SCHOLARSHIP APPLICATION GUIDE 2013
ELKHORN AREA HIGH SCHOOL LOCAL SCHOLARSHIPS - 2013 This booklet has been prepared to help Elkhorn Area High School seniors become aware of the scholarships that are available on a local basis. Please read this information carefully. Some of the scholarships are open to anyone and others are for those pursuing a specific career area. Some scholarships are based on financial need, while others do not take that into consideration. APPLICATION FOR SCHOLARSHIPS Included in this booklet are a scholarship checklist, a copy of the general application, and any supplemental forms that may be required for particular scholarships. All students wishing to be considered for any of the scholarships listed must at least complete the general application and checklist. Please read each scholarship description carefully and only apply for those for which you qualify. Please note, when applying for scholarships, you must supply the Guidance Office a copy of the application for each scholarship for which you are applying. For example, if you are applying for 15 scholarships, check all 15 off on the supplied checklist and bring 15 copies of the scholarship application to the Guidance Office. Only one copy of the checklist is required. All scholarship applications are due in the Guidance Office by Tuesday, April 23, 2013 at 3:00 p.m. No late applications will be accepted. COMPLETING THE APPLICATION To complete your scholarship application, you have two options: 1. You can type your responses directly on to the attached application form and checklist (handwritten applications will not be accepted) and turn in, or 2. Access the application online at www.elkhorn.k12.wi.us. Click on the High School tab, then click on Guidance on the left side of the page, then click on EASD Scholarship Foundation click on Scholarship Application and complete the application. You will not be able to save the information and return to it, so plan enough time to complete it in one sitting, or print a blank form and type in the information at your convenience. Applications must be typed! Hand written applications WILL NOT be accepted! SELECTION OF SCHOLARSHIP RECIPIENTS Scholarship recipients are selected according to the desires of the scholarship sponsor. The sponsoring group or individual may select the individuals or they may designate that the Elkhorn Area High School Scholarship Committee do this. This Scholarship Committee is composed of faculty representatives and one representative of the EAHS/Scholarship Foundation Committee. This committee reviews all of the applications for each scholarship, selecting the most qualified student to receive the award.
Important Note* In order for a student to be eligible for scholarships sponsored through or by the District, he/she shall have been a student in the Elkhorn Area High School for the last four full semesters before graduating from Elkhorn Area High School. Students who transferred into Elkhorn Area High School after the start of their Junior year are not eligible to receive any scholarships that are sponsored by and selected by the District, but can apply for and receive scholarships that are sponsored by and selected by community based organizations or individuals not associated with the District. EAHS SCHOLARSHIP FOUNDATION The was organized and is operated exclusively to promote higher education through the granting of scholarships and financial assistance to students and former students of Elkhorn Area High School. Scholarships from donated monies have been established and are listed within this book. Other Foundation Scholarships will be announced at Honors Night. PAYMENT OF SCHOLARSHIP MONIES If you receive a scholarship at Honors Night, you will receive directions on who to contact to collect your award. Most monies will be available in mid-august after you have registered so that you may show proof of registration. You will also be given information on who to thank. We appreciate the support of our scholarship sponsors and believe it is crucial to the continued success of this program that you send a Thank you note to the sponsor of your specific scholarship. If you receive a scholarship and later change your plans about attending school or your major, please notify your scholarship sponsor and Mr. Bill Trewyn at the Elkhorn Area School District Office immediately. You may be able to delay using your scholarship money according to the following: Scholarship recipients will be provided the opportunity to delay receipt of scholarship monies. The monies will be retained by the Elkhorn Area High School Scholarship Foundation and placed in an escrow account in the recipients name. Scholarship recipients will receive only those monies designated in the original award. Scholarship monies placed in an escrow account will be subject to the following time lines: 1. If a recipient enrolls in a four-year institution, escrow monies must be claimed within four years of the initial day of the recipient s freshman year. 2. If a recipient enrolls in a technical college, escrow monies must be claimed within two years of the initial day of the recipient s enrollment in the technical college. 3. If a letter is submitted to the Scholarship Foundation requesting a delay of payment on scholarship monies and citing specific circumstances, the request will be decided on a case-by-case basis. Income generated from monies placed in escrow will be retained by the Elkhorn Area High School Scholarship Foundation. Any unclaimed scholarship monies resulting from one through three above will become part of the corpus of the Elkhorn Area High School Scholarship Foundation. *** Please be advised that the Scholarship Foundation reserves the right to distribute scholarship money and make adjustments to funds as needed.
