1 Aims T give an verview f the care and health infrmatin exchange T explain the current acceptable user agreement and hw t fllw the cnsent mdel T be able t access the system thrugh the Single Sign On functinality T flag issues and request assistance 1 What is the Care and Health Infrmatin Exchange? Launched ver a decade ag, the care and health infrmatin exchange (CHIE) is a cmputer system used by the NHS and Scial Care which safely shares imprtant infrmatin abut a patient with thse treating them. Yu can access many Hampshire patients GP recrds, Cmmunity, and hspital recrds (pathlgy reprts, X-ray r scan reprts, discharge summaries and clinic letters) n the care and health infrmatin exchange directly frm cmputers cnnected t the NHS netwrk. The care and health infrmatin exchange is currently supplied with data frm many NHS rganisatins in the Suth: Hampshire Hspitals Fundatin Trust Suthamptn University Hspitals NHS Trust Prtsmuth Hspitals NHS Trust Ryal Burnemuth and Christchurch Hspital Trust Suthern Health Fundatin Trust Slent Health Trust Hampshire Cunty Cuncil Care UK Over 95% f GPs frm: Nrth East Hampshire and Farnham, Suth East Hampshire, West Hampshire, Nrth Hampshire, Fareham and Gsprt, Prtsmuth, Suthamptn and IW CCGs. In additin, many healthcare rganisatins actively use CHIE infrmatin t treat patients. These include: Suth Central Ambulance Service GP OOH services Frimley Park Hspital Care hmes Hspices 4
2 Acceptable Use CHIE is designed t make infrmatin available fr clinical staff at the pint f care. Feeling empwered t make a decisin t access a patient recrd is a key stage in delivering direct patient care. The Acceptable Use Agreement (AUA) fr clinical users f CHIE states: I will ensure that where practical, as a care prfessinal, I will ask the patient befre accessing the CHIE fr patient care. If the patient is uncnscius r nt present but wuld benefit frm my care, I may use my judgement abut accessing the infrmatin and will recrd my reasn fr ding s. Examples that illustrate apprpriate usage in line with the abve wrding include: A member f ED clinical staff, where the patient is uncnscius r therwise incapacitated and checking their recrd is fr their benefit. In an Outpatient setting where yu have received a referral fr a patient and need mre infrmatin befre seeing/cntacting them r t help decide n best curse f actin fr the patient. As a GP when the patient is registered with yu either permanently r temprarily. This change fllws discussins with the CHIE Infrmatin Gvernance Grup, which has representatin frm all the Data Cntrllers f CHIE. GPs n that cmmittee are represented bth by the LMC and the GP Chief Clinical Infrmatin Officer, wh als chairs the grup. The new wrding has been included which is intended t supprt users when making a decisin arund cnsent t view either in the absence f the patient r where the patient is uncnscius. We hpe this advice makes it clearer when it s apprpriate fr clinicians t access CHIE fr patient care. 3 Audit Trail All access t CHIE generates an audit trail t shw the name, date/time f the access, the patient s name and is subject t audit by yur rganisatin n a regular basis. 5
4 Accessing CHIE via EMIS Web Staff wrking in GP practices that use EMIS Web can access CHIE frm within the EMIS applicatin in ne f the fllwing ways: Click the icn, pint t Care Recrd and then select the required Care Recrd mdule. If yu have custmised yur quick access tlbar, click the required Care Recrd mdule. If yu are already in Care Recrd, click the required mdule tab, r press ALT and then press the required letter. Frm anywhere else in the system, press ALT, press E, press C, and then press the required letter. On the hmepage, click the link, if cnfigured. At the left-hand side f the screen, click t expand the View pane. In the External Views sectin, click Graphnet Prtal. The Graphnet Prtal view f CHIE is launched shwing infrmatin that has been upladed abut the selected patient. An extra tab is displayed at the tp f the screen. Click Clse n the ribbn bar t clse the Graphnet Prtal. 6
5 The Landing Page / Hme Screen This is the first page that yu will see, the cntent f which is determined by yur jb rle (Rle Based Access Level). Infrmatin is displayed in sectins and accessed via icns called Tiles. Yu can drill dwn t see mre infrmatin by clicking nt the tiles r the sectins. Depending n yur Rle Based Access level, yu may als be able t navigate frm the Landing Page t additinal infrmatin. 7
6 Accessing Infrmatin CHIE uses Tiles t grup/display similar r related infrmatin frm different surces. If yu knw that the infrmatin yu want is lcated n anther tile, then yu dn t have t wait fr the Landing Page (r any ther page) t fully lad befre clicking nt a different navigatin tile. When using GP Infrmatin pages, t see anther tile s detail frm the list, just click the GP Infrmatin navigatin tile again. T find lder dcuments that dn t appear in any f the tiles, click the ACD buttn.
