Purpose of the role: To provide a client/family/whanau centred safe, responsive physiotherapy service to inpatient and outpatient areas of Child Women and Family services. To provide cover to Rangatira ward, and participate in the weekend roster as required Services are delivered in accordance with Waitemata DHB Strategic Plans Clinical Practice KEY TASKS EXPECTED OUTCOMES is a member of the collaborative team and works within all collaborative team processes. Client s physical function is assessed and abilities and obstacles are identified in conjunction with the team members including the family / whanau. Therapy goals and interventions are planned with family / whanau and other team members to meet client s needs within a developmental, cultural and environmental context. Participation in provision of physiotherapy services to children on Rangatira ward who meet the criteria for intervention. Interventions are implemented in a timely, safe and effective manner in accordance with the agreed plan. Interventions will take place in the most appropriate environment. Regular reviews of client interventions, including progress and satisfaction, are carried out. Discharge planning is timely, effective, and involves consultation with community personnel as required. Education is provided for parents, caregivers and referral agencies enabling them to identify and support the need for an early intervention programme. Boundaries of practice are identified/recognised and referral on occurs as appropriate Autonomous caseload management, seeking senior support for complex situations. Page 3
Quality of service Quality Assurance KEY TASKS EXPECTED OUTCOMES Criteria for admission / prioritisation to service are followed. Active contribution to the functioning and development of the interdisciplinary team, including orientation of colleagues, meetings and case conferences There is participation in overall physiotherapy caseload management within the service, covering for other teams where necessary. Education to Waitemata DHB and external groups occurs in consultation with management. Professional networks with other physiotherapists are made and maintained to enhance the service for patients, and own professional development. There is liaison with community groups to ensure continuity of service for clients. Participates in the implementation of service developments, in conjunction with the Physiotherapy Professional and Clinical Leader. Provision of urgent physiotherapy cover for any of the other teams in consultation with the Allied Health Team Leader/ Physiotherapy Professional and Clinical Leader Professional Standards of Practice are met in accordance with the Physiotherapy Board of New Zealand and Physiotherapy New Zealand Demonstrates professional behaviour at all times Professional supervision takes place on a regular basis with professional supervisor Responsibility is taken for own learning by recognising training and skill needs and undertaking professional development activities on an ongoing basis. Participation in annual performance review with designated clinical or team leader. Demonstration of a self-directed approach to learning Participates in physiotherapy quality initiatives Participation in Service educational programmes and responsibility for own learning. KEY TASKS Service administration standards are met. EXPECTED OUTCOMES Promotion of seamless service provision through liaison with other physiotherapy colleagues. Provides training and education of students of own discipline (contributes to learning needs of students of other disciplines as appropriate). Completes Waitemata DHB mandatory training. Accurate client and statistical information is maintained as per service requirements and documentation policy. Equipment and resource record is maintained. Waitemata DHB resources are utilised effectively.