AURORA HEALTH CENTER-ELKHORN SCHOLARSHIP A scholarship of $1,000 will be awarded to a graduating senior planning to continue his/her education in a health career field. Consideration will be given to such career areas as medicine, nursing, physical therapy, occupational therapy, x-ray technology, nuclear medicine technology, medical laboratory, and others. Half of the money will be awarded the first year and the other half will be awarded the second year of school. Faculty Committee of Aurora Health Center AURORA LAKELAND MEDICAL CENTER/MEDICAL STAFF SCHOLARSHIPS Two (2) scholarships in the amount of $1,000 each will be awarded to graduating seniors planning to continue his/her education in a health career field. Consideration will be given to such career areas as medicine, nursing, physical therapy, occupational therapy, x-ray technology, nuclear medicine technology, medical laboratory, and others. The scholarships will be awarded to the recipients at the beginning of the second semester of the student s freshmen year with proof of passing grades and the intent to continue with their schooling. Selection by Barbara Jane Ward Morrissey Scholarship The purpose of the Barbara Jane Ward Morrissey Scholarship is to promote education though the granting of scholarships and awards to the students of the Elkhorn School District. Two (2) awards will be given, each in the amount of $2000. This is a one-year cash award. The following criteria are considered in awarding the scholarship: Recipient must be a graduating senior ranked in the upper half of his/her class. Plans to enroll at an accredited technical, vocational (two or four years), or a four-year college or university Will be judged on financial need, and thereafter on promise as a student, but not rank in top 15% of graduating class of the Elkhorn Area High School. CHIROPRACTIC CENTER OF ELKHORN MUSIC SCHOLARSHIP This $300 scholarship will be awarded to a student planning to pursue a major in vocal or instrumental music. Chiropractic Center of Elkhorn Sponsor
DAN ERSKINE MEMORIAL SCHOLARSHIP This scholarship of $500 is dedicated to Dan Erskine, a 2003 graduate of EAHS. He was keenly aware of current events and political issues, and wrote for WISN as a student contributor from UW Whitewater about social and political issues affecting the US and the world. He was pursuing a double major in sociology and political science, and aspiring to continue on and become a professor at the university level. Dan was a gifted intellectual, a talented athlete, and a great friend. The applicant for this scholarship must also share these three traits. Dr. Joseph Sweeney DANIELS SENTRY OF ELKHORN SCHOLARSHIP Daniels Sentry of Elkhorn will present one (1) $500 scholarship to a senior continuing his/her education. Recipient must be a current or past employee of Daniels Sentry of Elkhorn, WI. Daniels Sentry of Elkhorn Committee E.A.H.S. ALL SPORTS BOOSTER CLUB SCHOLARSHIPS The E.A.H.S. All Sports Booster Club are offering four (4) scholarships of $1,000, to graduating senior athletes. The recipients of these scholarships must meet the following criteria: Must be a graduating senior. Must be enrolled in a minimum one-year program within a year following graduation. Must have a minimum of three letters earned during high school sports participation. Must be present at the E.A.H.S. All Sports Booster Club Golf Outing to receive your scholarship. Other criteria that will be considered include: adherence to the athletic code, total number of letters earned in relation to scholastic achievement, financial need, applicant s interpretation of sportsmanship, and applicant s assistance with Booster Club Fund-Raising Events (fair parking, car wash, concessions, etc.) Application: packet E.A.H.S. ELKHORN AREA CHAMBER OF COMMERCE AND TOURISM CENTER SCHOLARSHIP The Elkhorn Area Chamber of Commerce and Tourism Center is pleased to offer (1) $500 scholarship to a graduating senior that meets the following criteria: One (1) scholarship in the amount of $500 will be given to a student planning on attending a technical college for the 2013-2014 school year. The student must have a GPA of 2.5 or higher. Elkhorn Area Chamber of Commerce and Tourism Center Committee Sponsor ELKHORN AREA WOMEN S CLUB SCHOLARSHIPS Scholarships of $1,000 each are awarded to students who are continuing their education. These are open scholarships selected on the basis of scholastic achievement, financial need, character, and community involvement. The student will receive the monetary payment upon satisfactory completion of the first semester at an accredited college, university or school of higher education; and by attending a hosted dinner meeting of the Elkhorn Area Women s Club; traditionally on the