Clinical Care/Care Coordination Professional Development of specialty practice Professional Leadership and Expertise Purpose of the role : To enhance early discharge of applicable patients from hospital to the Interim care scheme, ensuring patients, families/whanau and staff have full understanding of Interim care. Provides overview of patient care and safety whilst in dedicated facility in Interim care, liaising with the hospital teams. For example, PIMS, Concerto, Outlook, Interim care Database etc Develops and maintains the Interim care Database to maintain accurate records for patients on Interim care to enable coordination of care whilst in Interim care Maintaining own professional development by attending relevant IT educational programmes. Domain Two Management of nursing care at advanced practice level Includes accountability related to patient/client assessment and management of advanced nursing care that is supported by evidence Co-ordinates the management of Interim care patients Liaises with Gerontology team to identify and discuss new referrals and appropriateness of patient. Visits new patients within 24 hours of referral Develops a relationship with patients / families which demonstrate professionalism and sensitivity. Liaises with medical professionals of key areas, I.e. orthopedics, Gerontology, surgical and medical re: patients treatment options. Completes a holistic advanced patient assessment and develops an appropriate plan of care for Interim care in discussion with the patient, family and the facility Uses advanced assessment skills, identifying the most appropriate placement depending on the patients condition, circumstances and the type of treatment selected. Education and clinical teaching Clinical Care/Care Coordination Contd Monitors and evaluates the implementation of the care plan. Maintains up to date documentation at all times, in patients notes and on the Interim care database. Ensures education reflects patient and family needs and learning styles. Continues to update and produce new and appropriate education materials for patients. Provides education on Interim care, follow up and ongoing care. Involves cultural resource, social worker, OT and other allied staff as appropriate. Ensures safe effective discharge planning by; Ensure patient / family member is safe for discharge Providing necessary equipment for Interim care Work with designated Social Worker to set up placement and coordination of care of patients under the Interim Care scheme. Order appropriate diagnostic tests in liaison with appropriate medical team Monitor and document test results and report to medical team as necessary Book follow up appointment with medical team Liaising with Home Health care staff re care requirements in the community Daily monitoring of patients. Organisation of regular meetings with Social worker and GNS to discuss current and prospective Interim care patients. Administers IV antibiotics as needed to interim care patients. Uses expert knowledge and advanced assessment skills to provide clinical advice and facilitate problem solving First point of call for patients, family, community GNS and other health care providers looking after the Interim care patients with any concerns and problems. Troubleshooting any problems Determine and identify when patients are required to come back into hospital Organise re-admission in liaison with the admitting medical team and Outpatients Home visiting patient if any concerns or problems identified. Provides specialist nursing care and expertise both in direct care delivery and in support to other staff in the management of a defined patient group/area of specialty practice. Role models expert clinical practice, excellent communication skills and a variety of effective project management/leadership skills Facilitate the education of nursing and medical staff throughout Waitemata DHB and other Home Health Care providers Providing education (1:1 and group) for inpatient, community and interim care staff relating to essentials of care to enable them to manage patient s routine and emergency situations. Provides education and support to ward nursing staff and medical staff on Interim care program. Assists the District Nurses to prepare for their ongoing case management role. Promotes cost effective practice. Develops and maintain links with the relevant community services. Provides education and training to interim care facilities that will be receiving Interim care patients into their care. Provides an advisory role provides patient education to the patient group to improve patient outcomes and gain participation with their treatment Supports other staff in the management of a defined patient group/areas of specialty practice Acts as a nursing resource providing expert advice and education to nursing staff and health care professionals / family-whanau within specialty scope of practice, demonstrating skilled mentoring/coaching, teaching and supervision. Creates and maintain a database of patient details, including numbers of referrals, reasons why patients were accepted or declined, treatment course and outcome. Maintains and updates processes for patient referral, assessment, care planning, education and documentation. Maintains and updates patient education details as necessary. Establishes and maintains current policies and procedures which reflect best practice. Monitors standards of nursing practice. Works with the Medical Heads of Division to establish and monitor clinical indicators. Utilises patient experience and family feedback to further develop the service e.g. Patient satisfaction surveys. Produces a monthly report on productivity and distribute to the relevant people within Waitemata DHB. Documents patient outcomes.
