Scholarship Number: 271 Amount of Each Scholarship: $1,000 Forrest Goetsch Memorial Scholarship Dedicated to recognizing outstanding achievement by youth who have helped others through difficult times or who have personally overcome challenges (medical, educational, family, or economic) in their life. Number of Scholarships to be Awarded: Varies scholarships will be awarded to most qualified applicants from all area schools. Forrest Goetsch - August 22, 1995 September 21, 2009 BACKGROUND: Many people know Forrest Goetsch as the 14-year old D.C. Everest student who collapsed and died during warm-ups at cross-country practice on September 21, 2009, but that is just the sad conclusion to a story that ended way too soon. Forrest Goetsch was born on August 22, 1995 at St. Joseph s Hospital in Marshfield, WI. He was born 7 weeks before his due date and spent over a month in the neonatal ICU in Marshfield before coming home. That was the beginning of a long relationship Forrest would have with doctors and hospitals. Forrest was also born with a rare facial deformity. Before Forrest was old enough to start kindergarten, he had numerous jaw surgeries. Despite the medical challenges that Forrest had to face in his life, he had great perseverance, dedication, determination and motivation to succeed in life, school and sports especially running. He never felt sorry for himself about the challenges and surgeries that he would have to face in the future. He looked forward to the future and dreamed of going to college at Duke and playing basketball for Coach Krzyzewski. Forrest made many friends during his life and was loved by all who let him into their lives. Forrest truly lived each day to the fullest and had a wonderful sense of humor. He loved to tease his family and friends and play practical jokes. Forrest loved sports and spent much of his free time playing soccer, basketball, golf and running. He also enjoyed the outdoors as well as hunting and fishing with friends and family. There are many things that we have learned from Forrest in his short time with us, the most important of these being Live your life everyday without feeling sorry for yourself and always take time to tell the people you love that you love them everyday. This scholarship fund was created through the contributions of many donors, to perpetuate the memory of an outstanding young boy who lived life to the fullest with no regrets. Because Forrest s life here on earth had such a profound impact we wanted to celebrate his life and honor his memory in a permanent fashion. Forrest Goetsch Charities a 501 (C)(3) is dedicated to recognizing outstanding achievement in our communities by youth and/or organizations who have helped others through difficult times or who have personally overcome challenges in their lives. Forrest Goetsch Charities Board Members and their committees are responsible for raising funds for this memorial. Forrest Goetsch Memorial Scholarship is dedicated to recognizing outstanding achievement by youth who have helped others through difficult times or who have personally overcome challenges (medical, educational, family, or economic) in their life.

Forrest Goetsch Memorial Scholarship (continued) Member of this year's Mosinee High School graduating class, who will attend any accredited college, university, or technical school in the United States. Financial Need will NOT be a determining factor. To receive scholarship payment the recipient must enroll in any accredited college, university, or technical school in the United States, and provide proof of admission and payment of fees to the appropriate High School Official or to Forrest Goetsch Charities. (Less than one-year programs will not be considered) Please include with your application a typed paper, double spaced, and use font Times New Roman 12 describing how you have helped others through difficult times or how you have personally overcome challenges in your life. The first and last name of the applicant must appear on the upper right corner of each page. Failure to follow guidelines may disqualify your application The annual deadline for this application is March 11th. The applicant gives permission to Forrest Goetsch Charities to publish their essay for means deemed necessary by Forrest Goetsch Charities Board of Directors. Forrest Goetsch Charities Board of Directors shall establish all criteria for this scholarship and determine who is to receive this scholarship. Their decision is final. They reserve the right to change any or all aspects of this scholarship. Forrest Goetsch Scholarship committee will review all qualifying applications with the final decision coming from the Board of Directors of Forrest Goetsch Charities. Scholarship monies, if Mosinee applicant(s) is chosen, will be awarded at the Senior Recognition Night in the form of a check made out to the recipient and the post secondary school they will be attending. Unused Scholarship: If recipient does not attend school, money will be given to the alternate.

