Purpose of the role : To provide a professional social work perspective as a member of the multidisciplinary team, for those Tangata Whaiora referred to the service. To provide triage, crisis assessment and intervention to Tangata Whaiora. To identify and assess the psycho-social needs of Tangata Whaiora/whanau and significant others. To provide recovery focused social work interventions to those people referred to the service. To contribute to the development of an effective acute home based treatment service and provide intensive support and interventions to Tangata Whaiora. To provide effective Crisis Intervention and HBT skills as required. To practice as a social worker, providing specialist social work interventions within a Waitemata DHB Adult Mental Health Community Team Provide focused social work assessment and interventions with Tangata Whaiora as per the Adult Community Mental Health Social Work Role. Social work issues for Tangata Whaiora are highlighted to the clinical team. Social work interventions and outcomes will be recorded in clinical files, and communicated to the clinical team. To work as an effective Clinician Social Worker within the team complying with written guidelines of the service Clinical practice is carried out to professional standards and legal requirements and in line with Waitemata DHB policies and procedures. Triage, assessment and intervention is in accordance with written guidelines. Assessment and care planning including risk assessment and risk management plans are well documented, safe and in accordance with service policies and protocols. Work with Taurawhiri (Cultural Advisors) aspects that are recognised and incorporated into clinical practice. Contribute to team care coordination in clinical practice. Provide specific recovery focused interventions to designated Tangata Whaiora. To demonstrate effective communication within the clinical team. Demonstrated workload management skills are evident in prioritising demands according to urgency and acuity and responding in a flexible manner. Observe and identify roles, functions and philosophies of the multi-disciplinary team. Share clinical responsibility with the multidisciplinary team. Provide individualised, coordinated plans and interventions with a focus on providing intensive support and therapeutic treatment options Actively participate in the Wawata (multidisciplinary team), to achieve treatment goals. Participate in regular clinical and business meetings. Attend planning days. Demonstrate adherence to Waitemata DHB Policy and Procedures. Client rights and responsibilities are actively supported. Demonstrate understanding of Recovery Principles. Ensures that all administrative and HCC data entry is completed in a timely manner. Flexibility and innovation in service delivery models and practice and a willingness to be involved in research. Targeted outcomes and goals are negotiated with Tangata Whaiora and whanau centered approaches are used with identified small caseload incorporating intensive support in a flexible way. All Tangata Whaiora have a current written comprehensive plan developed collaboratively. All interventions are documented in a timely and accurate manner. Specific clinical interventions are implemented in accordance with the treatment / risk plan. All Mental Health Act documentation is completed in a tidy and accurate manner. Information is provided and referrals made to other services and agencies as relevant for addressing service user s social, occupational and other needs as appropriate. Demonstrate understanding of the MoH Involving Families Guidelines 2000.
