VERSION 3-15 th Audiology Adult (17 years +) Service Description: Inhealth Netcare assessment, hearing aid fitting, follow up. GSTT Complex audiology services as tinnitus, balance (referrals through ENT). This department also provides a comprehensive rehabilitation service, including specialist tinnitus, vestibular and balance rehabilitation. St Georges - Complex audiology services as tinnitus, balance (referrals through ENT), specialised diagnostic and rehabilitative services for patients presenting with dizziness, imbalance, hearing loss and tinnitus, and also provides specialist learning disability clinics. Inhealth, GSTT and St Georges will provide ingoing repair and management of hearing aid after their 8 week follow up appointment How to refer: Direct access. Acute Caseload - Acute caseload consists of those children and young people requiring short-term intervention or nursing care from the team. The team adopts a holistic view of health which encompasses both physical health and mental health and well-being; building local capacity by facilitating education & training as well as engaging and developing partnerships with key service providers and recipients. Their core work includes :- Translating health promotion theory into practice by developing, supporting and managing community based health promotion project Providing a range of generic and tailored training opportunities to increase the capacity of front line staff and community providers to deliver, monitor and evaluate effective health promotion activities for priority groups. Providing consultancy, advice and guidance to a range of providers and voluntary sector groups, promoting community development and partnership working to increase the relevance, appropriateness and effectiveness of local health promotion activity. Bladder & bowel assessments undertaken by generic nurses for housebound adults, children and adolescents through the general adult community nursing and childrens nursing teams with a focus on the promotion of continence. Children with special needs and enuresis can be referred to the childrens bowel and bladder service. 2. A Continence Specialist Nurse providing training, advice & support to generic nurses across the borough. SLAM manage the staff rota s, supervision and triage the referrals. The team are based with SLAM but work across LCH and other health and voluntary sector sites in Lambeth. The PWP s provide brief evidence based psychological interventions to people with mild to moderate depressions and anxiety problems. These individuals are identified after having an NHS Health check either at their GP practice or in a community setting, carried out by the NHS Health Checks outreach team. They and their partners are offered the programme in a community health centre or other convenient community venues. The MyAction is a 8-12 week programme delivered by a multidisciplinary team of a lead nurse, dietitian and physical activity specialist. VERSION 3-15 th NHS Health Checks Outreach team Service Description: This is a team of two specialist nurses who offer CVD risk assessment and advice aiming to reduce the risk factors of those vulnerable to developing vascular disease and to detect early indication of disease. The service is targeted at adults (male and female) between the age of 40 to 75 years who reside in Lambeth or who are registered with a Lambeth General Practitioner (GP) and who have no previous diagnosis of cardiovascular disease. Most of the health checks are provided through general practice, however, the LCH service has adopted an outreach focus with clinics are provided in various locations around Lambeth to maximise access for hard to reach groups.
