1.0 Information about NHS Eastern Cheshire Clinical Commissioning Group NHS Eastern Cheshire Clinical Commissioning Group is a membership organisation of 23 GP Practices working within five town based locality peer groups (Figure One). The CCG became a statutory NHS Body on 1 April 2013. Figure One GP Practices and locality peer groups in Eastern Cheshire Our five localities, known as General Practice Locality Peer Groups, are: Alderley Edge, Chelford, Handforth, and Wilmslow Bollington, Disley, and Poynton Congleton and Holmes Chapel Knutsford Macclesfield The main purpose of the CCG is to commission (buy) the highest quality of health care services within available funds, and monitor the quality of these services. We are responsible for commissioning health services to meet all the reasonable requirements of our local population, with the exception of certain services commissioned directly by NHS England, health improvement services commissioned by Cheshire East Council, and health protection and promotion services provided by Public Health England. Our main commissioning responsibilities include: elective hospital care rehabilitation care urgent and emergency care, including GP Out of Hours and NHS 111 most community health services Page 3 of 17

mental health and learning disability services prescribing and medicine optimisation emergency and patient transport ambulance services NHS continuing healthcare and NHS funded nursing care. We also have the responsibility for commissioning emergency and urgent care services for the population within our boundaries as well as for commissioning services for any unregistered patients who live in our area. A list of commissioning functions and duties for CCGs can be viewed at: http://www.england.nhs.uk/wp-content/uploads/2013/03/a-functions-ccgs.pdf From 1 April 2015, the CCG will start to undertake joint commissioning arrangements with NHS England around primary medical services, which opens up an exciting but challenging new chapter in the CCGs history. The CCG s full statutory responsibilities are detailed within its constitution. The main responsibilities include: upholding the NHS Constitution, 1 CCG Constitution 2 and governance standards quality assurance and quality improvement of commissioned services quality improvement of GP services in partnership with NHS England safeguarding children and vulnerable adults reducing health inequalities Public Sector Equality Duty public involvement in CCG and promotion of choice training, innovation and research environmental sustainability delivering on relevant areas of the Governments mandate to NHS England and the NHS England planning guidance Everyone Counts 3 achieving financial balance. NHS Eastern Cheshire CCG is currently meeting its statutory duties. 1.1 About the Eastern Cheshire Healthcare Economy Eastern Cheshire is located in the North West of England and includes towns such as Macclesfield, Knutsford, Wilmslow, Poynton and Congleton as well as many villages and rural areas. It has a population of over 204,000 and most local people are classed as white British. The CCG area has 53% of the population of Cheshire East Council. With NHS South Cheshire CCG, the two CCG s are co-terminus with the boundaries of the Council. Health and social care spending on the residents of Eastern Cheshire is around 346 million per year. Around a third of this is spent on hospital care, a thirteenth on GP practices (Primary Medical Care), one sixth on community care, one seventh on social care, one twentieth on mental health and the rest on other services such as prescribing and specialist care. Figure Two provides a summary of key facts about the Eastern Cheshire healthcare economy, described within this section. Figure Two: Key facts about Eastern Cheshire In Eastern Cheshire there are 38 Pharmacies, 42 Dentists and 48 Opticians and over 60 care/residential homes. There is one District General Hospital in Macclesfield and two community hospitals (Congleton and Knutsford), run by East Cheshire NHS Trust. The hospitals and community health services (e.g. District Nurses who visit patients in their homes) are also managed by East Cheshire NHS Trust. Mental Health services are managed by the Cheshire and Wirral Partnership NHS Foundation Trust. The proximity of Eastern Cheshire to Greater Manchester provides Eastern Cheshire residents with significant access and choice of general acute hospital services and access to a range of specialist care providers (Figure Three). For example the population in the north of the CCG access hospital services across Greater Manchester. There is already an innovative model of providing specialist services locally with larger, specialist hospitals, like The Christie Hospital NHS Foundation Trust, enabling chemotherapy to be administered at East Cheshire NHS Trust, and a number of other locally delivered services Page 5 of 17

