The Facilitator will work with Ability Links staff and people with disability and supporters over 12 months to deliver training and support on SDM. The Facilitator will develop knowledge of both SDM and Ability Links, and make recommendations on the future application of SDM within the Ability Links initiative. Supported Decision Making supports people with disability to make decisions for themselves and increases their choice and control over their own life. SDM offers a framework for improving capacity of families, carers and service providers to move towards a person centred approach. The Facilitator will contribute to the development of the principles and practice in this area of work with support, training and assistance from ADHC, St Vincent de Paul Society NSW and other Supported Decision Making facilitators. St Vincent de Paul Society The St Vincent de Paul Society is a member and volunteer based organisation that has been assisting people experiencing disadvantage in NSW for more than 130 years. The Society was founded by a 20 year old man named Frederic Ozanam in 1833, who with a group of friends, wanted to make a difference to the poverty and disadvantage that he saw around him in post- Revolution France. Leading by example and with boundless energy, Frederic started what is now a worldwide movement in just 20 short years. The St Vincent de Paul Society NSW has around 16,000 members and volunteers and 1,400 staff. Our local member networks, referred to as Conferences, are present in over 420 communities across NSW. Our members and volunteers provide assistance to people experiencing disadvantage including food parcels and vouchers, financial assistance, assistance with energy bills and other debt, budget counselling Every day in NSW the Society helps thousands of people through Home Visitation, Hospital Visitation, Prison Visitation, Homeless Services for Men, Women and families, Migrants & Refugee Assistance, Support for those living with a mental illness, Supported Employment Services for People with an intellectual and other disabilities, Vinnies Centres, Overseas Relief, Disaster Recovery, Budget Counselling and Youth Programs.

Supported Decision Making Ability Links Facilitator Position Description August 2014 and the provision of other material items such as furniture, clothing and bedding and any other household items. Importantly, our members also provide vital emotional support and referral services as needed. The St Vincent de Paul Society is a leading provider of community support services and has close to 100 Special Works across the State in the areas of homelessness, mental health, disability, financial counselling, supported accommodation, food provision, refugees and migrants and addiction counselling. Our work is funded by all levels of government, the generous support we receive from the community and our 250 Retail Centres across NSW. Service Overview As disability services move towards self-directed services and individualised funding, there is a need to build the capacity of people with a disability to make informed decisions. Supported Decision Making (SDM) involves a person with disability and one or more trusted family members, friends or supporters. These supporters work with the person to enable them to make their own decisions about life. Support could be to find information, help the person weigh up choices or communicate their decision. After the completion of an initial pilot delivered by ADHC and The Public Guardian and NSW Trustee and Guardian, ADHC have provided funding for an extension of this work for a 12 month period to the St Vincent de Paul Society and others. This position works within the Ability Links team. St Vincent de Paul Society NSW is funded by the NSW Government s Ageing, Disability and Home Care department to deliver Ability Links in the Metro South region, in partnership with Settlement Services International. Ability Links NSW (ALNSW) is a key foundation in the NSW Government s plan to reshape and improve the way people with disability, their families and carers in NSW are supported by placing them at the centre of decision-making. ALNSW is one of a number of decision-support resources available to assist people with disability. ALNSW works within local communities to assist people with disability, their families and carers to be valued and active members of their community. ALNSW works with communities to facilitate social and economic inclusion of people with disability. This is a new service, and forms part of the NSW preparation for the National Disability Insurance Scheme. Duties and Responsibilities 1. Gain knowledge and expertise in Supported Decision Making (SDM) through formal training opportunities and mentoring, and facilitating SDM implementation 2. Build the capacity of a select group of Ability Links staff and support them to facilitate SDM 3. Identify and facilitate SDM with people with disability, their families, friends and carers 4. Provide support, information, education and guidance about SDM to people with disability, families, friends and carers, the supporter of the person with a disability participating in SDM, and Ability Links staff 5. Develop peer relationships with other SDM facilitators (within St Vincent de Paul Society and other agencies) to enable collaborative and supportive delivery of SDM 6. Regularly report to the Ability Links Manager on the progress of SDM implementation, and provide guidance on effective practice of SDM within Ability Links 7. Participate in the development of data collection systems to meet SDM and organisational requirements. 8. Document lessons, approaches, resources and analyse trends in implementation of SDM in Ability Links throughout the year. 2
