1 EXECUTIVE SUMMARY On Friday, April 21, 2017, an equipment failure at a Pacific Gas and Electric (PG&E) substation at Larkin and Eddy Streets caused a power outage to northern and central City and County of San Francisco (CCSF) affecting an estimated 88,000 customers. The outage occurred at approximately 0919 hours and extended through the day until all customers were restored at approximately 1700 hours. The Department of Emergency Management (DEM) activated the Emergency Operations Center (EOC) to support City services throughout the outage and restoration process. In cooperation with the Building Owners and Managers Association (BOMA) and the San Francisco Chamber of Commerce (SF Chamber), CCSF Departments responded to the EOC at a full activation level. All CCSF Departments provided assistance to the EOC to assess the overall impact in the affected area and throughout the City. As a part of the EOC, the Joint Information Center (JIC) activated to better coordinate public information and warning. The JIC utilized AlertSF, traditional media, and social media to share information with the public throughout the day. The EOC s objectives were to: Coordinate CCSF Departments in response to the power outage; Ensure continuity of CCSF Services throughout the City including the affected area; Gather, coordinate, and disseminate situational information among participating agencies and to the public; Serve as a liaison to state, federal, and private sector partners as needed; Monitor situational status, with particular focus on Access and Functional Needs (AFN) dependent on electricity, in order to proactively take steps to mitigate potential hazards or threats. The purpose of this Statement of Initial Findings is to document what is known about the incident at this time. This information can be used to inform the After Action Report. The purpose of the After Action Report is to analyze CCSF s coordination and response efforts, to identify strengths to be maintained and built on, to identify potential areas for further improvement, and to support development of corrective actions. 2 TIMELINE Activation Level Prior to the Power Outage: Duty Team (Duty Officer/Manager on Call); Emergency Operations Center (EOC) is not activated. 0907: Power to the Department of Emergency Management which houses the Dispatch Operations Center and Emergency Operations Center went offline. As planned, generator power kicked in immediately to ensure continuity of operations. 0912: Dispatch Operations contacts PG&E to confirm power outage surge protocols were initiated. Six supervisors joined the 12 call takers on duty to answer calls. At this point the call log showed 29 calls waiting in the system. 0915: Dispatch Operations issues notification to DEM Duty Team and public safety partners. 0919: PG&E reported a power outage to DEM Duty Officer. Initial reports estimated 13,000 customers without power in Northern and Central San Francisco. 0939: Initial Public Alert: Tweet sent out from San Francisco Department of Emergency Management (SFDEM) stating: Large Power Outage impacting Northern SF. PG&E working on restoration. DEM advises call 911 only in case of emergencies. Tweet sent to more than 83,000 followers. (Appendix A) 0939: First media inquiry received. Department of Emergency Management Public Information Officer and later the EOC Joint Information Center (JIC) were engaged with the media throughout the day in order help push information out to the public. 0945: San Francisco Unified School District (SFUSD) posted a message to their website. (Appendix B) 0950: PG&E reported to the San Francisco Dispatch Operations Center (9-1-1) and the Duty Officer that up to 90,000 customers were without power. 1000: call volume stabilized to manageable levels. During this period ( ) the center received 405 emergency calls of which 226 were abandoned before a dispatcher could answer (all abandoned calls were called back). 201 non-emergency calls were received of which 181 were abandoned (by policy nonemergencey calls do not reveive a call back). By comparison on Friday April 14 ( ), the center received 70 emergency calls with 13 abandoned (all called back). For non-emergency calls, it was 103 calls with 36 abandoned. 1008: EOC Level A activation initiated. Level A means the EOC is put on standby mode and staffed by SFDEM Staff. 1016: EOC Level D activation initiated. Level D is San Francisco's highest activation level with all sections and branches activated. 1020: AlertSF (San Francisco's Text Message and Alert System) message sent to more than 59,000 citywide subscribers. Tweet also sent from SFDEM. Message stated: Power outage in the Northern part of San Francisco. The City is coordinating with PG&E as they work to resolve the issue. 