HEALTH CARE FACILITY: Ang Mo Kio Thye Hua Kwan Hospital Ang Mo Kio Thye Hua Kwan Hospital is a 360-bed community hospital located in Ang Mo Kio, Singapore. The hospital, which was officially opened in 1993, has been managed by THK, a VWO / Charitable Organisation, since 2002. As a community hospital, care is primarily focused on providing affordable rehabilitative and intermediate care for geriatric patients. MAX. NUMBER OF PARTICIPANTS: 30 2. HEALTH CARE FACILITY: Changi General Hospital With over 1000 beds serving more than 1 million residents in the East, Changi General Hospital (CGH) offers a wide range of clinical specialties and services, including (but not limited to) the Integrated Sleep Service, Changi Sports Medicine Centre, and a Clinical Trial Research Unit that facilitates biomedical research for better patient care. In 2015, a 280-bed facility, The Integrated Building was opened and is jointly operated with St Andrew's Community Hospital. The first of its kind in Singapore, it was built in support of the new 1

model of care for Geriatric and Rehabilitation services. The building includes mock-up apartments to train patients to resume daily activities so as to get them ready for home. The new Medical Centre completed in 2018, is dedicated to meet complex specialist outpatient needs. MAX. NUMBER OF PARTICIPANTS: 30 3. HEALTH CARE FACILITY: Institute of Mental Health (IMH) The Institute of Mental Health (IMH) is a 2,000-bed acute tertiary psychiatric hospital situated on a 23- hectare campus at Buangkok Green Medical Park. Set amidst serene surroundings, IMH offers a comprehensive range of psychiatric, rehabilitative and counselling services for children, adolescents, adults, and the elderly. 2

4. HEALTH CARE FACILITY: Khoo Teck Puat Hospital Khoo Teck Puat Hospital is a 660-bed hospital located at Yishun in Singapore. The hospital was officially opened by Minister Mentor Lee Kuan Yew on 15 November 2010, but began seeing outpatients and day surgery patients on 28 March 2010. Spanning over 3.5 hectares (8.6 acres) in the Yishun Central Area overlooking the scenic Yishun Pond. The hospital offers an extensive range of medical services and healthcare options for residents living in the north. 5. HEALTH CARE FACILITY: National Healthcare Group Polyclinics (NHGP) National Healthcare Group Polyclinics (NHGP) is the primary healthcare arm of the National Healthcare Group (NHG). Its six polyclinics serve a significant proportion of the population in the central and northern parts of Singapore. NHGP provides a comprehensive range of health services for the family, functioning as a one-stop health centre providing treatment for acute medical conditions, management of chronic diseases, women & child health services and dental care. The visit include an 3

introduction to public primary health care system in Singapore and guided tour of Ang Mo Kio Polyclinic MAX. NUMBER OF PARTICIPANTS: 30 6. HEALTH CARE FACILITY: National Heart Centre Singapore (NHCS) NHCS prides itself as a national and regional referral centre for cardiovascular diseases. It provides comprehensive preventive, diagnostic, therapeutic and rehabilitative cardiac services to local and overseas patients. The clinical visit will include guided tour to the specialist outpatient clinics, Cardiovascular Rehabilitation Unit, Short Stay Unit, Coronary Care Unit and Cardiothoracic Intensive Care Unit. 4

7. HEALTH CARE FACILITY: National Skin Centre (NSC) National Skin Centre (NSC) has a wide range of facilities with specialists to treat the various skin conditions. This includes two general clinics, 18 subspecialty clinics and two surgical suites Laser Suites and Procedure Suites). NSC also manages the Department of STI (Sexually Transmitted Infections) Control. A tertiary institution that provides comprehensive training to medical, nursing and allied health professionals. MAX. NUMBER OF PARTICIPANTS: 20 8. HEALTH CARE FACILITY: National University Hospital The National University Hospital (NUH) was established in 1985 and it serves as a tertiary hospital, clinical training centre and research centre for the medical and dental faculties of the National University of Singapore (NUS). NUH is also a referral centre for a wide range of medical, surgical and dental specialties including Cardiology, Gastroenterology and Hepatology, Obstetrics and Gynaecology, Oncology, Ophthalmology, Paediatrics and Orthopaedic Surgery and Hand and Reconstructive Microsurgery. A member of the National University Health System, it is the principal teaching hospital for the NUS Yong Loo Lin School of Medicine. It is a 1,160-bed tertiary hospital serving more than 670,000 outpatients and 49,000 inpatients. 5

