Dietetic services are delivered in accordance with professional and organisational standards, as part of a collaborative team process. These tasks include assessment, programme planning, intervention, regular review and discharge planning. The Dietitian is a member of the collaborative interdisciplinary team and works to team processes. Nutritional standards of clinical practice and safety are maintained. An agreed model of client care is used to assess, plan, implement and evaluate quality care, which may be delivered in community and clinic settings. Professional and caregiver education is provided as part of clinical practice. Regular liaison occurs with other team members and joint interventions are planned as appropriate. Evidence-based interventions are implemented in a timely, safe and effective manner in accordance with the agreed plan. Boundaries of practice are identified/recognised and appropriate referrals made as required. Regular monitoring of client interventions including progress and satisfaction is carried out. Discharge planning is timely, effective, and involves consultation with community team members and/or primary care providers as required. Integrates health promotion and client parent education to optimise self care and improve health outcomes. Develops a strong customer focus. Implements interdisciplinary care coordination with patients and families and where possible maximises the client and families control over treatment. Provides support and guidance for the client and family ensuring that they receive consistent information from the interdisciplinary team. of dietetic services. Ensures care is culturally appropriate and utilises relevant cultural support services in a collaborative and effective way. Administrative tasks are completed in a timely and efficient manner. Documentation is up to date, legible, complete and written in accordance with Waitemata District Health Board Documentation Standards. Equipment and environmental systems are maintained effectively and efficiently. Statistics are completed in accordance WDHB policies. Effective working relationships are formed with key persons in Health and other Child Services. Works within the community of the client and is responsive to this community & referral sources. Relationships are established within the cultural context of the client and family/whanau. Communication is maintained with each child s wider health/education teams. Participation in service operations occurs. Is an effective and active member of the Child Health Team. There is consistent and professional contribution and attendance at relevant staff meetings. Contributions are made to service development initiatives as appropriate. Quality of Service Follows criteria for admission to service and other related guidelines. Monitors caseload and escalates risk to line manager as appropriate. Assists with audits and caseload reviews as appropriate. Identifies and participates in the implementation of quality improvement initiatives. Establishes and maintains positive strategic and service relationships as required, e.g. with Waitemata DHB nutrition and dietetics services, and regional peers.
