LHN Pharmacy Residency Program Mission Statement The mission of the LHN Pharmacy Residency Program is to empower pharmacy residents to become independent, competent, confident pharmacists in an environment that fosters professionalism, teamwork and accountability. PGY-1 Pharmacy Practice Residency Overview The LHN Pharmacy Practice Residency is a comprehensive post-graduate training program providing unique learning experiences with diverse patient populations in multiple facilities. The resident will have the opportunity to advance their clinical skills and knowledge base, further develop their leadership skills and provide patient-centered pharmaceutical care as an integral member of the healthcare team. The resident has the opportunity to provide additional staffing coverage as desired for additional compensation.

The Lutheran Health Network The Lutheran Health Network is a network of eight hospitals, five of which are located in Fort Wayne, Indiana and serve 23 counties in northeastern Indiana, northwestern Ohio, and southern Michigan. Lutheran Hospital is a 403-bed hospital. St. Joseph Hospital is a 191-bed community hospital. The Rehabilitation Hospital of Fort Wayne is a 36-bed rehabilitation hospital. Dupont Hospital is a 131-bed community hospital. The Orthopaedic Hospital is a 39-bed orthopedic hospital. Lutheran Health Network also encompasses other Indiana hospitals including Kosciusko Community Hospital in Warsaw; Dukes Memorial Hospital in Peru; and Bluffton Regional Medical Center in Bluffton. Lutheran Health Network provides pharmacy services to Community Memorial Hospital in Hicksville, Ohio. Other LHN members also include RediMed urgent care centers, multiple physician practices, ambulatory, surgical, laboratory and diagnostic centers in Fort Wayne and the surrounding areas. Lutheran Hospital offers a wide variety of specialty services from neonatal to geriatric patient populations. Lutheran is the only hospital in Northeastern Indiana that performs heart and kidney transplants. There are six intensive care units to take care of patients with coronary, cardiovascular, medical/ surgical and pediatric needs. The hospital also has a Children s Hospital within the hospital. That area includes a 10-bed critical care unit and a 25 private-bed neonatal unit. Lutheran Hospital is a verified Level II Adult and Pediatric Trauma Center. Additionally, Lutheran Hospital is an accredited Chest Pain Center, certified Stroke center, Bariatric Center of Excellence, and the region s only accredited Heart Failure center. Primary service lines for Lutheran Hospital include cardiac care, oncology, pediatrics, trauma, bariatrics, transplant and neurological services. St. Joseph Hospital has the region s only verified Burn Center by the American College of Surgeons and the American Burn Association. St. Joseph offers a wide variety of specialty services, including cardiology, cardiac surgery, pulmonology, orthopedics, geriatrics, critical care, and rehabilitation services. The hospital also operates a regional wound care/hyperbaric clinic, and a transition care unit/skilled care facility. St. Joseph Hospital is an accredited Chest Pain center. Department of Pharmacy Pharmacy Services Philosophy Distributive and clinical services are both considered equally important components of the pharmacy department and are closely intertwined. The approach to the provision of pharmacy services within the Lutheran Health Network is a hybrid approach in which all pharmacists are expected to be involved and competent in both distributive and clinical functions. It is the philosophy of the pharmacy department that there is a significant clinical component that is involved in the distributive functions and that all pharmacists need to be involved and knowledgeable in both functions. By understanding how the clinical services conducted on the floor affect the distributive processes and how the distributive processes affect the ability to provide clinical dosing and monitoring services, the pharmacy department is able to truly work together as one team to provide the highest quality of pharmaceutical care to the patient. The Pharmacy Departments at Lutheran, Dupont and St. Joseph Hospitals provide comprehensive clinical and distributive services 24/7. The Pharmacy Department at Lutheran Hospital serves The Orthopaedic Hospital and the Rehabilitation Hospital. Additionally, the Pharmacy Department at Lutheran Hospital provides 24/7 comprehensive clinical and distributive pharmacy services to Community Memorial Hospital remotely as well as provides remote after hours prospective pharmacist review of medication orders, clinical services and drug information to Dukes Memorial Hospital. The LHN Pharmacy Departments work together as a network department, all sharing the same formulary, policies and procedures, clinical protocols and staff development opportunities. Each Pharmacy Department is led by a Pharmacy Director. The Pharmacy Director for Lutheran Hospital serves as the Network Pharmacy Director and coordinates and guides automation, operations and initiatives for the entire network. The Director of Pharmacy Clinical Services and Residency Program coordinates and guides clinical pharmacy services, protocols, formulary management and staff development and education, as well as serving as the LHN Residency Program Director. Clinical Coordinators at Lutheran and St. Joseph Hospital provide assistance in the development and implementation of clinical pharmacy services and have individual specialized clinical service responsibilities based on areas of expertise. A technician supervisor at Lutheran Hospital also oversees all technicians, the technician training program and some of the operational responsibilities for Lutheran Hospital. Additionally, a team of a pharmacy informatics coordinator, informatics pharmacist, automation technician and a technician overseeing chargemaster and patient care assistance programs provide network pharmacy informatics support.

