Position Title: Medical Director for the Practice Division: Clinical Reports To: Chief Medical Officer Department: Medical Department FLSA: Exempt FTE: 0.4 (16 hours weekly) Position Summary: Codman Square Health Center s mission is to be a resource for the physical, mental and social well-being of our community. The Health Center strives to deliver services that support the achievement of its mission, while creating a sustainable care model that adapts to the changing healthcare environment. Through continuous quality improvement, the Health Center strives to provide high quality services and maintain optimal patient safety. Reporting to the CMO, the Medical Director for the Practice (MDP) is a practicing clinician, who is responsible for overseeing the providers in the primary care and urgent care departments. In collaboration with the other senior clinical and operations leaders, the MDP sets the tone for high quality of care, excellent customer service, and continuous improvement. The MDP supervises all Department Medical Directors, and is therefore responsible for oversight of all medical providers at the Health Center. The MDP collaborates with the senior nursing, behavioral health, and operations leaders to ensure the successful management of the medical departments and to ensure the development of high functioning, collaborative, and engaged management teams. In addition, the MDP focuses on the implementation of the clinical care model for primary care and urgent care, the professional development and performance of the medical leaders and providers, and continuous improvement of the practice to achieve the Health Center s Organizational Goals. Primary Responsibilities: Leadership and Communication The Medical Director for the Practice is the medical leader for the primary care and urgent care practices. The MDP: Articulate the organization s mission, vision, strategy and priorities. Facilitate team work, demonstrate trust and confidence in staff, and acknowledge the contributions of others. Demonstrate honesty in interactions and accountability in behavior. Model creativity and innovation. Participate in organizational strategic planning (Clinical, Operational, and Fiscal) with a focus on the medical model and clinical innovation. Work collaboratively with senior nurse, operations, and behavioral health leaders to oversee the integrated medical-behavioral health practice.
Support the work of other medical, nursing, behavioral health, and operations leaders in fulfilling the organizational strategic priorities. Oversee implementation of medical practice goals in collaboration with other organizational wide leaders, including growth planning, development of care models, quality and performance improvement, and budgeting. Represent the medical providers and serves as a conduit of reciprocal communication between the medical directors and the providers with the CMO. Foster effective interactive communication between Medical Leaders and other members of the Senior Management Team, and other Health Center Departments. Chair Medical Leadership Meeting. Co-chair Provider Meetings with CMO. Co-chair practice model implementation meetings. Attend Practice Transformation Meetings, Leadership Meetings, Provider Meetings, and Provider Trainings, and other meetings as assigned. Meet weekly or every other week with Senior Nursing, Operations and BH Leaders. Meet weekly or every other week for Supervision with CMO. Update department medical directors and providers on topics from Leadership meetings. Provide feedback from department medical directors to Leadership during meetings and through other communications. Responsible for development and implementation of health center wide provider trainings. Management of the Practice The Medical Director for the Practice collaborates with other senior managers (Nurse Director, Operations Director, BH Director) to ensure the successful management of the primary care practices, with a focus on provider clinical and administrative performance, provider mentorship, and the development of effective team based care models and protocols, specifically: Collaborate with the organizational Nurse, Operations, and BH leaders on all aspects of medical practice oversight, including: o Establishment of organizational parameters for appropriate staffing levels (providers & support staff) to support team based care and appropriate provider support. o Oversight of the development and implementation of clinical and administrative protocols and procedures. o Oversight of department budgets annually, including cost projections (e.g. position control, supplies, training) and revenue projections (e.g. projected visits, projected panel size, grants). o Resolution of issues between medical providers and staff members. Collaborate with medical directors in management of patient complaints, incidents, and occurrences involving providers. Implementation of the Practice Model The MDP collaborates with the Medical Director for ACO and Population Health, the Nurse Director for ACO & Care Management, the Senior Director of BH, and the Director for Operations and Ancillary Services to develop and implement a successful medical-behavioral health Care Model that achieves the organizational and ACO transformation goals.
Support the Medical Director for ACO and Population Health and the Nurse Director for ACO & Care Management in the development of the ACO Transformation workplan. Oversee implementation of clinical protocols and workflows to achieve organizational and ACO transformation goals. Draft selected clinical protocols and workflows as mutually agreed upon with the CMO. Obtain input from department medical directors and providers on clinical protocols and workflows. Model and mentor providers on team based care models and behaviors. Recommend new initiatives for the medical practice and ensure alignment with the organizational strategic priorities. Develop protocols for approved initiatives based on best practices and evidence-based care as mutually agreed upon with the CMO. Lead and participate in performance improvement initiatives as mutually agreed upon with the CMO. Participate in grant writing, grant review, and grant compliance as mutually agreed upon with the CMO. Ensure Department Achievement of Organizational Goals The MDP oversees provider performance on organizational clinical quality, operational, and fiscal goals through leading education, training, and feedback to providers and department medical directors. In collaboration with CMO and Medical Director for ACO and Population Health, educate department medical directors and providers on organizational goals. Oversee provider performance on organizational goals utilizing the Provider Scorecard and other analytics. Utilize provider meetings and other means of communication to inspire providers and staff to achieve organizational goals. In collaboration with department medical directors, provide feedback to providers on their performance on clinical quality, operational and fiscal goals. Provide support and mentorship to department medical directors to implement department processes, protocols, and workflows. Supervision The Medical Director for the Practice supervises the department medical directors Manage the recruitment, hiring, orientation, and training of medical directors and oversee department medical directors in the performance of the same for all medical providers. Ensure that all medical directors have sufficient management and leadership training to perform their job functions. Provide mentorship and coaching for department medical directors. Perform Annual Review (and six month reviews for new employees) for all department medical directors and support completion of reviews by department medical directors for all medical providers within established timeframes. Provide ongoing feedback to department medical directors and support ongoing feedback by department medical directors for all medical providers.
Oversee medical provider compliance with health center policies, protocols, and standards of care. Oversee medical provider compliance with Quality, Productivity & Practice Improvement goals, including Health Center Organizational Goals. Collaborate with the CMO, Human Resources, and medical directors in the disciplinary actions of medical providers. Ensure compliance with documentation of medical provider work history including disciplinary actions, payroll, mandatory training, credentialing, and performance competencies. Ensure all medical providers are appropriately licensed and credentialed. Responsible for medical director professional development and performance improvement through: o Establishing personal professional development improvement goals for each medical director. o Working with each medical director to identify their strengths and areas in need of improvement. o Mentoring, coaching, and providing training to each medical director to improve performance and develop professionally. o Mentoring medical director on Team Based Care and Teamwork. Collaborate with the CMO and medical directors to develop medical practice growth plan. Oversee Organizational Plan for Resident, Medical Student, NP student, and PA student programs. Provides professional medical services as assigned. Experience: 4 years of clinical experience preferred 2 years of management experience preferred Experience working in the Primary Care setting preferred Experience working at a Community Health Center preferred Competencies, Skills, and Attributes: Focus on health center mission, vision and goals.