Garald Emerich Memorial Scholarship Scholarship Number: 111 Contributor: Garald Emerich Memorial Fund Amount of Each Scholarship: $500 Number of Scholarships to be Awarded: One This scholarship will be presented to an individual pursuing a career in education such as teaching, counseling or administration. Applicant must plan to attend on a full-time basis. (12 credits or more) Scholarship may be used at any accredited college including UW Centers. Applicant must have a scholastic average of C+ (2.500) or better. Preference will be given to those who have taken a very rigorous college preparation curriculum. Member of this year's Mosinee High School graduating class. Applicant must complete a General Scholarship Application, checking this scholarship under Education Scholarships and attach a one page typed essay detailing why you have chosen the field of education, what teaching experience you have had and why you think you would make a good teacher; second semester schedule and return it to the Student Services office by the due date. Applicant may be asked to have an interview with the Local Scholarship Committee. Recipient will be selected by the MHS Education Screening Committee. Scholarship monies will be awarded at the Senior Recognition Night in the form of a check made out to the recipient and the post secondary school they will be attending. Unused Scholarship: If an applicant isn't selected or if the applicant does not attend school, the money will be used next year.

Helen and Oscar Rockman Memorial Scholarship Scholarship Number: 120 Purpose of scholarship: In memory of the deceased members of the Rockman Family Amount of Each Scholarship: $1000 Number of Scholarships to be Awarded: One Applicants should have an average GPA of 3.0 or better. Students with lower averages may be considered. Applicant plans on attending an accredited college or post-high school institution during the next school year and meets all entrance requirements for that school. Member of this year's Mosinee High School graduating class. Financial need may be a consideration for this scholarship. Each applicant will be required to fill out a General Scholarship Application, checking this scholarship under General Scholarships. Students also need to include a Financial Need form and return both to the Student Services office by the due date. Selection of the recipient will be made by the Donor. Scholarship monies will be awarded at the Senior Recognition Night in the form of a check made out to the recipient and the post secondary school they will be attending.

Herb Kohl Educational Foundation Health Scholarship Amount of Each Scholarship: $150 Number of Scholarships to be Awarded: One Applicants need to be pursuing a career in teaching, specifically in the health field: i.e. School Health, Public Health or Health Education, and/or a health care related field, special education/adaptive PE/coaching. Applicant must have a cumulative GPA of 3.25 or better. Applicant must be accepted and plan on attending a 4 year accredited university on a fulltime basis after graduation. (12 credit minimum) Member of this year's Mosinee High School graduating class. Each applicant will be required to fill out a General Scholarship Application, checking this scholarship under General Scholarships, and return it to the Student Services office by the due date. Applicants must also attach a one page, 200 word minimum, essay detailing why he/she chose a health field and describing career goals. Selection of the recipient will be made by the Donor. Scholarship monies will be awarded at the Senior Recognition Night in the form of a check made out to the recipient and the post secondary school they will be attending.

JAMES T. SCHREMP MEMORIAL SCHOLARSHIP FUND a component fund of the Amount of Each Scholarship: $1000 per year up to four years ($4,000 total) Number of Scholarships to be Awarded: PURPOSE OF THE FUND: One The James T. Schremp Memorial Scholarship Fund was established within the Community Foundation of North Central Wisconsin shortly after his death on August 14, 2000. Family, friends and colleagues established the memorial fund as a means of honoring Jim s memory through the awarding of scholarship funds to graduating scholar / athletes from Mosinee High School. In administering this fund, we honor the qualities of his life that defined him. The student is eligible to renew their scholarship during their undergraduate studies. To qualify for continued funding, the student must remain in good academic standing and provide proof of registration and a transcript of their grades to the Foundation each semester. In addition, they must submit an annual update of their progress each year. APPLICATIONS AND ELIGIBILITY: Students are encouraged to apply through the Community Foundation s online application site at www.grantinterface.com/cfoncwscholarships/common/logon.aspx. Completed applications must be submitted by March 11 to be considered by the Selection Committee. Information will be held in the confidence of the Selection Committee, the Community Foundation s board of directors, and its staff. Candidates for this scholarship must meet the following criteria: First Choice: A graduating senior scholar-athlete from Mosinee High School Accepted for enrollment at the University of Wisconsin- Madison Planning to major in a business-related field Second Choice: A graduating senior scholar-athlete from Mosinee High School Accepted for enrollment at the University of Wisconsin-Madison Planning to major in any field of study Third Choice: A graduating senior scholar-athlete from Mosinee High School Accepted for enrollment at any University of Wisconsin System college or university Planning to major in a business-related field