HEALTH CARE FACILITY: National University Polyclinics National University Polyclinics (NUP), a member of the National University Health System (NUHS) is committed to providing affordable, accessible, comprehensive and coordinated primary care to all in the Western region of Singapore. NUP comprises a network of six polyclinics Bukit Batok, Choa Chu Kang, Clementi, Jurong, Pioneer and Queenstown. NUP focused on providing quality patientcentred care to our patients. Collaboration is central to the work we do, and we are developing NUP into an integrated and relationship-based primary care network that plays a key role in the community we serve. MAX. NUMBER OF PARTICIPANTS: 20 10. HEALTH CARE FACILITY: Ngee Ann Polytechnic 6

Ngee Ann Polytechnic (NP) is an institution of higher learning (IHL) in Singapore with more than 14,800 full-time and 3,000 part-time students, more than 1,800 staff, and 148,000 alumni. As an industry-oriented alternative to a broader based junior college education, polytechnic graduates in Singapore are sought after for work or many continue to complete university degrees. 11. HEALTH CARE FACILITY: Ng Teng Fong General Hospital (NTFGH) The professional site visit to the integrated 700-bed Ng Teng Fong General Hospital (NTFGH) and 400- bed Jurong Community Hospital (JCH) would last 3 hours, conducted in English. The itinerary includes a corporate presentation by a senior-level host, followed by a tour of the technology and special features of the two hospitals: NTFGH Specialist Clinic, innovative fan-shaped acute wards in NTFGH, the outdoor healing garden that connects the two hospitals, LIFE Hub which features a three-room Singapore flat mock-up, which serves to help patients ease back into their home environment, and the Mobility Park in JCH. Total walking distance is about 900m. Please be advised to wear light clothing and comfortable footwear. 7

HEALTH CARE FACILITY: Raffles Hospital Pte Ltd Raffles Hospital is a hospital built around the needs of our patients, providing specialist services combined with some of the most advanced medical technology. A tertiary care hospital, it is the flagship of the Raffles Medical Group, a leading private healthcare provider in Singapore and South East Asia. Raffles Hospital offers 24- hour emergency services, family medicine services, health screening and a wide range of multi-disciplinary specialist clinics. It is equipped with beautifully appointed wards, day surgery, angiography suites, delivery suites, operating theatres, intensive care unit, and neonatal intensive care unit. It also has its own radiology, clinical laboratory, pharmacy, dietetics, rehabilitation and Traditional Chinese Medicine departments. MAX. NUMBER OF PARTICIPANTS: 25 13. HEALTH CARE FACILITY: Singapore General Hospital With 1785 beds, Singapore General Hospital (SGH) is the first and largest hospital in Singapore. It provides affordable specialist care for patients, training for doctors and other healthcare professionals, and conducts research to bring better care to its patients. A not-for-profit institution, SGH is wholly owned by the government of Singapore and the flagship hospital of the public healthcare system. SGH is a member 8

of the SingHealth cluster of healthcare institutions. 14. HEALTH CARE FACILITY: Singapore National Eye Centre (SNEC) Singapore National Eye Centre (SNEC) is the designated national centre within the public sector healthcare network, and spearheads and coordinates the provision of specialised ophthalmological services with emphasis on quality education and research. 15. HEALTH CARE FACILITY: SingHealth Polyclinics (SHP) - Bedok Polyclinics 9

SingHealth Polyclinics, a leader in family medicine, provides seamless, patient-centred and preventive healthcare that is affordable and accessible to all. SingHealth Polyclinics' network of polyclinics provides primary healthcare services to the community. It plays an integral role in promoting a healthy lifestyle within the community and empowering families with the knowledge of common health issues, care and treatment options. Visit to Bedok Polyclinic - Introduction to SHP and guided tour to the various service areas (eg Immunisation rooms, women screening services, child health surveillance, dressing room / minor surgical procedure rooms and integrated care team) MAX. HEALTH CARE FACILITY: SingHealth Polyclinics (SHP)- Punggol Polyclinics SingHealth Polyclinics, a leader in family medicine, provides seamless, patient-centred and preventive healthcare that is affordable and accessible to all. SingHealth Polyclinics' network of polyclinics provides primary healthcare services to the community. It plays an integral role in promoting a healthy lifestyle within the community and empowering families with the knowledge of common health issues, care and treatment options. Visit to Punggol Polyclinic - Introduction to SHP and guided tour to the various service areas (e.g. Immunisation rooms, well women services, child health surveillance, dressing room / minor surgical procedure rooms and integrated care team) 10

