LIST OF UESTIONS ND METRO RESPONSE(S) The following questions and METRO Responses are hereby made a part of and subject to the Request for Proposals. Item uestions / nswers Would METRO consider a separate bid item for dry holes in excess of 8 deep or 4 wide for exploratory effort? Yes, but METRO will approve on case-by-case basis. Confirm that data is due 7 working days after excavation, but for how any holes? 10? 50? Yes, data must be completed within 7 calendar days, the average number of potholes per request is (10) to (30). Is there a mechanism for METRO to reimburse all or a portion of TRFFIC CONTROL or is that cost incidental to pothole bid price? 2 DDENDUM No Is there a reason an air knife is specified? Will H.P. water knife be allowed? Yes, the ir Knife is industry standard that minimizes damages to the underground utility being located. High Pressure water knife will NOT be allowed unless there is a good reason, then it MUST be approved by METRO Inspector prior to work being performed. There is some confusion on what is a SBE and what is a DBE Please clarify (currently used on other projects). n SBE is a Small Business Enterprise that may be certified through the City of Phoenix for work on locally funded projects. DBE is a Disadvantaged Business Enterprise certified through the rizona Unified Certification Program, a federal certification program, for work on federally funded projects. The City of Phoenix and its sub-recipients (METRO) are currently in a state of Race Gender-Neutral Participation with no required DBE Goals. ll DBE participation must be achieved through Race- and gender-neutral measures. How will Utility Mapping be derived (i.e. survey of Bluestake)? Mapping provided by Offeror is what I refer to. utility base map will be provided by METRO for each pothole request, there may be some field adjustments based on Blue Stake Marking in the field. METRO Inspector will make the decision in the field. Will we have a chance to amend our exceptions to RFP and sample contract if declared as non-responsive because of exceptions? No, METRO will not accept revisions to the exceptions after receipt of proposals if deemed non-responsive. ll exceptions to the solicitation / contract / scope of services, etc. must be identified with the submittal of proposal by Offeror. See also Request for Proposal, paragraph 15 Modification of Proposal and paragraph 21.C.4 wherein it states; fter presentations and discussions, Offerors may be provided an opportunity to submit a Final Revised Proposal. Remember, Final Revised Proposals are optional to the gency. Will there be a need for surveying and CD work? Barricade Company of Contractor s choice, submit traffic control plans with a Valid COT Right-Of-Way Permit / CIP Project Number to trafficbarricade@tempe.gov. These permits can be attained through the City s Engineering Department. Yes, rolling arrow board can be used. Shadow Trucks are acceptable within the City of Tempe for Potholing work. The Phoenix Barricade Manual states: 1 hour maximum per location, the City of Tempe is 30 minute maximum per location. If your Potholing takes longer than 30 minutes to complete per location, then a complete lane restriction will be required. It is METRO s intent to award to one Offeror, see Request for Proposal, paragraph 24 Contract ward wherein it states; The Contract will be awarded to the one responsive and responsible Offeror whose Proposals are most advantageous to the GENCY, on a Best Value basis, price and other factors considered. Will a fiberglass air knife be required? No, METRO does not require a fiberglass air knife. 12 Will you allow the same dig process as in Mesa? The standards are different. Contractor must adhere to City of Tempe standards. See Scope of Work, section 4; paragraph C for Tempe s website address to obtain standards. This can be: * an existing published wage determination * a survey underlying a wage determination * a Wage and Hour Division letter setting forth a position on a wage determination matter * a conformance (additional classification and rate) ruling On survey related matters, initial contact, including requests for summaries of surveys, should be with the Wage and Hour Regional Office for the area in which the survey was conducted because those Regional Offices have responsibility for the Davis-Bacon survey program. If the response from this initial contact is not satisfactory, then the process described in 2.) and 3.) should be followed. With regard to any other matter not yet ripe for the formal process described here, initial contact should be with the Branch of Construction Wage Determinations.