JAMES T. SCHREMP MEMORIAL SCHOLARSHIP FUND (CONT.) SELECTION GUIDELINES: A Selection Committee, named by the Community Foundation, will review applications based on the following guidelines. Applicants must demonstrate: Academic excellence Involvement in high school athletics Characteristics, qualities, and attributes that display potential for success Need is not a criteria for selection. SELECTION COMMITTEE: The Selection Committee appointed by the Community Foundation will meet in April to review all applications. All applicant information must be kept in the strictest confidence and must not be discussed outside the committee. The Selection Committee may include, but is not be limited to: Representation from the Schremp family Representation from the community at large Representation from the Community Foundation Members of the immediate families of any of the Selection Committee members shall be eligible to apply for scholarship funding, with the provision that the related member not serve on the committee during the year in which the application is considered. ANNOUNCEMENT AND PAYMENT: Foundation staff will contact Mosinee High School to ensure announcement of the scholarship recipient is included in their Senior Awards Ceremony to be held generally in May. The Schremp family will name a presenter to award the scholarship at the ceremony; the Community Foundation, or a school representative, are willing to make the presentation if a family representative is not available. The recipient will receive a congratulatory letter from the Community Foundation after the awards ceremony, instructing them on how to receive their scholarship funding. Scholarship funds, in the amount of $1000 per year up to four years ($4,000 total), will be paid by the Community Foundation directly to the school the student will be attending. The student must provide proof of registration to the Community Foundation office for payment to be initiated. Jen & LVO Memorial Scholarship Scholarship Number: 203 Contributor: Elvio Misoni s Family and Friends Amount of Each Scholarship: $500 Number of Scholarships to be Awarded: One Applicant must be accepted at the college or university they plan on attending. Applicant should have a GPA of 2.0 or better. Students with lower averages may be considered. Scholarship may be used at any accredited school. Financial need will not be a determining factor. Member of this year's Mosinee High School graduating class. Applicant must complete a General Scholarship Application, checking this scholarship under General Scholarships Must also attach a typed essay on My Greatest Life Experience and return both to Student Services office by the due date. Selection of the recipient will be made by the Donor. Scholarship will be given to recipient at the Senior Recognition Night in the form of a check made out to Recipient and the University/College they will be attending. Unused Scholarship: Donor will select an alternate if recipient does not attend college.

John & Fritzi Howland Memorial Scholarship Scholarship Number: 221 Contributors: Fritz Howland Friends & Family Amount of Each Scholarship: $250 Scholarships to be Awarded: Two Scholarship may be used at any accredited technical or four year college. Applicant demonstrates qualities of leadership. Applicant must pursue a post-secondary education in any field. Member of this year's Mosinee High School graduating class. Applicant must complete a General Scholarship Application, checking this scholarship under Community Scholarships, and return it to the Student Services office by the due date. This scholarship will be chosen by the MHS Scholarship Committee. It is the intent of the Howland family that the scholarship be awarded to outstanding students whose accomplishments have not adequately been recognized by other means. Scholarship will be awarded in certificate only at the Senior Recognition Night. The recipient must provide proof of registration of their enrollment. Proof of registration should be sent to Sue Nelson, Community Foundation of North Central Wisconsin, 500 1st Street, Suite 2600, Wausau WI 54403. (715) 845-9555. Payment will be made directly to the post-secondary school recipient will be attending.

