JOB DESCRIPTION JOB TITLE: GRADE: International Fellow in Medicine Junior Clinical Fellow (JCF) HOURS: 40 Hours (Band 1A) RESPONSIBLE TO: ACCOUNTABLE TO: Dr Joble Joseph, Clinical Director for Medicine Dan Gibbs, Interim Divisional Manager, Trauma, Emergency and Medicine (TEaM) Posts are at the Princess Royal University Hospital (BR6 8ND) and Orpington Hospital working within Medicine. JCF postholder will have rotations through the Acute Medicine General Internal Medicine wards gaining experience of the services. Whilst on the Acute Medicine rota, the focus will be on acute medical take and management of acutely unwell patients within the first 48 hours of admission. If the patient requires a hospital stay of longer than 48 hours then they will be transferred to a post-amu ward. This post would be suitable for any trainee who has, or are on course to, successfully complete a 2-year Foundation School programme or has achieved equivalent competencies. Ideally trainees will have an up to date certification for Adult Life Support training. Although this appointment is not accredited for training, the appointee will be expected to participate fully in the educational and training programmes within each department and the hospital. The appointee will have a named clinical/ education supervisor. Annual leave and study leave is commensurate with other post foundation doctors. The experience and opportunities offered would be excellent experience for applicants planning a career in medicine. Previous postholders have been successful in gaining entry to training programmes upon completion as well as completing their Royal College membership exams.

PERSON SPECIFICATION ESSENTIAL EDUCATION/QUALIFICATIONS Eligible for full registration with the GMC. MB BS (or equivalent). PREVIOUS EXPERIENCE Experience in general medicine. SKILLS/KNOWLEDGE/ABILITY Knowledge and experience of general medicine. Successful completion of a Foundation School Training Programme in the UK (or able to prove equivalent competencies). Competent to work without direct supervision where appropriate. Clear, logical thinking showing an analytical/ scientific approach. Ability to prioritise clinical need. KING S COLLEGE HOSPITAL NHS FOUNDATION TRUST EDUCATION PROGRAMME FIRM/SPECIALTY GRADE YOUR POSTGRADUATE DEAN/ CLINICAL TUTOR Medicine Junior Clinical Fellow Mr T.J. Lasoye Although these posts are not approved for training they will provide excellent experience for general professional training at Core Medical Training Level. All appointees will have a designated clinical/educational supervisor who they will be expected to meet regularly to support their professional development. PART 1: HOSPITAL POSTGRADUATE FACILITIES 1. The PGMDE Department is based within the Education Centre at the Princess Royal University Hospital. The Education Centre also hosts a library and a restaurant. In addition there is the Education Annex which has a Simulation Suite. Both the Simulation Suite and Education Centre run a range of courses, teaching sessions, clinical skills practice and Simulation courses. 2. Weekly Grand Round takes place every Friday from 1pm 2pm in the Education Centre with lunch provided. 3. Appointees will be welcome at the Core Medical Training weekly sessions Wednesday or Thursday 1-2pm and MRCP Paces teaching sessions. 4. The Teaching the Teachers Course run across the Trust would be available to appointees. 5. Study Leave arrangements and funding are available and advice and guidance is available through the PGMDE department. PRINCESS ROYAL UNIVERSITY HOSPITAL (PRUH) ACUTE MEDICINE There is a 56 bedded acute medical unit comprising of two wards (EAU and M9) that take all acute medical admissions. It is run by eight Acute Medicine Consultant Physicians each supported by junior teams. The majority of medical admissions (either from primary care or the emergency department) are admitted through this unit. There are daily Consultant led ward rounds for all patients. Each ward is supported by multidisciplinary teams of nurses, therapists, pharmacists, clinical administrators and social workers. There is an Ambulatory care clinic which facilitates early discharges and provides an opportunity to manage complex patients in an outpatient setting. This is a valuable opportunity to provide ongoing continuity and learn about the natural history of the conditions that present to the PRUH. The Acute geriatric service comprises of a 40 bedded ward. This is run by three Consultants who provide an in-reach service onto the acute medical unit. Patients with a length of stay of less than 5 days are transferred to this ward. There is daily in-reach from cardiology, gastroenterology and respiratory teams. There will also be opportunities to acquire skills in practical procedures including ultrasound skills and focused echocardiography under the supervisor of the Acute Medicine Consultants. It is a dynamic unit with extensive opportunity to learn from Acute physicians with specialist interests e.g. Renal, Gastroenterology, Cardiology and Endocrine/Diabetes. There is a weekly teaching session and development of professional portfolio whilst under close supervision by Consultants. Patients who are identified as requiring an admission of longer than 48 hours will be moved to one of the post-amu wards in either General Internal Medicine or to one of the specialist clinical gerontology wards. We also have close working relationships with acute stroke care, neurology and critical care.

HYPER ACUTE STROKE UNIT (HASU) The post holder will contribute to the operation of the 14 bedded HASU as part of a multidisciplinary team of doctors, nurses and therapists, and also provide out of hours cover for stroke. Responsibilities will involve management of HASU patients, and attendance for acute stroke calls in A&E with a Band 6 stroke nurse and a stroke consultant. Ward rounds are carried out each day, including weekends, lead by a stroke consultant. This HASU attachment is an excellent opportunity to gain extensive experience in various aspects of hyper acute stroke care, including brain imaging and delivery of thrombolysis. GENERAL INTERNAL MEDICINE These wards are staffed by a team of Consultants and junior medical staff (from F1 to specialist register level) for the ongoing care of patients who require a hospital stay of longer than 48 hours. Multidisciplinary team working with nurses, therapists and other professions allied to medicine is expected of all doctors within General Medicine. Each day starts with a multidisciplinary board round to check and confirm expected discharge arrangements and identify acute clinical need. There are ward Clinical Governance and Multidisciplinary Performance Meetings to ensure quality of patient care and the appointees would be expected to participate in these processes. These wards encompass the following specialties Cardiology, Gastroenterology, Respiratory, Stroke and Gerontology. BROCKLEHURST UNIT, ORPINGTON HOSPITAL This is a new development of two clinical areas (38 beds/chairs) on the Orpington Hospital site is in progress. This facility will form part of the service offered to frail older people (Integrated Health Care) assisting in admission avoidance (including step-up beds) and a step down service, for people requiring a further period of rehabilitation/recovery once medical stable. The medical support to this will be from Consultants in Geriatric Medicine, and may also be supported though a combined approach with GPs with a specialist interest in Older People. This aims to provide seamlessly integrated, innovative care for frail patients. GENERAL The postholder has a general duty of care for their own health, safety and wellbeing and that of work colleagues, visitors and patients within the hospital, in addition to any specific risk management or clinical governance accountabilities associated with this post. INFECTION CONTROL STATEMENT The postholder has an important responsibility for and contribution to make to infection control and must be familiar with the infection control and hygiene requirements of this role. These requirements are set out in the National Code of Practice on Infection Control and in local policies and procedures which will be made clear during your induction and subsequent refresher training. These standards must be strictly complied with at all times. All medical staff are expected to comply with and follow the Trust s infection control policies, which includes appropriate antibiotic use following the Trust s antibiotic formulary and observance of appropriate infection control procedures at all times. Appointees will be trained and assessed for IV line competencies. TRAINING OPPORTUNITIES AND SUPERVISION Although these appointments are not accredited for training the appointees will be expected to participate fully in the educational and training programmes within each department and the hospital. Appointees will have a named clinical/educational supervisor. ANNUAL LEAVE Annual leave (27 days per annum) and study leave (30 days per annum) is commensurate with other post foundation doctors; the rota has been redesigned and fixed leave is in place.
