Purpose of the role : To provide a professional social work perspective as a member of the multidisciplinary team. To identify and assess the psycho-social needs of clients, their families / whanau and significant others. To provide recovery focused social work interventions as per the DMHS Social Work Scope of Practice. To act as Care Coordinator for an identified number of clients To provide generic mental health assessment as part of a multi-disciplinary team KEY TASKS 1. To practice as a social worker, providing specialist social work interventions within a WDHB CAHMS Team. 2. To work as an effective team member. EXPECTED OUTCOMES. Provide focused social work assessment and interventions with clients as per the DMHS Social Work Scope of Practice (Community Social Work Role). Social work issues for clients are highlighted to the clinical team. Social work interventions and outcomes will be recorded in clinical files, and communicated to the clinical team. Understand and work within Team Standard Operating Procedures. Observe and identify roles, functions and philosophies of the multi disciplinary team. Share clinical responsibility with the multidisciplinary team. Actively participate in the multidisciplinary team, to achieve integrated outcomes for clients. Participate in regular policy, quality and clinical meetings. Attend planning days. Demonstrate adherence to the National Mental Health Sector Standards. Client rights and responsibilities are actively supported. Ensures that all administrative and clinical data entry is completed in a timely manner 3. Work in partnership with clients, whanau / families and significant others. Demonstrate understanding of the MoH Involving Families Guidelines 2000. Supports involvement of and communication between the clinical team and family / whanau regarding client s progress and care plans. Identify and assess the psychosocial needs of family / whanau / significant others. Establish liaison and advocacy role to support client s in their personal relationships. Provide necessary information to assist others in meeting the client s needs. 4. Establish and maintain community networks. Identify and attend local community mental health networking groups. Maintain regular contact / consultancy with NGO s. Maintain updated information on community resources, accessible to the multidisciplinary team. Support students with agency placements. 5. Demonstrate a commitment to personal and professional development. Complete orientation and mandatory training. Participate in WDHB Performance Development Review process. Proactively maintain a professional knowledge base. Undertake project work as agreed with Team Manager and SMO Attend WDHB Social Work forum and other agreed and relevant forums related to community mental health care/services. Take responsibility for role modeling and supporting the DMHS Social Work Scope of Practice. 6. Good time management skills Preferred Degree in Social Work or equivalent Full Membership of ANZASW Professional Registration Previous employment within a mental health service agency. Experience and knowledge of working with Maori and Pacific Island clients. Experience with a child mental health team preferable Significant experience of provision of community based services.
