of importance, being the crucial link between the doctors, staff and patients. Full training will be given in all aspects of the job. Telephone calls are monitored and used for training purposes and to accurately record patient and GP information You will be required to wear a name badge, which will be provided for you. You will be expected to attend regular staff meetings. The meetings provide an opportunity to discuss matters arising from the job and can be used for in-house training. They will demonstrate safe, clinical decision-making and expert care for patients within the general practice. They will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. In order to work at this level, NMC requirements for advanced practice must be met. PURPOSE To deliver a high standard of patient care as a Nurse Practitioner (NP) in general practice, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base working who works collaboratively to meet the needs of patients, supporting the delivery of policy and procedures whilst managing their own workload to deliver the practice priorities and vision.
KEY RESPONSIBILITIES To manage a clinical caseload, dealing with presenting patient s needs in a primary care setting To develop the nursing services offered by the practice To provide clinical care to patients, this will include consultation sessions, telephone triage, clinical administration, medicals, reports, home visits, surgery meetings etc. To see and treat patients autonomously with undifferentiated/undiagnosed problems within competency & NMC Guidelines. Provide clinical care in line with agreed local and national protocols or guidance. Identify medical emergencies and take the appropriate action. Perform advanced skills within competency & NMC Guideline and safely use equipment to aid physical examinations including for example; Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs. Recording clear and contemporaneous consultation notes to agreed Practice standards and NMC guidelines using SystmOne for all patient information. Read code appropriately and collect data for audit purposes as agreed. Implement and support vaccination and immunisation programmes for both children and adults. To communicate effectively with patients and carers, recognizing the need to alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating. As an independent prescriber, prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence based practice and national and practice protocols and within competency and NMC guidelines. To work with patients in order to support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed or over the counter medication on medication regimes, side-effects and interactions. To encourage health promotion, education and self care for all patients and advice them of other available services. The post holder will work within their individual competency and NMC guidelines and in accordance with local CQC policies and procedures.
