1 DEPARTMENT OF THE ARMY HEADQUARTERS, 4TH INFANTRY DIVISION AND FORT CARSON 6105 WETZEL AVENUE, BUILDING 1435 FORT CARSON, CO REPLY TO ATTENTION OF AFYB-CSM 30 September 2016 MEMORANDUM FOR SEE DISTRIBUTION SUBJECT: Memorandum of Instruction (MOI) for the FY17 Fort Carson Sergeant Audie Murphy Award Selection Board (SAMA) REFERENCES: FORSCOM Regulation Sergeant Audie Murphy Award, 2 April ELIGIBILITY: The SAMA Selection Board is open to all assigned NCOs CPL through MSG/1SG. Brigade Command Sergeant Majors are directed to ensure the widest dissemination of this memorandum to ensure maximum participation. The Brigade Command Sergeant Major will ensure all requirements for phases 1-3 are met in accordance with FORSCOM Regulation prior to sending the nominee to the final selection board held at 4 TH Infantry Division. NCO s can participate if they meet the following: a. The NCO must meet all Army Retention Standards. b. The nominee has not received any Article 15s within two years. c. Nominees who do not have direct supervision of Soldiers may submit proof of active participation in external leadership roles. d. Pass Army Physical Fitness Test /Height and Weight standards; and obtain recommendation by Company First Sergeant, Battalion Command Sergeant Major, and Brigade Command Sergeant Major. 1. The Sergeant Audie Murphy Award (SAMA) Board will convene on the following dates: Packet Due dates: Board Proceedings: 5 December December February March June June Recommend sponsors are SAMC Members, if a SAMC member is not available, supervisors are encouraged to be the sponsor. SAMC Members and candidates will report on the scheduled APFT, meet and greet for packet turn in, and board date. The events of the Division Sergeant Audie Murphy Award Selection Board will take place over two days. Before attending the Installation level board, Sergeant Audie Murphy Award Candidates must complete the following SAMA Selection process: 1

Phase One- is Senior NCOs Evaluation/Nomination: Senior NCOs who recommend NCOs for the award of SAMA will screen and evaluate candidates prior to appearance before initial selection board. The recommendation will be presented to the board in the form of a written recommendation highlighting the following: 1) Describe leadership achievements and NCO performance that merits special recognition. 2) How has the NCO contributed to the development of a professional NCO Corps and a combat ready Army? 3) How has the NCO exemplified leadership characterized by personal concern for the needs, training, development, and welfare of Soldiers and concern for families of Soldiers? (Include information outlined in phase 2 performance test, APFT and weapon qualification date and unit averages will be included). b. Phase Two- is Performance Test: The SAMA performance test is based on recorded accomplishments of the candidate and their subordinates (when applicable) and must be enclosed with the Senior NCOs evaluation/nomination. Phase two performance test information will be included in phase one memo. c. Phase Three- is Initial Selection Board: An initial selection board will be conducted at brigade level under the direction of the unit s senior NCO. Board members will be provided a copy of the candidate s packet. Each candidate s packet will contain at a minimum: 1) Senior NCO s Evaluation/Nomination and the performance test. 2) Current DA photo. 3) A biography of the service member. 4) Current Enlisted Records Brief (DA Form 2A, for Army Reserves and Army National Guard Soldiers). d. The board will be comprised of voting members senior to the Candidate and will include at least one voting member of the same sex as the candidate being considered. b. Candidates selected to continue will be scheduled to appear before the final selection board. Requests to appear before the SAMA final selection board will be submitted in a memorandum format to the final selection board president, signed by the brigade level or higher senior NCO. e. Phase Four- is the Final Selection Board: SAMA candidates will be considered by a final selection board appointed by the FORSCOM installation Senior CDR s CSM. Four additional CSMs or SGMs will be appointed by the president and a recorder without vote. At least one board member must be of the same sex as the nominee being considered. The final selection board will determine whether or not the candidate has reached a level of knowledge in a range of subject matters to warrant the FORSCOM SAMA; all questions within the board proceeding will be scenario driven. 3 AFYB-CSM SUBJECT: Memorandum of Instruction (MOI) for the FY17 Fort Carson Sergeant Audie Murphy Award Selection Board (SAMA) selection board will determine through a scenario-driven question and answer system whether or not the candidate has reached a level of knowledge in in a range of subject matters to warrant the FORSCOM SAMA. The board president has the final approval on all nominees appearing before the board. 