Program Manager Position Summary: Provides comprehensive planning and active supports for the care and treatment of agency clients. Coordinates the development, implementation, scheduling, and evaluation of all residential programming in assigned program. Provides case management and networks with area community providers for the benefit of the clients. Interviews and trains qualified candidates for the positions of residential case managers, residential counselors, mental health workers, volunteers, and student interns. Job Requirements: Master s degree in social work or related field plus three years experience in the mental health field. Bachelor s degree acceptable if combined with at least two years experience in human services field, staff supervision and/or general administration. To Be Determined Clinical Case Manager Position Summary: Completes screenings and intake process for new client referrals and functions as liaison for incoming referrals with clinical and external providers. Conducts ongoing face-to-face encounters while providing comprehensive planning and active supports based on unique needs of each individual. Serves as a lead to provide supervision and support to Community Support Specialists and direct care employees as assigned. Completes, submits, and monitors records based on fidelity, federal funding, or other contracting requirements. Seeks and obtain preauthorization and reauthorization of services. Networks with area community providers and provides case management supports for the benefit of the clients. Performs other duties as assigned. EOE Job Requirements: Master s degree preferred with a minimum of one (1) year experience in behavioral health providing comprehensive case management activities. Bachelor s degree considered with a minimum of three (3) years experience in related behavioral health field. Prior experience working with people with severe and persistent mental illness and/or substance abuse experience is required. Valid Connecticut Driver s license is required. Scattered site and non-24 hr. program positions require the employee to maintain reliable transportation and be willing to transport clients in personal vehicles. First Aid/CPR certification or willingness to obtain certification within 90 days of hire. Employee is responsible to maintain ongoing certification. Proficient computer skills required, including Microsoft Office and electronic record keeping. Knowledge of the community support system and resources is preferred.
