These individuals are identified after having an NHS Health check either at their GP practice or in a community setting, carried out by the NHS Health Checks outreach team. They and their partners are offered the programme in a community health centre or other convenient community venues. The MyAction is a 8-12 week programme delivered by a multidisciplinary team of a lead nurse, dietitian and physical activity specialist. VERSION 3-15 th NHS Health Checks Outreach team Service Description: This is a team of two specialist nurses who offer CVD risk assessment and advice aiming to reduce the risk factors of those vulnerable to developing vascular disease and to detect early indication of disease. The service is targeted at adults (male and female) between the age of 40 to 75 years who reside in Lambeth or who are registered with a Lambeth General Practitioner (GP) and who have no previous diagnosis of cardiovascular disease. Most of the health checks are provided through general practice, however, the LCH service has adopted an outreach focus with clinics are provided in various locations around Lambeth to maximise access for hard to reach groups.