Figure Three: Access to hospital services across Eastern Cheshire and Greater Manchester At the end of 2012 we came together with Cheshire East Council and other partners to launch the Caring Together programme so as to look at new ways of providing high quality care services for the residents of Eastern Cheshire. Since then we have undertaken an extensive programme of engagement with local residents and healthcare staff to help co-design and co-produce a new model of care for Eastern Cheshire and to identify what we need to do to demonstrate improvements in care. We are now entering into an exciting phase of the Caring Together programme as we start to release a variety of products to our patients, carers, public and workforce that helps explain further how care will change in eastern Cheshire. Now more than ever there will be a need to clearly articulate the changes that will need to come about and why it is still so important for people to continue to keep being involved in help design and deliver Caring Together. Eastern Cheshire, as part of a Cheshire wide bid, was selected within the first cohorts of national pioneer areas around integrated care. To inspire and provide strong leadership for the respective project/programme, ensuring strong matrix working across the CCG including key stakeholder in health and social care. To develop and maintain strong working relationships with the key stakeholders across the health and social care system working at a senior level and with senior clinicians in Primary and Secondary Care. Key Duties/Responsibilities: Transformation Manager is responsible for developing and delivering change activities for patients in Eastern Cheshire as well as supporting continuous performance improvement. Responsibilities are set out below and are not an exhaustive list. Leading transformation Leading the co-ordination of activities relating to the transformation agenda and specifically in the area of Primary Care. Facilitating whole system collaborative working. Fronting the design of the future health and social care system. Establishing and implementing effective change activities to underpin successful transformation. Driving continuous performance improvement and service innovation. Interpreting and implementing corporate and national policies appropriately by considering them in a local context that relate to the specified transformation programme. Full ownership of the programme requirements from idea to implementation. Co-ordinate the development of business cases in line with the requirements of the programme and the CCG s business cycle. Ensure that risk/issue management is a key component of programme and project planning and management, and that the risk register is populated and utilised as appropriate to escalate/de-escalate issues and risks Supporting the Directorate Lead Advising and supporting the Strategy and Transformation Director in the development of service strategy and transformation. Supporting the Strategy and Transformation Director as required in all aspects of transformation and the commissioning requirements of the CCG. Communicating Providing regular reports as required on the development and implementation of the programme and on associated areas of performance. Managing potentially difficult areas around process and clinical service change management which may be sensitive, complex, contentious and confidential issues. Participate in relevant internal and external working groups/projects, services and initiatives to provide project information and analytical advice and expertise. Present programme information and issues, explaining complexities, to a wide range of internal and external stakeholders. Functional Responsibilities 3.1 Operational requirements Working within the overall Transformation Directorate, devise, implement and monitor the delivery of aspects of the programme allocated to the post holder. Provide overall management to the specific projects and aspects of the programme as assigned. Establish, maintain and deliver regular meetings for work streams and project groups. Report progress against the programme plan by written reports to the Caring Together Programme Management Group, and through personal representation at forums as required. Present complex information on all aspects of the programme in a clear, understandable and audience-appropriate manner. Ensure that an appropriate agreed process is in line with business objectives and is fully cascaded. Oversee the tracking of progress of specific projects and aspects of the Programme against plans and transition milestones, ensuring appropriate processes are in place to flag issues, risks and concerns with the relevant stakeholders. Work with the Leads for Communication and Engagement, Cultural Transformation and Finance for the Programme, to support the delivery of the programme. Page 10 of 17