second Tuesday of January. Elkhorn Area Women s Club Sponsor- Will be awarded at the dinner meeting ELKHORN CHEMICAL & PACKAGING/ MARY ANN BUTLER MEMORIAL SCHOLARSHIP Elkhorn Chemical & Packaging is pleased to award one (1) scholarship in the amount of $500 in memory of Mary Ann Butler. The recipient of this scholarship must meet the following criteria: Pursuing a post high school education at a college, university, technical or trade school Student must be able to demonstrate financial hardship and substantial financial need. Award will be distributed upon proof of 1 st semester grades. Elkhorn Chemical & Packaging Scholarship Committee Sponsor- Checks will be sent to the school student will attend. ELKHORN EDUCATION ASSOCIATION (EEA) SCHOLARSHIPS The EEA awards two (2) scholarships in the amount of $750 each to students seeking teaching certification in a University s College of Education. Recipient must have a GPA of 3.0 or higher. Committee of Elkhorn Education Association Members
ELKHORN F.F.A. ALUMNI SCHOLARSHIPS The Elkhorn F.F.A. Alumni wishes to present three (3) scholarships to students planning to continue their education with career interests in Agricultural Production, Processing, Marketing, Research, Education, Agricultural Technologies, or Agricultural Science are preferred, but all career interests will be accepted. One (1) scholarship in the amount of $500 will be given in memory of Rick Walter to a student planning to attend a 4-year college program. One (1) scholarship in the amount of $400 will be given to a student planning to attend a twoyear technical college. One (1) scholarship in the amount of $300 will be given in memory of Charles Papcke to a student planning to attend an agricultural short course. Elkhorn FFA Sponsor ELKHORN KIWANIS CLUB - ANDY OLESON, MUD EAMES, JIM DOW AND DR. NORBERT SABIN SCHOLARSHIPS These four (4) open scholarships are $500 each. The students chosen are active and concerned individuals who plan to continue their education, and who are in need of some financial assistance. ELKHORN KIWANIS CLUB - DR. HENRY MOL SCHOLARSHIP One (1) scholarship in the amount of $500 will be presented to a graduating senior, who has demonstrated the capacity and desire to serve others, but more importantly appears to have the potential to continue to unselfishly serve others in whatever career he or she chooses to pursue. Preference to a student who is in need of some financial assistance. ELKHORN LIONS CLUB SCHOLARSHIPS This organization promotes its interest in and concern for special education by presenting up to
three (3) $1,000 scholarships to students who choose to further his/her education in this field or in the field of general education. Recipients must be in the upper 1/3 of his/her graduating class. The students will receive the scholarship upon satisfactory completion of their first semester, and proof of registration for their second semester. Lions Scholarship Committee Sponsor ELKHORN MEMORIAL VFW SCHOLARSHIP One (1) scholarship in the amount of $500 will be awarded to a student who meets the following criteria: Relative (has a parent, brother, sister, aunt, uncle or grandparent who is a veteran). Student must have a known history of community service. Student must be entering college and is able to demonstrate financial need. ELKHORN PENGUINS SWIM TEAM One (1) scholarship in the amount of $500 is to be given to someone selected by the Penguins Scholarship Committee. Candidates must have swam for the penguins swim team. Penguins Scholarship Committee ELKHORN ROTARY CLUB SCHOLARSHIPS One (1) scholarship in the amount of $750 is being given in memory of Susan Davey, a former member of the Elkhorn Rotary Club. This scholarship will be awarded to a student planning to attend a Technical College. Students do not need to be majoring in any specific field to qualify. One (1) scholarship in the amount of $750 is being given in memory of Ray Gruman, former Principal of Elkhorn Area Middle School. To be eligible the candidate should have participated in a variety of co-curricular activities, which include, but are not limited to athletics, (letter preferred), music, and leadership activities. This award will be presented to a graduating senior continuing his/her education at a four-year college. One (1) scholarship in the amount of $1000 will be awarded to a senior planning to continue his/her education at a four-year college. The selection will be made to a student who has participated in community service and who exemplifies Service above Self. Ellen Holly Scholarship