3. The sponsor will ensure that the President of the Board receives their candidate's Leader s Book prior to the board appearance. Sponsors that are a Sergeant Audie Murphy Award recipient must be in good standing, which is not limited to being current on club membership dues and actively participating within the organization. Additionally, sponsors are not limited to the First Line Supervisor, Platoon Sergeant, Company First Sergeant, Battalion Command Sergeant Major, and Brigade Command Sergeant Major. Command sponsorship is highly encouraged; exceptions to sponsors will be granted if the above listed is unavailable. If operations dictate that a sponsor is not available, prior coordination must be made with the final phase SAMC Representative for the purpose of updating and obtaining approval from the President of the Board. The uniform for SAMA candidates and sponsors is the Army Service Uniform; board members will be in Army Combat Uniform. 4. Sergeant Audie Murphy Award nomination packets will be routed through Headquarters, 4th Infantry Division and Fort Carson 6105 Wetzel Avenue, Building 1435 Fort Carson, CO Packets will be secured with paperclips (no staples) in a manila folder. The upper left-hand corner of the folder will be the Soldier s rank, name and unit. Furthermore, submit a digital packet via in a legible word document and PDF to usarmy.carson.4-id.list.g1-awards@mail.mil or call / Packets will not be accepted after the turn in suspense date. Packets should be reviewed by a Brigade level CSM and SAMC member for completeness and accuracy. If packets are incomplete this may disqualify candidates and they will not be allowed to continue with the board proceedings. Please use the attachments as a sample product for all submitted documents. Ensure all items on the checklist are complete and accurate; improperly formatted products may not be accepted. CSMs, it is imperative that you check your candidate s packets thoroughly to set them up for success. 6. The board will be held at 4 th Infantry Division Headquarters, 1 st Floor, in room 107. The board president has the final approval on all nominees appearing before the board. The order of appearance will be published at the beginning of the scheduled board. 7. CANDIDATES: a. Candidates must know the Sergeant Audie Murphy biography verbatim. b. Candidates must know the Creed of the Noncommissioned Officer. c. Candidates will possess no derogatory actions in his/her record within the past two years. 3

Candidates participating in phase one through four board proceedings must be administered an Army Physical Fitness Test/Height and Weight Screening on the day of the board. During phase three simultaneous events are highly encouraged during the board proceedings, examples include weapons familiarization and assembly/disassembly of individual assigned weapons and crew served weapons, properly loading a SKL/FM; the above mentioned are example expectations during board proceedings conducted on the day of the scheduled board. The final phase board will consist of in depth situational/scenario driven questions with no limitations with time for evaluation. f. Candidates will be administered a record APFT from the final phase board on the day of the scheduled board. If the NCO does not pass the APFT then the NCO will not be authorized to participate in board proceedings. Previous APFT administered at the Brigade Level date is limited to 30 days for the packet submission to the final phase SAMA Selection Board. Candidates must be able to perform a three event APFT (pushups, sit ups, and 2 mile run) on the day of the APFT. No waivers are granted in this area unless the Soldier has a permanent profile that exempts them from the event. A copy of the profile must accompany the candidate on the day of the final phase board and candidate s packet. g. Candidates must have a range qualification within 90 days for the final selection board. h. Candidates are highly encouraged to volunteer with the Sergeant Audie Murphy Club Members, any documented or previously documented volunteer service hours maybe submitted with the candidate s packet and annotated on the Brigade Command Sergeant Majors letter of recommendation. i. Candidates must also be prepared to write an essay on the topics provided 48 hours prior to the final phase board, which will be disseminated by the final phase SAMC Representative. This process will aid in assessing the candidates competency, cognitive critical thinking skills, and prepare the candidate to respond intellectually to questions during the final phase SAMA selection board. Candidates must be able to respond in a professional and articulate manner. j. Candidates will be administered a written test question examination on the day of the board. Test questions will serve as an assessment intended to measure candidate s depth of knowledge using current Army Doctrine (see AR (A- 4) for a list of topics for the written test. This non standardized examination includes multiple choice questions and descriptions of principles, practices, procedures, methods, techniques, and regulatory explanations. The ability to apply understanding and applying the appropriate response of information in the particular subject area can complete and help choose the correct answer. 4