Work closely with all stakeholders to ensure that they are able to align the required transformation and respond to changes with their organisation strategies. Ensure the securing of value for money, giving due consideration to all relevant factors including risk, quality and other factors. 3.2 Financial and Physical Resources Act in a way that is compliant with Standing Orders and Standing Financial Instructions in the discharge of this responsibility. Constantly strive for value for money and greater efficiency in the use of resources and strive for value for money and greater efficiency in the design of the future care models. 3.3 Staff Management To work in a matrix management style, across boundaries and foster close mutually respectful working relations with all stakeholder personnel. The post holder will provide programme leadership to the project leads in relation to delivery of the Programme. Disseminating information on training or service improvement to leads and other managers as appropriate. To manage, motivate and develop staff within the programme groups and activities to ensure that they are able to deliver the new responsibilities of transformation. Including, consider welfare and safety matters, taking into account instructions and guidelines issued and the management of disciplinary procedures as required. Uses personal development policies and procedures, and ensure that personal development is undertaken regularly. 3.4 Information Management The post holder will contribute to the development of systems to store and maintain all programme documentation in a systematic and consistent manner including full version control. Responsible for the operation and maintenance of information systems, adapting systems where necessary to suit changing information needs. Responsible for the collection and reporting on information of expenditure both within and outside the organisation, including compliance reporting. Ensures processes and staff behaviours are in place for appropriate information sharing. 3.5 Research and Development Ensures that relevant groups are able to access best practice and current information about Integrated Care and related issues in line with the transformation programmes. Commission any research needed to take forward specific elements of the programmes. Ensures that benefits from research and development and from innovation are realised by stakeholder organisation. 4. Operational Responsibilities 4.1 Policy and Service Development To identify and assess opportunities for new services and threats to existing services and market development. The post holder will need to maintain a good knowledge of emerging policies from government departments. This will assist in the thinking and definition of the Programme discussions for the Programme and stakeholders. Page 11 of 17

5. Freedom to Act The post holder will ensure that changes within Eastern Cheshire are in line with national policy and ensure that the Programme maximises the use of national opportunities. The post holder is required to ensure the delivery of the programme and ensure that difficult decisions are implemented via project leads that are not directly line managed by the post holder; this requires a high level of persuasive and negotiating skills. The post holder is accountable to the Strategy &Transformation Director but has a significant degree of autonomy in how he/she carries out the role. Able to work flexibly and on own initiative within the CCG policy and governance framework. 6. Making Every Contact Count Staff are in an ideal position to offer support and advice on how to improve health and wellbeing. Staff should use their interactions with the public to give them additional advice on health and wellbeing. Staff will be given training and support to help them to signpost people to other services which may improve their health and wellbeing. Confidentiality All CCG staff and contractors working for the CCG have both a common law duty and a statutory duty of confidentiality to protect patient (and indeed any personally identifiable) information and only use it for the purposes for which it was intended. The disclosure and use of confidential patient information needs to be both lawful and ethical. Information Governance CCG staff must keep up-to-date with the requirements of information governance and must follow CCG policies and procedures to ensure that CCG information is dealt with legally, securely, efficiently and effectively. Staff must appropriately manage the records they create or hold during the course of their employment with the Trust, making the records available for sharing in and confidentiality policies, procedures and guidelines (e.g. Freedom of Information Act 2000, Caldecott guidelines). Health and Safety All staff have a duty to ensure the health and safety of themselves and others whilst at work. Safe working practices and health and safety precautions are a legal requirement. ALL accidents must be reported to your manager and in line with the general philosophy of the CCG; you must participate in accident prevention by reporting hazards and following relevant policies and procedures including Moving and Handling guidelines. Risk Management You are required to contribute to the control of risk and use the incident reporting system to alert the CCG of incidents or near misses that may compromise the quality of services. Infection Control: All staff therefore have a duty to comply with all Eastern Cheshire CCG policies and guidelines in relation to Infection Prevention and Control. You have a duty to ensure that you minimise the risk of infection and infectious diseases. This responsibility includes minimising the risk by highlighting any concerns you may have to the appropriate person as identified in the policies and guidelines and challenging inappropriate infection control and hygiene practice.
