Heart Care Coordinator - ACT Division Permanent, Part Time Title: Heart Care Coordinator Team: Health Position type: 0.4 FTE Reports to: Health Director Company: ACT Division Hours per week: 15hours Grade: 6 Location: Phillip, Canberra Effective date: November 2014 ORGANISATIONAL CONTEXT The Heart Care Co-ordinator role works within the ACT Division, working closely with the Health Director ACT on strengthening our engagement with the primary, acute and sub-acute health sectors, and to inform our advocacy agenda regarding improvements required in the health sector to enable earlier detection and better management of cardiovascular disease. Position Purpose: To assist the Heart Foundation to identify and support strategies in ACT that will: Improve the earlier detection and better management of cardiovascular disease risk in primary care Improve the acute and secondary care for patients experiencing cardiovascular conditions, specifically acute coronary syndrome and heart failure. Develop a network to support ACT CVD related researchers and provide for ACT researchers to showcase their work. The position contributes to the development and implementation of the strategies and actions under the Heart Care and Equity Goals of the ACT Strategic Plan. Page 1 of 7

In consultation with the Health Director, manage the implementation of activities in ACT to extend the reach of the following areas of work under the Heart Care Goal: Absolute Risk Warning Signs and Emergency Care Acute Coronary Syndromes (ACS) - includes acute and secondary care (cardiac rehabilitation) for ACS, as well as improving management of chronic heart failure Equity issues (as identified under the Equity Goal) must also be considered when delivering activities under the Heart Care Goal. Ensure compliance with Division s budget. Key milestones and Key Performance Indicators for Heart Care projects under the Operational Plan are achieved. All identified activities successfully implemented. Improving trends for ACT in relation to relevant indicators in the HeartWatch survey which relate to the Heart Care Goal. Financial sign off up to $100. After consultation with the Health Director and approval of CEO obtained, explore opportunities, and when directed prepare submissions for external funding in line with the Heart Care Goal of For all Hearts and the ACT Operational Plan. Coordinate relationships Develop strong relationships with all health professional groups working in the acute, subacute and primary care sectors to deliver the CVH themes in the ACT Strategic Plan. Ensure all ACT health professionals have timely access to and professional education on Heart Foundation endorsed guidelines. Relevant contacts made, relationships developed through regular communication & outcomes reported against strategic objectives. Improvement in all indicators from the primary care workforce in the Health Professional tracker survey. Increase awareness amongst health professionals of Heart Foundation guidelines and position statements. Communication channels identified and used. Increased alignment of ACT-developed policies, clinical pathways, data collection and programs to Heart Foundation policy, particularly as they relate to Absolute Risk, Emergency Care, Acute Coronary Syndrome, secondary prevention (cardiac rehabilitation) and chronic heart failure. The prevention and management practices in ACT with regard to cardiovascular health position align with Heart Foundations. Advocacy Participate effectively with other team members Support the positioning of the Heart Foundation as the peak agency in the area of CVH in ACT. Feedback from team members as appropriate Feedback from Health Director, stakeholders, HeartWatch survey and Health Professional tracker survey. Relevant media opportunities taken (in consultation with the Marketing and Communications Officer) to promote messages related to Heart Care Goal. Leverage relationships and alliances to further the Heart Care Goals in the CVH Operational Plan, ensuring a focus on equity. In consultation with the Health Director represent the Heart Foundation on interagency, government and other joint initiatives consistent with the ACT Strategic Plan. Concise, relevant and effective outcomes reported on time. Compliance When requested by the Health Director, act as the CVH Team s representative and/or spokesperson on cardiovascular health matters. Model appropriate behaviour and support compliance with all internal policies. Ensure compliance with all relevant state and federal legislation e.g. Workplace Health and Safety, fundraising acts. Accurate representation of Heart Foundation policy position. Competent completion of Heart Foundation Induction including Workplace Health and Safety and Emergency procedures. CHALLENGES: Influencing external stakeholders to adopt Heart Care related policies and recommendations Ensuring an equity focus is given to the work being undertaken under the Heart Care Goal Provide accurate and timely answers and advice to inform our advocacy and communications work as it relates to Heart Care and Equity Goals KEY COMMUNICATIONS: Internally: Provide relevant reports and presentations as required to the CVH team (and where relevant to the broader ACT Division team) about the work being undertaken under the Heart Care Goal. Participate in relevant Heart Foundation committees and/or working groups as agreed by the Health Director, to assist with the development and achievement of Heart Care Goal activities under the For all Hearts strategy. Externally: Identify, establish and maintain effective relationships with potential national and state level strategic partners. Represent the Heart Foundation on inter-agency, government and other joint initiatives consistent with For all Hearts and the ACT CVH Operational Plan. KNOWLEDGE, SKILLS & EXPERIENCE Essential Requirements: A tertiary qualification in a health related discipline (e.g. nursing, allied health, health promotion, public health) clinical experience in primary, acute or sub-acute care sector with demonstrated success Awareness of critical issues and current developments in cardiovascular health, particularly in relation to the principles of prevention and management of cardiovascular disease, and the principles of Acute Coronary Syndrome management Sound understanding of state level health structures, processes and networks Demonstrable success in negotiating and building partnerships with key stakeholders Well developed interpersonal skills, including proven communication, collaboration and negotiation skills Excellent writing skills (including reports and/or submissions) with attention to detail Current ACT Drivers licence Ability to undertake some interstate travel Desirable Requirements: A postgraduate qualification in public/population health or a related discipline Clinical experience in primary prevention of cardiovascular disease A postgraduate qualification in health or a related discipline Demonstrated experience in attracting external funding through the preparation of submissions and tenders Page 5 of 7