5 AFYB-CSM SUBJECT: Memorandum of Instruction (MOI) for the FY17 Fort Carson Sergeant Audie Murphy Award Selection Board (SAMA) Candidates will be prohibited from reconstructing, or reproducing the test material in anyway, either orally or in writing, in the event this becomes evident disqualification of candidate can be considered. Testing time limitation and the standard for go/no go will be explained at board proceedings. 8. NOMINATION PACKETS: a. SAMA nomination packets consist of the following documents in the order listed in FORSCOM Regulation (A-6). This regulation contains guidance on how to prepare the packet. b. BDE CSM Nomination Memorandum. Nominees are responsible for ensuring all documents submitted are up to date (especially ERB) and contain the most current information on the nominee appearing before the board. Below is an explanation of contents that are required for attending the SAMA selection board. 1) Senior NCO Recommendation with test results for SAMA/BDE CSM to the Board President. 2) ERB (Current) 3) DA Form 705, APFT Scorecard. (Complete & Current) 4) DA Form 5500-R (Male), DA Form 5501-R (Female), Body Fat Content Worksheet if applicable and must be current. Profile if applicable. 5) DA Form 3595-R, Record Fire Scorecard, or equivalent weapons scorecard. (no more than 90 days) 6) Biography (Encl 5) 7) Official DA Photo (Current) If a copy of the DA Photo is unavailable then a 5X7 head and shoulder photo of the NCO in ASUs sufficient (CPLs, SGTs and junior SSGs only) 8) Essay no less than 400 words and no greater than 500 words, will be announced 48 hours prior to the board, formatted APA 6th ed. on one of the following subjects: (The candidate should be prepared to provide further explanation and answer question regarding response or perspective and Army Doctrine. The essay must be submitted to the POC listed below.) 9) DA (NCOER). Provide the last three evaluations (NCOERs). 5

All Brigades shall provide a Board Member. Units and subject area assignments are outlined below. Board members should prepare their own questions in a situational style to elicit a procedural type response from candidates, instead of cut and dry answers. The G-1 will submit all approved Sergeant Audie Murphy Award selectees packets to FORSCOM for approval. Those Brigade CSMs of selectees will be notified when nominee medallion, membership card, and ARCOM is processed and received. ARCOMs will be processed at the Division level only to ensure no duplicate of award has been given. Brigade CSMs are responsible for the nominee s induction ceremony as separates or combined effort in accordance with the date corresponding with the availability of the Commanding General and Division Command Sergeant Major for presentation. 17. All DR. Mary E. Walker Award and MG Aubrey Red Newman Award packets must be submitted to be considered for recommendation during the SAMA Board. 18. SSG Wells, Abbey is currently serving in (Unit). She has served in the Unit for (Years). Her various leadership achievements consist of. She has deployed to (OIF). SSG Wells has participated in various Brigade FTX and Company FTX s, 1 JRTC rotation, more than 50 Combat Logistical patrols and forty combat missions as a female searcher for Charlie Troop throughout the Diyala Providence AO, which merit special recognition. 2. SSG Wells, is a consummate professional that leads from the front in every aspect. Her actions have contributed to the development of a professional NCO Corps and a combat ready Army through applying her warrior task and battle drills during the technical and tactical while mentoring and sponsoring the FY12, 13, and 14 winners of the 1 ST Cavalry Division NCO of the year, which resulted in all first place winners. Her hard work ethic is marked by excellence and clearly separates her from her peers, she is currently serving as the most junior ranking Platoon Sergeant in the grade of Staff Sergeant, and was selected for the position above four Sergeant First Class and fifteen Staff Sergeants. 3. SSG Wells has truly exemplified leadership characterized by personal concern for the needs, training, development, welfare, and concern of Soldiers and their families. SSG Wells supervises 20 NCOs and 22 Soldiers. 25 of 42 of the NCOs and Jr. Soldiers in the platoon are enrolled in college level courses with an overall average of 3.2%. SSG Wells currently has a Bachelor of Business of Science, and Masters of Business Administration. Her military education consist of. 4. SSG Wells was administered the APFT on 15 May SSG Wells current APFT score is as follows: PU: 40 SU: 68 2MR: 1545 (AVG): 274. The unit APFT (AVG): 247. SSG Wells qualified on her assigned weapon 10 April SSG Wells qualified on her assigned M4 as an Expert 40/40. The unit (AVG) weapon qualification is marksmanship. 