KEY TASKS 2. Management, co-ordination and evaluation of patientcentred care ensuring that the patient receives the right care from the appropriate service in the most clinically safe environment in a timely manner by; Be the link person and ensure the appropriate escalation to other areas such as radiology and ward if necessary and DNM, ongoing communication is vital throughout the shift. Work closely with flow specialist and follow up on ED escalation Work as a flexible team member towards achieving the shorter stay in ED targets The flow co-ordinator will be based between ED and ADU and have an overview of both ED and ADU capacity and flow issues Promote smooth seamless flow of patients through the ED /ADU to admission or discharge, by progress-chasing, updating and liaising with all staff Early identification of capacity problems Ensure proactive rather than reactive response Free nurses time by carrying out appropriate administrative tasks Improve the accuracy of audit data by the timely entry of key times and points of referral Report on breeches to the target with evidence based real time information to allow areas for further improvements to be identified Be the link person and ensure the appropriate escalation to other areas such as radiology and ward if necessary and DNM, ongoing communication is vital throughout the shift.
