Post Title: Community Nursery Nurse Band: 4 Business Unit: Children & Families Responsible to: Clinical Service Manager Job Description Accountable to: Head of Children and Families Purpose To support and assist the senior team members in the provision of a high standard of care and the promotion of physical, mental and emotional health to individuals and client groups. To be an effective member of the Multidisciplinary Team carrying out assigned tasks involving direct care in the support of senior team member. Principal duties to include: To work as part of the multidisciplinary team participating in procedures and duties in accordance with agreed standards relevant to role. To participate in the assessment process of care needs and implementation of programmes of holistic care within set standards. To work in partnership with clients ensuring privacy and dignity is maintained. Involved in supporting, educating and informing family/carers where appropriate. To assist in the promotion of health and wellbeing of clients. To undertake clinical interventions as delegated by a senior team member. To maintain close liaison with the senior team member who is responsible for the workload. To maintain clear and comprehensive, signed and contemporaneous records according to company procedures. To assist with clerical and administrative duties as required. To work in co-operation with all other professionals and agencies involved in the care of the client. To be aware of and act upon, where necessary, the procedures in place to protect vulnerable individuals. To participate in the company s Clinical Governance Strategy. To participate in the company s Risk Management Strategy. Support families in relation to infant/ child nutrition in the clinic, home or other setting. Carry out home visits to selected families to teach play skills and promote early learning through demonstration and shared activities. Support families in general management of child care both in the home and in the clinic, document all contacts and feedback information to the relevant Health visitor within 24 hours or before depending on urgency Participate in Child Health Clinics Undertake child health assessments as directed by the health visitor Observe children s developmental progress and feedback to the health visitor if there are any deviations from the norm In conjunction with the Health Visitor, promote prevention of accidents and encourage safe practice in childcare Post Title: Page 1 of 5 Date
