The Hospice at Home service provide Specialist Palliative Care to adults and children living in Counties Limerick, Clare and North Tipperary. The aim is to support people who choose to be cared for, and sometimes die, in their own home or in the local community hospital or nursing home. QUALIFICATIONS: Candidates must be registered in the relevant division of the Register of Nurses as maintained by An Bord Altranais or be entitled to be so registered. Have at least 5 years post registration experience of which 2 must be in the speciality. Experience working in the Community and/or management experience desirable. Have the clinical, managerial and administrative capacity to properly discharge the functions of the role, management qualification desirable Demonstrate evidence of continuing professional development at the appropriate level HOURS OF WORK: JOB SUMMARY: Normally not more than 39 hours per week. The CNM 2 will be a member of: The Nursing Management Team for the Hospice at Home Nursing Team. The Senior Nursing Team, Nursing Department, Milford Care Centre. The Specialist Palliative Care Community Services Operations Group, Milford Care Centre. He/she will: Assist the Community Services Manager in the overall management, co-ordination and supervision of patient care within a multidisciplinary setting, ensuring a high quality of patient care in a safe and caring environment. Play a pivotal role in service planning, in co-ordinating and managing activities and resources (material and human) in a cost effective way with due regard to budgetary control. PRINICPAL DUTIES and RESPONSIBILITIES NOTE: The level of responsibility will vary in accordance with roster and role being undertaken 1. Clinical Responsibilities Play a pivotal role in service planning, co-ordinating and managing activities and resources within the clinical area. Provide a high level of professional and clinical leadership. Manage patient care to ensure the highest professional standard including resource and case load management, staff rostering and staff development. Facilitate communication, co-ordination and co-operation across all the health care teams. Participate in clinical audit as required in consultation with the Community Services Manager. Maintain professional standards in relation to confidentiality, ethics and legislation. Be aware of safety procedures / emergency procedures and implement them when required. Ensure completion of incident / near miss forms / clinical risk reporting.
Participate in the development and delivery of induction, education, training and development programmes for nursing and care staff. Be aware of training schedules for staff and ensure staff compliance with ongoing training, education and professional development. Maintaining a safe environment for all linking with General Services for the ongoing servicing / maintenance of all equipment. Act as liaison with medical and para-medical staff in the co-ordination of patient care. Act as a resource for all staff in the co-ordination and delivery of patient care. Take responsibility for the safe medication management practices in accordance with Milford Care Centre procedures and current legislation, reporting errors without delay. Participate in and support the quality assurance processes. 2. Management Responsibilities. Lead, manage and supervise the nursing and care staff. Provide an adequate, efficient rostering system that ensures daily staffing levels are adequate and allow for an appropriate skill mix when required Participate in the triage and management of referrals to ensure patients receive a timely and appropriate response from the service. Ongoing evaluation of nursing practice Actively participate in the management of absenteeism. Liaising with Human Resources and Finance Departments on related matters as required Ensure all care areas are adequately equipped and furnished in accordance with agreed standards and protocols, liaising with General Services as required. Liaise with relevant CNMs on staff related issues of relevance. Provide the necessary supervision, co-ordination and deployment of nursing and support staff to ensure the optimum delivery of care in the designated area. Promote effective communication at ward and departmental level and facilitate team building. Provide staff leadership and motivation which is conducive to good working relations and work performance Promote a culture that values diversity and respect in the workplace Assist in the formulation, implementation and evaluation of service plans in consultation with the Community Services Manager. Manage all resources efficiently and effectively within agreed budget Prepare TMS returns, manage annual leave and other leave entitlements and maintain proper records in these areas of responsibility in accordance with MCC policy when required. Lead and implement change in consultation with the Community Services Manager. Promote, facilitate and participate in the development of nursing policies and procedures. Monitor as appropriate and lead on proactive improvement Ensure compliance with legal requirements, and the requirements of An Bord Altrainais Maintain all necessary clinical and administration records and reporting arrangements Engage in IT developments as they apply to service administration. 3. Personnel Management Responsibilities Promote integration between the nursing team in Hospice at Home, all clinical areas and MDT. Participate as required in the recruitment, selection and appointment of nursing and care staff. Participate in the appraisal of the performance of members of the nursing team as required and in accordance with MCC policy Deal with Industrial Relations issues relating to nursing and care staff in consultation with the Community Services Manager and the Human Resources Department.
Foster a high level of morale among staff by effective motivation and communication. Deal with Staff complaints, discipline and grievance issues in accordance with the policy of Milford Care Centre and report to the Community Services Manager, as necessary, any serious or unresolved issues. Seek the advice of the HR Department as required Provide support and act as an information link to the Community Services Manager. 4. Education and Research Contribute to the activities of the Education, Research and Quality Department as agreed in consultation with the Community Services Manager and the Senior Nursing Team Ensure that research based findings form the basis of nursing practice in the clinical area. Demonstrate a knowledge and understanding of research skills Participate in ongoing relevant research and ongoing and current research into issues affecting patient care. Liaise with the Community Services Manager to review individual learning and development needs and goals 5. Health and Safety To be familiar with the Safety Statement Milford Care Centre and the Safety, Health and Welfare at Work Act, 2005. To ensure that the working environment is a safe and hazard-free zone. To ensure that all equipment is maintained in a safe working order. To investigate all accidents and incidents, reporting them to the appropriate personnel, along with relevant documentation, in a timely manner. To be aware of the policy in relation to fire prevention and the procedure to be followed in the event of fire in Milford Care Centre. Adhere to the Milford Care Centre Policy on Moving and Handling. Ensure new members of staff are familiar with this and carry it out at all times. Coordinate and evaluate the quality of patient care delivery by all members of the nursing team under your supervision. 6. Key Performance Criteria The extent to which there is patient satisfaction with nursing care. The extent to which good working relationships are fostered and maintained throughout the nursing and care service and between nursing and other disciplines. The effectiveness of performance in relation to personnel management and development The quality and standard of nursing care. The extent to which service plan objectives have been achieved.