The Pharmacy Departments are fully automated operations offering complete unit dose and IV admixture services. The automated distributive and informatics services include paperless FAX-imaging order processing, computerized prescriber order entry, access to the electronic nurse documentation system, access to lab and medical documentation, automated TPN compounding, pharmacy dispensing robot technology and automated drug distribution cabinets. The pharmacy order entry system provides robust drug allergy and interaction checks, lab integration, clinical intervention documentation and customized electronic alerts based on labs, medications, patient populations and therapy types. The pharmacy robot provides medication cart fill assistance for all hospitals of Lutheran Health Network and 24/7 first dose dispensing of medications for Lutheran, Orthopaedic and the Rehab Hospitals. Decentralized automated drug distribution cabinets are on patient care areas for distribution of PRN medications and first doses. Computerized Prescriber Order Entry as well as bedside drug administration verification assisted by bar code technology has been implemented at three facilities. IV Infusion Smart Pumps have been implemented throughout the Network. The Lutheran Health Network has full access to electronic drug and medical online databases including Micromedex, NeoFax and UpToDate. The Pharmacy Department at Lutheran Hospital has a satellite pharmacy located in the OR which provides pharmacy services to the three surgery departments, endoscopy suites and Cath Lab, staffed Monday thru Friday by a pharmacist and technician. The Pharmacy Department is also about to open a second satellite in the Oncology area which will serve the adult and pediatric oncology clinics as well as the inpatient oncology units. Pharmacotherapeutic Services Specialized clinical pharmacy services include a well-established pharmacokinetic dosing service, anticoagulation service, TPN dosing service, pain management service, renal dosing service and erythropoetic growth factor dosing service with pharmacy responsible for the complete dosing and monitoring of patients. The pharmacists are all ACLS certified and are members of the code blue team assisting in the preparation and administration of code medications as well as documentation of code process and outcomes. Pharmacists counsel patients as part of a medication discharge program, and provide inpatient and outpatient classroom teaching as a member of the diabetic education programs at Dupont and St. Joseph Hospitals. There is a Critical Care Pharmacy Service at Lutheran Hospital with two pharmacists providing comprehensive pharmacy services to the adult critical care units for ten hours a day, seven days a week. In addition to routine clinical dosing and monitoring services, the Critical Care Pharmacists complete full chart reviews for all critical care patients, prospective ADR monitoring, pharmacotherapeutic recommendations as well as nursing and Medical Staff education. For the Transplant Programs at Lutheran Hospital, a dedicated transplant pharmacist serves on the multidisciplinary team with daily rounds, patient medication education and providing comprehensive pharmacy services for this patient population. Pharmacists serve on the multidisciplinary Burn Care team at St. Joseph Hospital, participating in rounds, staff education and providing clinical pharmacy services. Pharmacists participate on the multidisciplinary congestive heart failure team at Lutheran Hospital, providing pharmaceutical care services for both inpatients and outpatients. Additionally, there is significant pharmacist involvement on the Antibiotic Stewardship Team at Lutheran Hospital, providing comprehensive review of positive cultures and appropriate antimicrobial recommendations for inpatients. The pharmacists are also involved in several outpatient clinics including anticoagulation clinics, cystic fibrosis clinics and transplant clinics. The Anticoagulation Clinic at Lutheran Hospital is a pharmacy-run clinic with nursing participation that provides care for approximately 2000 patients per year. There is also a pharmacy-managed Anticoagulation Clinic at Dupont Hospital. Several pharmacists also provide dedicated pharmacy services to the adult and pediatric cystic fibrosis clinics two days each week. These pharmacists oversee the pharmaceutical care of the cystic fibrosis patients when they are admitted into the hospital as well and provide education and competencies for the rest of the pharmacy staff on specialized care for this patient population. The transplant specialist provides comprehensive services in the heart and kidney transplant clinics with two pharmacists who assist in her absence.