MAX. NUMBER OF PARTICIPANTS: 25 17. HEALTH CARE FACILITY: SKH Campus (Sengkang General Hospital and Sengkang Community Hospital) The SKH campus when fully opened with a capacity of 1400 beds will form a vital part of Singapore s master plan to provide quality and accessible care to better serve the healthcare needs in North-Eastern Singapore. SKH offers a wide spectrum of specialist services and co-located with Sengkang Community Hospital (SKCH) within the campus. The integration of SKH (1000 beds) and SKCH (400 beds) in one campus allows patient to a seamless transition of care from initial diagnosis and treatment to rehabilitative care. SKH is part of the newest integrated hospital campus delivering multidisciplinary and patient-centric care covering all major healthcare disciplines. Partnerships with the National Heart Centre, Singapore National Eye Centre, National Cancer Centre, National Neuroscience Institute and KK Women s and Children s Hospital offer residents easy access to these specialty care within SKH. Patients with multiple problems will have one primary specialist who will coordinate all treatments from pre-illness to discharge back to the community. 11

18. HEALTH CARE FACILITY: St Andrew's Community Hospital (SACH) St. Andrew s Community Hospital (SACH) is a service of St. Andrew s Mission Hospital. Established in 1992, SACH was the first community hospital in Singapore set up to provide affordable intermediate medical care. We are focused on providing rehabilitation and sub-acute care to children, adults and the elderly; including those with dementia or needing palliative care. Our outpatient services include day rehabilitation, community therapy, primary care, home healthcare and other senior care services. MAX. NUMBER OF PARTICIPANTS: 20 19. HEALTH CARE FACILITY: St Luke Community Hospital St Luke's Hospital is a community hospital in Singapore that provides professional healthcare services. It was named after the patron saint of the medical profession, St Luke. St Luke's Hospital for the Elderly Sick was registered as a voluntary welfare organisation in the Registry of Societies in October 1991. It was renamed as St Luke's hospital in September 2004 to signify the hospital's role as a community hospital while retaining its focus on geriatric care. St Luke's Hospital is headed by a board of directors and various committees that oversee the daily operations of the hospital. 12

20. HEALTH CARE FACILITY : Tan Tock Seng Hospital & Renci Community Hospital/NCID Tan Tock Seng Hospital (TTSH) is one of Singapore s largest multi-disciplinary hospitals. The hospital has 45 clinical and allied health departments, 16 specialist centres and is powered by more than 8,000 healthcare staff. TTSH sees over 2,000 patients at its specialist clinics and some 460 patients at its emergency department every day. TTSH is part of the National Healthcare Group. Ren Ci is a charity healthcare institution started in 1994 with the primary mission of providing affordable medical, nursing and rehabilitative care services for the community, living to its name of compassion and benevolence. Ren Ci serves all irregardless of background, race and religion. Over the years, Ren Ci s spectrum of services have expanded to support the healthcare needs of the senior community, helping them to regain functional abilities and to return home to age in place. 21. HEALTH CARE FACILITY: Yishun Community Hospital 13

Yishun Community Hospital is one of Singapore's largest community hospitals to provide intermediate care for recuperating patients who do not require the intensive services of an acute-care hospital. Situated beside Khoo Teck Puat Hospital, the two hospitals bring about an integrated care experience for patients, through a network of healthcare teams, information flow and treatment plans. MAX. NUMBER OF PARTICIPANTS: 30 22. HEALTH CARE FACILITY: KK Women s and Children s Hospital KK Women s and Children s Hospital has evolved over the decades, since its founding in 1858, into a leader in Obstetrics, Gynaecology, Paediatrics and Neonatology. Today, the 160-year-old hospital is a referral centre providing tertiary services to handle high-risk conditions in women and children. The 830-bed hospital also runs the largest specialist training programme for Obstetrics and Gynaecology and Paediatrics in Singapore. Both programmes are accredited by the Accreditation Council for Graduate Medical Education International (ACGME-I), and are highly rated for the high quality of clinical teaching and the commitment to translational research. 14

23. HEALTH CARE FACILITY: National Kidney Foundation (NKF) NKF is a non-profit health organisation in Singapore. Its mission is to render services to kidney patients, encourage and promote renal research, as well as to carry out public education programs on kidney diseases. As of May 2018, NKF has 34 dialysis centres and Integrated Renal Centres in Singapore, with more in the pipeline. He NKF HQ houses the Kidney Discovery Centre (KDC), now comes boosted and equipped with more exhibits that make use of innovative technologies. Through multisensory storytelling, exhibits with touch screens, and even motion detectors, visitors to the centre will be taken through an enhanced, experiential learning journey disease. 15
