Purpose of the role Provision of case management to clients who are pregnant and/or parents of children under three years of age, who are experiencing alcohol and drug issues and are poorly connected to health and social services with the aim of improving outcomes for their children. Assertive Outreach Assessment Risk assessment and management Case co-ordination in conjunction with client and relevant stakeholders Support clients to formulate and review strength based goal plans, including the involvement of family/significant others as directed by the client Support client to develop sustainable service and natural supports Actively participates within the multidisciplinary team including: Review meetings Supervision Completes non case-management tasks as requested (eg: policy review, processing referrals, attending external forums, preceptoring staff) Provide education and appropriate interventions to clients in relation to child health and development Provide advocacy, education and appropriate interventions in relation to the custody and care and protection of children. Engagement of client with PPS Client receives comprehensive assessment Risk is assessed and managed resulting in a reduction of harm to themselves, their children and others Coordinated provision of services to clients and inclusion of families/ significant others as directed by client Effective communication between professionals involved with the client Clients are able to utilise strengths and selfefficacy in order to set and achieve their goals Family and significant others are included in treatment in order to improve outcomes. Provide education and appropriate interventions to clients in relation to parenting Provide screening, appropriate consultation and referral in relation to mental health Provide education regarding Blood Borne Viruses, contraception and sexually transmitted infections and treatment options Provide education in relation to their own and their family s physical wellbeing Provide AOD assessment, diagnosis, planning and intervention within a harm reduction framework Provide assessment, education and appropriate referral for family violence issues Provide consultation and education for health and social agencies on issues relating to AOD use and the impact on pregnancy and parenting Include family/whanau and/or significant others in goal planning as directed by client PPS provides advocacy and education to other services in regard to the impact of substance use on parenting to ensure safe and realistic plans and outcomes occur Clients children live in a safe and protective environment Clients are able to access community supports and services related to housing and financial needs Client receives education regarding parenting skills, attachment and resources Client is able to parent in a safe and nurturing way Client involves family and significant others to support them in parenting safely Client engages with parenting intervention Clients receive support, education and advocacy Clients have access to appropriate mental health treatment. Client is educated about BBV s, STI s and contraception in order to make an informed choice about treatment options Clients are referred to appropriate treatment interventions and develop the ability to meet the ongoing physical health needs for themselves and their family Clients access and engage with Lead Maternity Care The harm associated with AOD use is reduced Client is able to utilise relapse prevention strategies and skills and develops insight into the impact AOD use has on their life Client is educated regarding the effects of AOD use in pregnancy and in parenting resulting in reduction of harm to the unborn and children Client and family can identify the AOD treatment supports they require and can access these Client receives education on the impact of family violence, including on children s brain development Client has a safety plan for themselves and children Client and significant others are engaged with family violence services Client develops and experiences safe relationships Raise awareness and increase knowledge of needs specific to pregnant and parenting clients in order to improve service provision Family/whanau and/or significant others are included in interventions and are able to access further treatment and support Family/Whanau is active in clients recovery Statutory & Treaty of Waitangi obligations Ensures the professional and political integrity of WDHB by carrying out all functions in compliance of Waitemata District Health Board -JOB DESCRIPTION - Page 3