3 1021: Board of Supervisors Update #1 sent via . The message was sent to 9 of 11 Supervisors. Supervisors Jane Kim and Aaron Peskin were accidently left off the distribution list. During the initial response, an ad-hoc EOC team was supplemented with administrative personnel from SFDEM until regular personnel could arrive. Unfortunately, an error was made by staff that does not normally operate in the EOC nor corresponds with the Board of Supervisors. This error was later discovered and corrected in subsequent correspondence. (Appendix C) 1035: SFUSD sent update to more than 9,000 employees. (Appendix B) 1049: SFDEM Tweets message to remind public to refrain from calling unless it is a life safety issue/emergency. 1100: EOC Level D reached and functionally staffed. At this point, most branches and units have sufficient staff and resources to support the incident. SFDEM's Division of Emergency Services is located off-site and staff must travel to the EOC. Staff from the City and County of San Francisco, community organizations, and the private sector must also travel from different locations in the City in order to staff the EOC. 1105: Mayor Edwin M. Lee arrives for a briefing with Acting DEM Director William Lee and Emergency Operations Center Manager Thomas Chin. 1116: SFDEM Re-Tweeted an update from PG&E stating that power should be restored to most customers by 1:00 p.m. 1120: Press Release #1 issued that includes a partial list of neighborhoods affected by the power outage. EOC staff attempted to find more accurate information regarding what Northern and Central San Francisco neighborhoods were affected. Unfortunately, this information was unavailable. (Appendix A) 1130: JIC staff hosted a conference call with citywide public information officers in order to share information and coordinate messaging. 1131: SFDEM Re-Tweeted an update from BART. (Appendix A) 1145: The San Francisco Public Utilities Commission (SFPUC) posts an urgent alert to more than 150,786 Nextdoor users. SFPUC public information officers are part of the JIC along with SFDEM, SFUSD, 311, Mayor's Office on Economic and Workforce Development, and PG&E. 1149: SFDEM Re-Tweeted an updated from Muni. (Appendix A) 1158: SFDEM Re-Tweeted an update from SFPD. (Appendix A) 1230: Human Services Agency (HSA) and the Mayor s Office on Disability (MOD) staffing the EOC initiate contact with home-bound residents relying on electricity for medical mobility purposes. An American Sign Language (ASL) interpreter was present for the press conference. 1415: SFUSD sends text message update to more than 57,000 families. PIO announces situation is stable and improving. 1519: Situation Report sent to City Departments and Partner Agencies. (Appendix D) 1528: SFDEM tweets asking people on electrical medical devices, to get in touch with service providers. EOC begins stand down from Level D to Level B- Partial activation with first responder elements. The San Francisco of Department of Emergency Management has activated the City s Emergency Operation Center to coordinate all response activities. The focus of all city efforts remains on public and life safety. We are closely coordinating with all critical facilities, including hospitals, schools, and public transportation agencies. Currently, California Pacific Hospital and Saint Francis Hospital remain open and are operating off of backup generators. Any new patients are being rerouted to other fully operational hospitals in the city. Public schools affected remain open and operating under standard blackout operating protocols. At this time, the cause for the outage remains under investigation by PG&E and other officials. All SFUSD school power has been restored. General Situation: At approximately 0919 hrs, PG&E reported a large power outage in the northern part of the City and County of San Francisco. DEM activated the Emergency Operations Center (EOC) at Level C department agency representatives and DEM staff at 1000 hrs to provide support and information coordination to services and agencies impacted by the power outage. PG&E reported 88k initial customers impacted and 23,000 affected users restored with power as of 1346hrs. DOCs Activated: CCSF EOC is deactivating from active support of Power Outage activities. PGE SFMTA SFPD Presidio SFUSD Deactivated at noon Status Levels: Green = Normal Yellow = Minor Issues Red = Significant Issues Operations Support Section Human and Health Services Status: Yellow Outlook: Improving In-home support services that are not under 24 hour care and are dependent on electrical medical devices will typically last only 4 hours. HSA is actively reaching out to the 150 vulnerable clients. Clients who are in need of assistance are being instructed to contact their care giver. If HSA is unable to reach them, an emergency contact is being called. All 150 identified IHSS clients have been contacted EMS ambulance at normal volume.
