Standard Position Requirements Required to hold and maintain a current Drivers Licence. Required to hold and maintain a Working with Vulnerable People Child Related Activities Registration (Tasmania). Required to provide a satisfactory Schedule 1 National Police Check Required to undertake Anglicare s Safeguarding Children and Abuse Prevention Programs prior to working directly with children or vulnerable people. Required to undergo an employment health assessment which allows Anglicare to be aware of potential and actual health issues for employees and (if appropriate) to work with you to ensure we manage these appropriately (e.g. tailoring the work environment to cater for specific requirements). Conform to Anglicare s conditions of employment, Code of Conduct, Confidentiality, Anti- Discrimination, Bullying and Harassment policies, Work Health and Safety policies including signing a new Personnel Declaration. Required to comply with Anglicare s Smoke Free Work Site policy. This position is mandated by law to report all current concerns about the safety, welfare and wellbeing of a child. Report any suspected abuse or neglect of adult with a disability or ageing person. Primary Tasks Case management Build and maintain effective working relationships with consumers, families and communities. Liaise with ACAT and other referral agencies to ensure that aged care packages are allocated in a timely manner. Undertake comprehensive assessments using a person centred approach. Provide direct nursing care (including the administration of medications) and provide case management to clients approved for high level package care. Comply with organisational Policies and Procedures and contribute to their development in relation to clinical care delivery. Work with the consumers and their families to develop a care plan that has an emphasis on wellness and reablement as well as the maintenance of independence. Liaise with health and other support providers to gather and verify relevant information to facilitate care plan development to optimise care delivery outcomes. Provide consumers and their families with information to assist them to make an informed choice on the types of services that will support their goals. Assist individuals and their families to establish and maintain their support networks. Assist consumers and their families to understand and manage their individual budget with the level of control that they wish to exercise. Work in partnership with regional care coordinators to ensure that processes deliver excellent outcomes for consumers and their families. Liaise and work cooperatively with Anglicare s Contact Centre, workforce coordination team, finance and administration and human resource teams. Engage with Human Resources and Anglicare Disability and Community Aged Care Services (DCACS) to ensure grievances and complaints are addressed in a timely manner. Ensure all activities comply with legislation, Organisational policies and ethical practice Community Development and Capacity Building Develops partnerships with individuals, families, local organisations and the broader community to build a more inclusive community. Develops a sound understanding of the key issues for older people to inform planning and policy development. Administration and Information Management Organise and maintain administrative records and contribute to the effective operation of case management services. Administer all aspects of direct funding in accordance with agreed polices and accountability benchmarks, including funding applications, development and review of funding plans, payments and acquittals. Provide data for reporting purposes and respond to requests for information. Assist with applications for funding. High quality record keeping, including competency with IT systems. Continuous Improvement Participate and contribute to team processes. Comply with and contribute to quality improvement programs such as the Community Care Common Standards and other program activities to meet service, legislative and accreditation standards. Maintain ongoing education and training in accordance with the Nursing and Midwifery Board of Australia. Provide training and supervision to support staff as required. Ability to respond and prioritise competing and often urgent requests in a calm and efficient manner while also maintaining high work standards and accuracy. Strong communication and interpersonal skills. Ability to adapt quickly to a changing environment. Ability to manage service delivery performance including sub-contracting arrangements. Maintain professional boundaries when delivering support to consumers. High level of competency in applying assessment tools to identify support needs. 2.