4. Three Soldiers were assigned to her platoon for rehabilitation, after six months under her leadership all three Soldiers were assigned as team leaders, and obtain promotable status, as well as recognized as Soldiers of the month and Soldier of the Quarter. 15 SFC William B. Doe 1 ST Stryker Brigade, 4 TH Infantry Division Sergeant First Class (SFC) William B. Doe was born in Las Vegas, Nevada on December 12, He graduated from El Dorado High School in June 1986 and entered the United States Army on December 29, He graduated from Military Police One Station Unit Training at Fort McClellan, Alabama as a Military Policeman. SFC Doe s stateside tours have included Fort Benning, Georgia, Fort Campbell, Kentucky, Fort McClellan, Alabama, and Fort Stewart, Georgia. He has served an overseas tour in Germany and has deployed to Saudi Arabia/Kuwait for seven months and to Bosnia for six months. His achievements include the Commandant s List at the Primary Leadership Development Course, Honor Graduate at the Military Police Basic Noncommissioned Officers Course, Distinguished Honor Graduate at Drill Sergeant School, Leadership Award Recipient at the Military Police Advanced Noncommissioned Officers Course, Sergeant Morales Club Member. Leader Books can be unit standardized but should not be mass produced just for the purpose of the SAMA Board. Leader Books are an individual effort. 3. Leader Books should focus on the NCO s subordinates and should have plans of action for correcting Soldier deficiencies such as PT & Ht/Wt Failures, weapon qualification failures; disciplinary actions; maintaining task efficiency-remedial training etc. 4. Counseling should be updated and should reflect good counseling techniques found in FM 6.22 Appendix C. Counseling should also be the correct type and focus on and reinforce the positive performance of subordinates. 5. Leader Books should demonstrate how the NCO is professionally developing his/her subordinates. 6. Hand printed notes in pen or pencil in Leader Books are authorized as long as they are neat, organized and readable. 7. LEADER BOOKS. There is no standard format for the SAMA Leader Books. 19 Sergeant Audie Murphy Biography Audie Leon Murphy was a legend in his own time. A war hero, movie actor, writer of country and western songs, and poet. His biography reads more like fiction than fact. He lived only 46 years, but he made a lasting imprint on American history. Audie was born on a sharecropper s farm in North Texas on June 20, As a boy, he chopped cotton for one dollar a day and was noted for his feats of derring-do and his accuracy with a gun. He had only 5 years of schooling and was orphaned at age 16. After being refused enlistment during World War II in both the Marines and Paratroopers for being too small (5 5 ) and underweight (110 lbs), he enlisted in the U.S. Army a few days after his 18th birthday. After basic training at Camp Wolters, Texas, and advanced training at Fort George G. Meade, Maryland, Audie was sent overseas. He was assigned to the famous 15th Infantry Regiment of the 3rd Infantry Division where he fought in North Africa, Sicily, Italy, France, and Germany. He earned a battlefields commission for his courage and leadership ability as well as citations and decorations including every medal for valor that America gives. He was also awarded three French and one Belgian medal. Lieutenant Audie Murphy was the highest decorated soldier in American history. Discharged from the Army on September 21, 1945, Audie went to Hollywood at the invitation of movie star James Cagney. He remained in California for the rest of his life and was closely associated with the movie industry, both as an actor and a producer. He acted in 44 films, starring in 39 of them. His best known film was To Hell and Back, adopted from the bestselling book of his war experiences by the same name. Most of his movies were westerns. In 1955, Audie Murphy was voted the Most Popular Western Actor in America by the Motion Picture Exhibitors. Audie wrote the lyrics to 16 country and western songs, the most popular of which was Shutters and Boards, written with Scott Turner in The song was recorded by over 30 pop singers, including Jerry Wallace, Dean Martin, and Porter Waggoner. He was an accomplished poet; unfortunately, only a few of his poems have survived. In 1950 Audie joined the 36th Infantry Division ( T-Patchers ) of the Texas National Guard and served with it until He was a Mason and a Shriner and belonged to several veterans organizations. Audie Murphy was killed in a plane crash on a mountain top near Roanoke, Virginia on May 28, Fittingly, his body was recovered 2 days later on Memorial Day. Audie could very well be the last American war hero. He was the greatest combat Soldier in the 200 plus year history of the United States. 19
