TITLE OF POSITION CLASSIFICATION LEVEL PROGRAM JOB DESCRIPTION Senior Case Manager LCC Level 5 (Salary packaging is available) Intensive Family Support Services, Ntaria NT OVERVIEW Lutheran Community Care (LCC) provides community services on behalf of the Lutheran Church in SA and NT through a range of programs. We support communities through responding to the needs of individuals and families, community development, learning opportunities, accommodation and support. Current programs include emergency relief, financial counselling and wellbeing, family support and education, foster care, housing and family shelter, and refugee services. LCC is committed to reducing barriers and encouraging inclusion and participation in the community of people with a disability, people of all ages, Lesbian, Gay, Bi-Sexual, Transgender, Intersex and Queer (LGBTIQ) people, Aboriginal and Torres Strait Islander (ATSI) people and people from Culturally and Linguistically Diverse (CALD) backgrounds. Programs in Central Australia include Early Childhood, Family Support, Emergency Relief and Financial Wellbeing and Capability. The Intensive Family Support Services (IFSS) program is funded by the Department of Social Services and aims to reduce child neglect by increasing the capacity of Families, Parents and Caregivers. ROLE SUMMARY The aims and objectives of this role are to: Support referred families to improve their ability to protect and care for their children using a strengths-based approach. Coordinate support with case planning and assessment. Conduct planned home/ family visits in locations of choice as part of the IFSS team. REPORTING RELATIONSHIPS AND ACCOUNTABILITY The Senior Case Worker reports to the Team Leader IFSS Program Ntaria. 1

ROLE CORE BEHAVIOURS/RESPONSIBILITIES RESPONSIBILITIES AND DUTIES SPECIFIC TO THE ROLE: Support families Assist families to address identified issues threatening their parenting capacity and other factors contributing to their disadvantage. Assist families in the process of obtaining alternative options if necessary and support them to address issues relating to child neglect. Provide practical support where necessary. Meet regularly with families, sometimes in their homes, to provide ongoing support. Provide intensive support to clients referred to IFSS Program as required. Appropriate use of brokerage funds (in consultation with the Team Leader) to ensure families have access to services not provided by IFSS. Recognises and celebrates progress and achievements. Communication Communicate effectively to engage and establish rapport with clients and team members. Provide respectful and appropriate support to Indigenous families. Network and promote LCC IFSS services to other organisations in a professional manner. Promote, facilitate and model positive collaboration between agencies and sectors. Liaise with agencies and service providers as necessary. Case Management Conduct intake and needs assessments with families referred to LCC using agreed models. Deliver client centred, strengths-based Case Management for long-term benefits. Assess the priority and provide appropriate responses to families that demonstrate high and complex needs. Write case management plans in conjunction and in consultation with clients. Identify and respond to factors with families that may put child at risk.. Maintain case notes and all other data required to meet funding guidelines. Meet regularly, monitor, and review case action plans with families. Ensure preparation of case reviews for supervision and actively participate in supervision with Team Leader.

Provide opportunities for families to access culturally appropriate group programs aimed at safety and wellbeing. Seek cultural and family advice from the Cultural Family Support Worker. Advice and Referral Provide families with options and resources to assist them improve their wellbeing. Provide practical support so that families access appropriate services. Advocate for clients with relevant government and non-government agencies to assist in achieving positive outcomes. Refer clients to appropriate services and agencies in line with their case management plan. Provide advice and options to the families in line with service delivery and funding Skills in helping families Model appropriate child management strategies based on sound understanding of children s developmental needs and trauma informed practice. Provide an integrated intervention service that responds to the needs of clients referred to the programs. Assist clients to address life control issues that are impacting on their children. Assist clients to address crisis as they arise. Data collection and record keeping Maintain up to date client case note files. Ensure client case files are kept in order in a safe, secure, locked location. Write reports and enter data as required. Leadership and Training Attend accredited training as required by the Department to fulfil contractual obligations and LCC requirements. Participate in personal development as discussed with the Program Manager/Team Leader. Assist in the provision of day-to-day supervision of workers. Assist in the facilitation the professional development of staff and assist them to participate in appropriate training. Lead effective meetings. Engage in difficult conversations and manages conflict. Contribute to recruitment and selection processes.

PERSON SPECIFICATION REQUIRED QUALIFICATIONS, SKILLS, KNOWLEDGE, EXPERIENCE AND ABILITIES Degree in Social Work and eligible for membership of the Australian Association of Social Workers or a degree of a tertiary institution which gives eligibility for membership of the Australian Community Workers Association, or qualification in another relevant discipline. Practical understanding of strengths-based family intervention approaches. Sound knowledge of Aboriginal culture. Community knowledge with reference to Central Australia Ntaria community. Skills in case management, assessment and referral in the context of providing child focused parenting support. High level of communication skills (listening, verbal and written). Ability to communicate effectively with Indigenous people. Aptitude and sensitivity in working with and supporting families at risk including understanding of child development and the impact of neglect and trauma on children and families. Ability to work independently and demonstrate initiative. Demonstrated ability to maintain case notes, data entry and generate reports on service activity. Strong team work focus and demonstrated experience in leading and managing people. Sound knowledge of child development, attachment issues and behaviour management. Skills in preparing and presenting information for training groups and for reporting to meetings. ATTRIBUTES THAT ARE DESIRABLE, BUT NOT ESSENTIAL Knowledge and appreciation of the Lutheran theology of caring. Experiences in mentoring Indigenous co-workers.