One (1) $500 scholarship will be awarded to a student with financial need who has demonstrated an outstanding commitment to serving their community through volunteering. Ellen Holly FIRST CONGREGATIONAL UNITED CHURCH OF CHRIST WOMEN S FELLOWSHIP SCHOLARSHIPS Two (2) scholarships in and amount up to $500 are available to graduating seniors who meet the following criteria: Must be accepted at an accredited college, university, or other institution of higher education. Must show evidence of a sincere interest in continuing education with a reasonable educational goal in mind. A student who has demonstrated conscientiousness in fulfilling academic requirements and must have a grade point average of at least 2.0. A student who has developed characteristics of accountability, integrity, and the ability to make positive moral judgments. $300 will be paid upon acceptance of enrollment for the fall semester. $200 will be paid upon satisfactory completion of the first semester and proof of enrollment for the second semester. First Congregational United Church of Christ Women s Fellowship Sponsor FIRST UNITED METHODIST CHURCH OF ELKHORN SCHOLARSHIPS First United Methodist Church of Elkhorn is pleased to be awarding scholarship(s) in the amount of $500 to high school seniors who plan to continue their education at any college, vocational or technical school. Recipients shall be determined on their church involvement and scholastic performance. Applicants must be a current member of the First United Methodist Church of Elkhorn. First United Methodist Church Scholarship Committee Sponsor. Checks will be mailed directly to the university / tech school. GETZEN COMPANY SCHOLARSHIP The Getzen Company is pleased to offer one (1) scholarship in the amount of $1,000 to a graduating senior planning to major in Music Education. Preference would be given to a student pursuing band or
instrumental music, however vocal music would be considered. Financial need will be considered in the selection process. GRIEBEL-WARD-MORRISSEY MEMORIAL SCHOLARSHIP One (1) scholarship of $1,500 will be given in memory of the Griebel-Ward-Morrissey Family to a graduating senior who understands the value of education and has demonstrated a strong work ethic. The student should have been involved in extra-curricular activities and community service. Although all applicants can apply, preference will be given to a student who meets the following criteria first: 1) Student is pursuing the field of Speech/Language Pathology. 2) Student will be attending Marquette University. 3) Student must be in the upper 25% of the class and has been accepted by the University/College of choice. Selection By: Payment By: HARLAN AND FRANCES LANEY ENDOWMENT SCHOLARSHIP One (1) scholarship in the amount of $500 will be given in the memory of Harlan and Frances Laney. This scholarship is open to any senior pursing further education with special emphasis on students who have demonstrated caring, concern, and service to others. Selection By: Payment By: JAMIE SENDT MEMORIAL SCHOLARSHIP/LAUDERDALE-LAGRANGE FIRE DEPT. This organization promotes its interest in fire science and emergency medical services by presenting two (2) $500 scholarship to a senior that chooses to further his/her education in either of these fields. Students pursuing the field of nursing may also be considered. Selection for the award will be based upon the student s overall high school record, community involvement, character and financial need. The student will receive the scholarship upon satisfactory completion of his/her first post-high school semester with a grade point average of at least 2.5 on a 4 point scale (or equivalent) and proof of registration for the second semester. Lauderdale LaGrange Fire Department JEAN F. AND MARY E. MORRISSEY MEMORIAL SCHOLARSHIP One (1) $500 award will be presented to a graduating senior continuing his/her education. The recipient will have a financial need, and plans to attend a vocational/ technical or community college. The student
will have a goal of pursuing a career in education or communication. JEAN F. AND MARY E. MORRISSEY MEMORIAL SCHOLARSHIP One (1) $1500 award will be presented to a graduating senior continuing his/her education at a four-year college, pursing a degree in Elementary Education, English or Journalism. The student must have demonstrated good curricular and extracurricular records, must show a history of community involvement and demonstrate a financial need. Special consideration will be given to students who have shown an interest in fine arts/music and drama. JOHN U. KARPEN EDUCATION FUND One (1) $500 award is being offered to any EAHS graduating senior choosing to further their education in the field of science, math, agriculture or medicine. Financial need will be considered. JON ERIC JACOBSON FUND SCHOLARSHIP The Jon Eric Jacobson Scholarship Fund in the amount of $1,000 will be given annually to a student