Lxtn Preschl Centre Medical Cnditins Plicy Purpse Medical cnditins include, but are nt limited t asthma, diabetes r a diagnsis that a child is at risk f anaphylaxis. In many cases these can be life threatening. Our preschl is cmmitted t a planned apprach t the management f medical cnditins t ensure the safety and well-being f all children at this service. Our preschl is als cmmitted t ensuring ur educatrs are equipped with the knwledge and skills t manage situatins t ensure all children receive the highest level f care and t ensure their needs are cnsidered at all times. Gals: Our preschl will minimise the risks arund medical cnditins f children by: Cllabrating with families f children with diagnsed medical cnditins t develp a Health Supprt Plan and t ensure they visit their General Practitiner r relevant health prfessinal t develp a Health Care Plan fr their child Infrming all educatrs, f all children diagnsed with a medical cnditin and the Health Care Plan prcedures fr these Ensuring all children with diagnsed medical cnditins have a current Health Care Plan that is accessible t all educatrs; and Ensuring all educatrs are adequately trained in the administratin f emergency medicatin. Practices: Enrlment f children int the preschl The Nminated Supervisr will: Ensure that any parent with a child enrlled at the preschl that has a specific health care need, allergy r ther relevant medical cnditin is prvided with a cpy f this Medical Cnditins plicy; Infrm parents f the requirement t prvide the preschl with a Health Care Plan f their child s cnditin; Plans are divided int tw areas: Persnal Care Plans (fr daily living supprt) and Health Care Plans (fr emergency respnse/first aid, medicatin, health cnditins and health related persnal care issues). Befre staff can assist, parents/caregivers must prvide written infrmatin frm their Dctr/treating health care prfessinal that utlines specific care needs in the apprpriate plan frmat. These Care Plan frms ensure that the Centre has infrmatin frm the treating health prfessinal relevant t the child s health, wellbeing, attendance, learning and care at preschl. These plans include medicatin, first aid, specific health infrmatin, e.g. asthma, seizure/epilepsy plan, diabetes, anaphylaxis. Staff can wrk with families t plan supprt fr children 2 P a g e

This supprt may be required as there are individual first aid requirements ther than basic first aid respnse, r the child has a predictable need fr additinal supprt with daily living tasks, r there is additinal need fr supervisin fr health related safety. Other frms can als include Medical Infrmatin, fr general health care r thse withut specific frms, and General Health Infrmatin. Ensure that all educatrs and vlunteers are aware f each individual child s Health Care Plan, that they can identify the child, and knw the lcatin f the child s medicatin; and If relevant, ensure that practices and prcedures ensuring that the child des nt attend the service withut medicatin prescribed by the child s medical practitiner in relatin t the child s specific health care need, allergy r relevant medical cnditin are develped and implemented; All frms will be kept in the Medical/Health Recrd Frms flder in the tall white cupbard adjacent t the staff tilet in the Art Preparatin area behind the child safe gate. An additinal cpy in frnt sleeve f individual child s medicatin bag als stred in white cupbard. Children s phtgraphs and a brief descriptin f their medical cnditin will be displayed n the inside f the cupbard, in the rll bk and n the back f the main ffice dr t ensure staff are aware. Ensure that educatrs are adequately trained in prcedures cntained in the Care Plans; and Infrm ther families enrlled at the centre f the need t prhibit any items which may present a hazard t children with diagnsed medical cnditins. Respnsibilities f Parents/Caregivers: Cmplete the enrlment frm with accurate health/medical infrmatin. Ensure Lxtn Preschl Centre has up-t-date infrmatin with parent/caregiver emergency cntact details and at least 2 ther peple fr back up emergency cntact. Request the relevant health care plan frms as needed fr the child. Have the Health Care Plan frms cmpleted and signed by the treating medical practitiner, sign them as parent/caregiver, and return t the preschl. Wrk with the Nminated Supervisr t cmplete a health supprt plan fr the child if necessary. Ensure all medicatin is delivered t the preschl as needed (in a cntainer with an riginal pharmacy label), and that a medicatin plan is cmpleted and signed by the dctr and signed by the parent/caregiver. Update the medical infrmatin as necessary with frms cmpleted and signed by the treating medical practitiner. Cmmunicate any changes in health-related issues with staff immediately. Prvide and maintain yur child s health equipment (e.g. spacer). 3 P a g e

Ensure Health Supprt Plans are develped, implemented, mnitred and rutinely reviewed. Identify and ensure access t the training required t meet the rutine and emergency health supprt needs f children. Ensure WH&S prcesses are inclusive f Health Supprt Planning requirements. Ensure all Health Supprt Plans and Health Care Plans are accessible t all educatrs. Ensure that all plans are current and kept up t date. Infrm families f their respnsibility fr updating their child s Health Care Plan/prviding a new Plan as necessary nce a term in the preschl newsletters. Educatrs will: Ensure they are aware f enrlled children with medical cnditins and be familiar with the Health Supprt and Health Care Plans f each child diagnsed with a medical cnditin. Medical Management Cmmunicatin Plan Our preschl will implement the fllwing cmmunicatins plan t ensure that relevant educatrs and vlunteers are: Infrmed abut the Medical Cnditins Plicy Easily able t identify a child with medical cnditins Are aware f the requirements f any Health Care Plans and Health Supprt Plans Aware f the lcatin f each child s medicatin Updated n the child s treatment alng with any regulatry changes that may affect practices fr specific medical cnditins. 4 P a g e

Lxtn Preschl Centre Medical Cnditins Plicy MEDICAL MANAGEMENT COMMUNICATION PLAN At the time f enrlment, parents are asked t cmplete details relating t their child s medical cnditin n the enrlment frm. Families will be advised as t whether they need t visit their General Practitiner r apprpriate medical prvider t gain Health Care Plans detailing the care f their child s medical cnditin. Frms will be prvided by the preschl if required. This Plan shuld: have supprting dcumentatin if apprpriate include a pht f the child if relevant, state what triggers the allergy r medical cnditin first aid needed cntact details f the dctr wh signed the plan state when the Plan shuld be reviewed. Cpies f the plan shuld be kept with the child s medicatin and als accmpany them n any excursins. Nte parents are respnsible fr updating their child s Health Care Plan/prviding a new Plan as necessary and will be regularly reminded by the preschl nce a term in the preschl newsletters. Any new infrmatin will be attached t the Enrlment Frm and kept n file at the preschl. Educatrs will ensure infrmatin that is displayed abut a child s medical cnditins is updated. A cpy f each child s Health Care and Health Supprt Plans will be kept attached t their enrlment frm, in the rll bk and in the Student Health Recrd flder lcated with the medicatins in the white cupbard near the Adult Tilet. A termly summary f all student medical infrmatin is printed via the Early Years System and attached t the inside dr f the medicatins cupbard, in the rll bk and in the Student Health Recrd flder. Children s phtgraphs and a brief descriptin f their medical cnditin will be displayed n the inside f the cupbard, in the rll bk and n the back f the main ffice dr t ensure staff are aware. 5 P a g e

Relief educatrs are asked t familiarize themselves with the Student Health Recrds prir t cmmencement f duty fr the day. Where a child has been diagnsed at risk f anaphylaxis, a ntice stating this must be displayed at the preschl s it is clearly visible frm the main entrance. (inside lckable ntice bard lcated by student lckers). The privacy and cnfidentiality f the child will be maintained at all times and the public ntice will nt name the child. The Allergy Aware (and Anaphylaxis) checklist fr Educatin and Children s Services will be implemented as required.