Kronenwetter Lions Club Scholarship Scholarship Number: 237 Amount of Each Scholarship: $500 Number of Scholarships to be Awarded: One The scholarship is to go to a student who is visually impaired. If no student is visually impaired, it is to go to a student who is physically impaired. If no student is visually or physically impaired, it is to go to a student who is going into the medical field. Applicant must rank in the top 25% of their class. No student shall receive this scholarship if they have a full ride scholarship or if they are to receive more than 3 scholarships. The Lion s may be their 3 rd. Member of this year's Mosinee High School graduating class. Applicant will be required to fill out a General Scholarship Application, checking this scholarship under Health Scholarships and return it to the Student Services office by the due date. Selection of the recipient will be made by the MHS Scholarship Committee. Scholarship will be awarded at the Senior Recognition Night in the form of a check made out to the Recipient and the University/College they will be attending.. Unused Scholarship: If the recipient does not attend school the money will be returned to the donor.

Lee Schultz Memorial Music Scholarship Scholarship Number: 264 Contributors: Bill Schultz Amount of Each Scholarship: $500 Scholarships to be Awarded: 1 Applicant must be or have been involved in music, either in the high school or community. Scholarship may be used at any accredited technical or four year college. Member of this year's Mosinee High School graduating class. Applicants must complete Lee Schultz Memorial Music Scholarship Application form and letter of recommendation and return it to the Student Services office by the due date. Selection of the recipient will be made by the Donor. Scholarship monies will be awarded at the Senior Recognition Night in the form of a check made out to the recipient and the post secondary school they will be attending. Donor is responsible for payment.

Louis & Mary Zebro Scholarships Scholarship Number: 134 Contributor: Stanley Zebro Foundation Amount of Each Scholarship: $800 Number of Scholarships to be Awarded: Two (1 male, 1 female) Applicant has been accepted for admission and plans to attend an accredited College, University or Technical Training school as a full-time student within 6 months after receiving the grant. Financial need is a prime consideration for this scholarship. Member of this year's Mosinee High School graduating class. Complete a General Scholarship Application, checking this scholarship under Community Scholarships, and Financial Need Supplemental Information Application and return to Student Services office by the due date. Selection of the recipient will be made by the MHS Scholarship Committee. Scholarship will be awarded in certificate only during the Senior Recognition Night. Recipient should contact Kristin Barry @ BMO Private Bank, Zebro Foundation, 500 Third Street Suite 302 Wausau, WI 54403, 715-845-3121, with regard to the scholarship distribution process.

Luke Kaiser Memorial Scholarship Scholarship Number: 232 Contributor: Luke Kaiser Family Members Amount of Each Scholarship: $250 Number of Scholarships to be Awarded: Two (1 music, 1 computer) Applicant must be planning on pursuing a degree in music or computers. Scholarship may be used at any accredited technical or four year college or university. Member of this year's Mosinee High School graduating class. Applicants must complete a General Scholarship Application, checking this scholarship under General Scholarships, and return it to Student Services by the due date. Selection of the recipient will be made by the Kaiser Family. Scholarship monies will be awarded at the Senior Recognition Night in the form of a check made out to the recipient and the post secondary school they will be attending. Unused Scholarship: An alternate will be chosen by the family.

Lake DuBay Lions Club Scholarship Scholarship Number: 129 Amount of Each Scholarship: $500 Number of Scholarships to be Awarded: Up to four Applicant must reside in the Lake DuBay Lions Club service area, see following page, or be a child or grandchild of a Lake DuBay Lion. Applicant must have had a minimum cumulative GPA of 2.75 on a 4-point scale. Applicant must be a student beginning his/her first year of higher education. Scholarship may be used at any accredited institution of higher learning. Student must be a full-time student. (12 credit minimum) Applicant must complete Lake DuBay Lions Club Scholarship application form and return to Student Services office by the due date. The applications of all qualifying candidates will be reviewed by the Education Committee of the Lake DuBay Lions. The committee will select the recipient of the scholarship, plus a first and second alternate. Scholarship monies will be awarded at the Senior Recognition Night in the form of a check made out to the recipient and the post secondary school they will be attending. Money may be used for tuition, room and board. Unused Scholarship: If the scholarship money will not be used by the chosen recipient, the alternate will receive the scholarship.
