GENERAL INFORMATION Registration and Fees In addition to being open to Kaiser Permanente members, many of our classes are also open to the community. To get the most value from the educational experience, individual classes are organized into Sessions. Participants may determine the number of classes they can attend within the Session. For more information or for class schedules, view or download our catalog at kpinhawaii.com/our-services, visit kp.org/classes, or call the phone number listed within the class description. All classes require pre-registration and pre-payment (if applicable). * To receive a refund, you must cancel at least 24 hours before the start of the first class. Class dates and times are subject to change/cancellation. Only registered participants will be notified of any schedule changes. * Class fees vary and are subject to change. In General Walk-ins are not advised, as the class may be full or may have been cancelled due to low enrollment. For safety, children are not allowed to attend any classes unless they are participants of the New Sibling Tour. Receipt of payment must be presented at your first night of class to participate. For Pregnancy and Childbirth Classes Payment is due one week after registration and must be made in full with a cashier at any Kaiser Permanente facility. If your payment is past due, your seat will be given away. Receipt of payment must be presented at your first night of class to participate. Refunds for any missed classes are not allowed unless medical verification is provided. For Exercise and Fitness Classes Drop-ins (participating) and/or auditing (observing) of classes are allowed on a one-time basis only. Drop-ins are assessed a fee. Payment is due no later than one week after registration and must be made in full. Pro-rating of class fee is only allowed at the time of registration and for a pre-arranged medical absence or a prearranged vacation. Refunds for any missed classes are not allowed unless medical verification is provided. Receipt of payment must be presented at your first night of class to participate. A participation waiver must be signed on your first night of class. Note: There are risks inherent in any exercise program. Kaiser Permanente recommends that you consult with your physician before beginning any exercise program. The Prevention and Health Education staff would be happy to answer any questions you or your physician may have about any exercise program. To reserve space in a class or to purchase any of our healthy resources, please contact Prevention and Health Education at 808-432-2260. Upon receiving your request, we will send you a payment slip by e-mail or postal mail. Present this slip to a cashier at a Kaiser Permanente facility near you. Payment can be made by cash, check, debit or credit card. Some products and services provided by an entity other than Kaiser Permanente. Learn techniques to restore your self-image and cope with appearance-related side effects. Licensed beauty professionals will provide tips on makeup, skin care, nail care, and head coverings. This program provides a complimentary cosmetic kit and is a partnership between the American Cancer Society, the Personal Care Products Council, and the national Cosmetology Association. Identify the types of support you need - logistical, emotional, financial, or legal. Discover what community resources are available to you and your family. Location: Moanalua Medical Center, Neuroscience Conference Room 4C, 4th Floor Dates/time: Saturdays, 1/6 or 5/5; 9:30 to 11:30 a.m. Nutrition and Exercise During and After Cancer Treatment (Members Only) Manage eating problems related to cancer treatment. Explore the importance of making healthy dietary choices and being active during treatment. Learn the importance of maintaining a healthy weight, adopting an active lifestyle, and making good dietary choices after cancer treatment. Healthy Kids, Healthy Families Healthy habits begin at home. We will cover eating and meal tips to improve health and energy. Families with children ages 6 to 17 can learn together on how to create a healthy lifestyle. Attendance by parent(s) and pre-registration required. Keiki Passenger Safety Is your child s car seat or booster seat installed properly? Get it checked by a child passenger safety technician certified by the National Highway Traffic Safety Administration (NHTSA). Join this hospital tour to help prepare your children for surgery. Studies show that children who take part in preoperative tours have an easier time during and after surgery. They become familiar with the hospital environment, equipment, and procedures and can ask questions and discuss concerns. Tours may also be offered at your child s pre-operative appointment for your convenience. Meet with others who deal with the day-today challenges of Type 1 Diabetes. Learn about resources, management tips, and the latest research and treatment options. Family members can participate and share how this illness has affected them along with the fears they have when assisting with management. How to Enroll in Kaiser Permanente FIT REWARDS 1. Visit kp.org/activeandfit and register to use the website. 1 2. Choose a gym. Call toll free 1-877-750-2746, Monday through Friday, 5 a.m. to 6 p.m. (Pacific time), and ask for your Fitness ID number. Your gym will ask you for this number. 3. Pay the annual $200 program fee directly to your gym. 2 There will be no additional monthly dues to worry about! Please note: If you re currently a 24 Hour Fitness member or would like to join 24 Hour Fitness, visit kp.org/activeandfit or call toll free 1-877-750-2746, to pay your annual program fee before going to the gym. Then, get fit and get your money back! After you enroll, work out at a participating gym at least 45 days for a minimum of 30 minutes per session by the end of the calendar year, and you ll get your annual program fee back once your gym reports your completion. 3 That s less than once per week! Eligibility All Kaiser Permanente Hawaii members, 16 years and older. 4 Home Fitness Option Don t want to go to a gym? Sign up for the Home Fitness Program for $10 a year. 5 Choose up to 2 Home Fitness Kits each calendar year and work out anytime. 1 The Active&Fit website is available for members who are 18 years or older. 2 Pay the annual program fee to a participating gym. Taxes and additional fees you pay your gym for membership upgrades, classes, services, or amenities are not included in the program and not eligible for reimbursement. Please consult with your tax advisor about the taxability of the reimbursement. Except for earning your annual program fee back by exercising 45 days a calendar year for at least 30 minutes a session, your annual program fee is not reimbursable and will not be prorated. 3 Meet the 45-day, 30 minutes per session activity requirement by the end of the calendar year to qualify for reimbursement. Fitness center visits made before your annual program fee is paid do not count towards the reimbursement activity requirement. Reimbursement is limited to your annual program fee each calendar year. 4 Medicare and Medicaid (QUEST Integration) members are not eligible to participate in the program. 5 Members can choose either the gym membership program or the home fitness program. The $10 Home Fitness Program annual fee is non-refundable. Kaiser Permanente Fit Rewards is a value-added service and not part of your medical benefits.