entering a four-year college preparatory program in English education. Special consideration will be given to those students who have shown an interest in fine arts, especially music and drama. In the event that no qualified English education applicant is available, consideration may be given to students interested in pursuing a teaching career in one of the related fine arts. Students applying for this scholarship should have at least a 2.0 grade point average. (The scholarship award will be divided over a two-semester period. In the event that a qualified recipient is not available in a given year, the scholarship will be presented to a former EAHS graduate who is already pursuing a degree in English education). Jon Eric Jacobson Memorial Committee KATE WILSON MEMORIAL SCHOLARSHIP One (1) $500 scholarship will be given in memory of Katherine (Kate) Wilson to a senior who wishes to pursue a career as a dental hygienist. This scholarship will be presented upon evidence of completion of
one semester in a dental hygiene program at an accredited technical college and proof of enrollment for second semester, with preference being given to a student wishing to attend Milwaukee Area Technical College (MATC). In the event that a qualified recipient is not available in a given year, the scholarship will be presented to a former EAHS graduate who is already pursuing a degree in dental hygiene or a career in the dental field. Academic achievement and participation in extracurricular activities will be considered. The Wilson Family Sponsor KIKKOMAN FOODS FOUNDATION, INC. SCHOLARSHIP This scholarship of $3,000 is sponsored by Kikkoman Foods Foundation, Inc. of Walworth, and is presented to a graduating senior planning to continue his/her education. Selection for the award will be based upon the students over all high school record. LES JOHNSON MEMORIAL SCHOLARSHIP The Les Johnson Memorial Scholarship is given in memory of Les Johnson, former President of the Production Credit Association of Elkhorn. This year, one (1) $1000 or two (2) $500 scholarships will be awarded to a recipient(s) who has grown up on a farm or is pursuing a career in production agriculture, technical agriculture, agri-business field, or a career in special education. The educational program could include a four-year program, or short course. Selection of the recipient will be by a special scholarship committee. The amount of the award and number given shall vary from year to year, and it will be made available to the student once he/she has completed their first semester in good standing with their educational institution. Les Johnson Memorial Committee MATHESON MEMORIAL LIBRARY FOUNDATION SCHOLARSHIP Matheson Memorial Library Foundation is pleased to offer one (1) $500 scholarship to a student who intends to pursue a degree in Library Science. If no such applicant is available, consideration would be given to an individual with demonstrated interest in Literature or English, or teaching in a field related to Literature or English.
Payment By: 2013 MEDAL OF HONOR AWARDS You, the nominator, have the opportunity to recognize one of your classmates for their display of courage, kindness, and concern for others. If you know of a classmate who has done something out of the ordinary for another person or the community, please fill out the nomination form you will find in the back of this packet. If your nomination is selected, you will receive $50 and your statement will be read on Honors Night. The person you wish to be recognized will receive $500. Nomination by: Application: packet Any graduating member of the senior class 2013 Medal of Honor Nomination Form, which is provided in the back of this NICK VAN DYKE MEMORIAL SCHOLARSHIP One (1) $1000 scholarship will be awarded to an Elkhorn Area High School graduating senior who is active in the Automotive Mechanics classes and programs, and intends to pursue a Mechanics Program or related field upon graduation. PEOPLES BANK SCHOLARSHIPS Two (2) $500 scholarships are sponsored by Peoples Bank and are to be given to graduating seniors planning to continue his/her education in a financial related degree. Selection will be based on academic achievement, community service and financial need. Recipients may not be related to a Board Member or an employee of Peoples Bank. ROBERT CHRISTENSEN MEMORIAL One (1) $500 Scholarship will be presented to a graduating senior with the following criteria: Recipient should have participated in sports, especially tennis, and have a good sense of humor. ROBERT E. HEISTAD MEMORIAL SCHOLARSHIP Three (3) $1500 scholarships will be offered to any graduating senior choosing to continue their education. Financial need will be considered. Payment By: SCOTT P. JONES