Purpose of the role : Manage referrals into Nasc according to the prioritisation guidelines and allocate to Needs Assessors KEY TASKS Referrals Prioritise referrals for urgency and risk. Identify when a specialized assessment is required. Contact Patients to review/confirm details in referral and ascertain current status. Respond to referrer if referral in inappropriate to NASC. Request interrai files from other DHB s or ARRC offices as required. Update patient information in momentum Manage referral spread sheet for Low risk Patients EXPECTED OUTCOMES Patients at greatest urgency and risk are prioritised and contacted first, within standard time frames. Identify when a referral for a specialized assessment is needed. Assessments will be booked to match prioritization guideline. Referrer will know whether referral is accepted. Momentum file will be available to NASC team. Overview page information will be current Low risk Patients will be allocated for assessment/review. Orientation of Community Teams Demonstrate to team members the color coding and outlook set up required Community team members outlook calendars will be maintained to a consistent standard. Community team will give access to their calendars to Leadership group, Triage and Duty roles Manage Outlook calendars for NASC Community Teams all sites Make necessary changes when there is unplanned leave Book assessments and reviews directly into outlook calendars Contact and reschedule assessments and reviews according to priority. Nasc Triage will be able to add in assessments and reviews Review Reviews support plans with Patients/Family/Whanau as planned or when a change in service is required. The review process complies with NASC standards. There is evidence of a Reassessment if there is a Waitemata District Health Board -JOB DESCRIPTION - Page 2

KEY TASKS Service Coordination Plan a package of support required to meet the needs/goals of the patient/family/whanau, as identified in the Needs Assessment. Authorise and coordinate access to non-funded and funded disability or personal health support services. Update the support plan EXPECTED OUTCOMES change in need or circumstances. There is evidence that non-funded and funded support options are explored Patients/Families/Whanau are given unbiased, standard information on all options available. The support plan reflects a match of services which meet identified patient needs, as well as any gaps in service provision. Standard Nasc authorization, referral and contracting procedure are followed. Patient/Family/Whanau choice can be evidenced. Budget Management Manage appropriate budgets both at an individual patient level and at a locality level. Documentation Maintain accurate timely recording and reporting of Patient and service data. Develop Relationships Build and maintain relationships with interdisciplinary teams, other Allied Health professionals, service providers and community groups. The cost of publicly funded service is within the SPA band allocation (unless there is signed approval from the NASC Manager). The support plan is updated to meet ongoing needs. Funded services are within budget limits, and according to the SPA band. All documented information meets corporate and service policies, procedures and guidelines. Demonstrates: Extensive knowledge of support services in the community. Evidence of working with community groups, disability and health professionals and service providers in understanding the NASC role. Exploration of provider development possibilities. Active participation sharing new resources and information. A contribution to the ongoing development to the NASC team and service. KEY TASKS Performance Management Performance management maintains, reflects and develops professional practice. Supervision is undertaken on a regular basis with a professional supervisor. Participates in peer review and case presentation forums.