MEMORIAL SCHOLASHIP One (1) $1000 scholarship will be awarded to a graduating senior who plans to further his education with a degree in business. Preference will go to a student planning to attend UW- Eau Claire. SOUTHERN LAKES MASONIC LODGE #12 SCHOLARSHIPS The Wisconsin Masonic Foundation/Southern Lakes Lodge #12 is offering two (2) $1,000 scholarships this year. Criteria for selection are as follows: Award based on the application submitted by a senior from an accredited Wisconsin High School who is planning on attending a four-year college, technical school or a junior college. Applicants will be judged on their promise as a student and on service to their school or community. This scholarship also recognizes student potential which should be rewarded. The recipient shall be selected by the Scholarship Selection Committee based on this criteria provided by Southern Lakes Masonic Lodge. Preference may be given to students with a Masonic affiliation in their families, such as Eastern Star, DeMolay, Job s Daughters, Shrine, Blue Lodge, York or Scottish rite and others. You must list your affiliation and indicate Lodge or Chapter, if known. JOHN S LUTHERAN CHURCH SCHOLARSHIP A Scholarship Fund is maintained by St. John s Lutheran Church for the purpose of aiding in the education of selected members of the congregation. Each year a scholarship(s) of $500 will be awarded to a graduating senior(s) from an area high school based upon the following considerations: The student must be an active member of St. John s Lutheran Church of Elkhorn. Must have maintained a 2.0 or above grade point average. Be actively seeking a post high school education at an accredited school. Attend worship regularly and be active in congregational activities (for example: Youth Group, Nursery attendant, Sunday school teacher or aide, Usher, Reader, Choir member, Musician).
Write an essay of approximately 150-250 words relating their Faith to their education goals and their future plans. Sponsor- Check will be sent directly to the university. ST. PATRICK S /ELKHORN KNIGHTS OF COLUMBUS SCHOLARSHIPS Up to six (6) scholarships of $500 each are sponsored by the St. Patrick s/elkhorn Knights of Columbus and are to be presented to graduating seniors planning to continue their education as follows: $500 Up to three (3) graduates who graduated from St. Patrick s Day School and are members of St. Patrick s Parish and have been confirmed in the Catholic Church prior to making application for this scholarship. $500 Up to three (3) graduates who are registered members of St. Patrick s Parish and who have been confirmed in the Catholic Church prior to making application for this scholarship. Applicants need not be graduates of St. Patrick s Day School. Recipients of awards must begin their continuing education within two years after notification of the award. St. Patrick s/elkhorn Knights of Columbus Committee Members STEVE DURKIN MEMORIAL SCHOLARSHIP The Steve Durkin Memorial Scholarship was created by Gateway Technical College staff to honor the memory of their dedicated co-worker, Steve Durkin, a long time Gateway counselor. Two (2) scholarships in the amount of $1500 will be awarded to a graduating senior who plans to attend a technical college. The scholarships will be presented upon proof of enrollment at a technical college. SUGAR CREEK LUTHERAN CHURCH SCHOLARSHIP The Sugar Creek Lutheran Church will award a minimum of (1) one scholarship in the amount of $500, based upon applications submitted by seniors who plan to continue their education at any college or vocational/technical school. Applicants shall be judged on their church involvement, financial need and scholastic performance. Applicants must be a member of Sugar Creek Lutheran Church. Sugar Creek Lutheran Church Committee
THELMA ADAMS MEYER SCHOLARSHIP One (1) scholarship in the amount of $1,500 is available to a graduating senior who meets the following criteria: A student who will be attending a two-year technical or vocational school. A student who has achieved a 2.0 GPA or higher. A student who has demonstrated an economic need that could keep the recipient from pursuing post-high school education. A student who will be employable in learning a career or trade. VIP KIWANIS AKTION CLUB SCHOLARSHIP The VIP Aktion Club would like to honor a student who represents the qualities of excellent character, a strong work ethic, determination and perseverance and is highly regarded by his/her peers and the staff. This student would be awarded with one (1) scholarship in the amount of $500 WALWORTH STATE BANK One (1) scholarship in the amount of $1,000 will be given by the Walworth State Bank to a deserving senior who plans on attending a four year college or university.
