1 TEXAN CORPS OF CADETS Application for Admission to the John Tarleton Academy 1 INSTRUCTIONS This application should be completed by all students who wish to participate as a member of the Texan Corps of Cadets in the John Tarleton Academy (JTLA). All Cadets must also apply for and be admitted to attend Tarleton State University through ApplyTexas.org. All Cadets must apply for and receive University housing. Acceptance to the University does not automatically lead to acceptance into the Corps of Cadets. You will not be accepted to the John Tarleton Academy until you are first admitted to attend Tarleton State University. After completion of the sophomore year, students remain in the Corps as an ROTC (Military Track) Cadet or a Drill and Ceremony (Civilian Track) Cadet. I plan to participate in the following: (please check all that apply) All ROTC participants must be in the Corps of Cadets. Participation in ROTC, including military labs and physical training, is an integral part of the Corps leadership experience at Tarleton. AROTC Cadets: students who plan to pursue a commission in the U.S. Army. Complete pages 1-11 and forms of this application which includes the Medical Fitness Statement (DA Form 3425-R). AROTC applicants must provide additional materials prior to the Acceptance decision. See the AROTC section of this application for more information. AFROTC Cadets: students who plan to pursue a commission in the U.S. Air Force. AFROTC applicants must provide additional materials prior to the Acceptance decision. Complete pages 1-11 and forms minus DA Form 3425-R (pg 9). See the AFROTC section of this application for more information. Marine PLC Cadets: students who plan to pursue a commission in the U.S. Marine Corps. Marine PLC applicants must provide additional materials prior to the Acceptance decision. Complete pages 1-11 and forms minus DA Form 3425-R (pg 9). See the Marine PLC section of this application for more information. Drill and Ceremonies (D&C) Cadets - students who choose to participate in the John Tarleton Academy, but do not plan to work toward a commission in the Armed Forces. Complete pages 1-11 and forms of this application which includes the Report of Initial Medical Fitness Statement (JTLA-1). To obtain admission into the Corps of Cadets, all incoming students must enroll in freshman-level courses of either Army ROTC or Air Force ROTC. The student DOES NOT incur a military obligation. In order to be accepted to the John Tarleton Academy: You must first be admitted to Tarleton State University. Apply at ApplyTexas.org. You must apply for and receive housing on-campus. The priority of your John Tarleton Academy acceptance will be based upon the date of your housing application and deposit. This application and the correct physical must be approved by the Commandant of the John Tarleton Academy. All other items must be submitted prior to the beginning of Tarleton Transition Week. An acceptance/declination letter will be sent once all documentation has been submitted. If accepted, a Welcome package will be attached and will include a summary of what to expect as a new Cadet in the Texan Corps of Cadets. Also, all new Cadets are required to attend the Cadet Orientation Training and Transition Week, usually starting ten (10) days prior to the first day of classes. 3 3 ARMY ROTC APPLICANTS If you are interested in applying to be a part of the AROTC program at Tarleton State University, please review the application information below. If you have any questions regarding the application process, contact the AROTC recruiting officer on campus at (254) ELIGIBILITY REQUIREMENTS U.S. Citizen Must not have reached 29th birthday during year in which graduation and commissioning are anticipated Have a high school GPA of at least 2.50 Score a minimum of 920 on the SAT (math/reading) or 19 on the ACT (excluding the required writing test scores) Applicants with prior military service may be eligible for age adjustments for the amount of time equal to their prior service, on a month-by-month basis, for a maximum of 36 months if they will not reach their 29th birthday by December 31 of the year in which graduation and commissioning are anticipated High school graduation or the equivalent certification is necessary by August 1 of the year of entrance into fouryear AROTC scholarship program (NOTE: Two-year and three-year scholarships are also available to qualified candidates) No moral obligations or personal convictions that prevent conscientious bearing of arms and supporting and defending the Constitution of the United States against all enemies, foreign and domestic Agree to accept a commission and serve in the Army on Active Duty or in a Reserve Component (Army Reserve or Army National Guard) PHYSICAL AND MEDICAL HISTORY REQUIREMENTS Candidates must pass a stringent physical examination to qualify for the AROTC program. The physical and scholarship qualification processes are independent determinations. Applicants must receive a favorable endorsement in both areas before any AROTC scholarship can be granted. Some common medical conditions which might be considered disqualifying are: vision not correctable to 20/20 (each eye), no color vision; history of asthma; acute or chronic cardiac/pulmonary conditions; chronic skin disorders such as cystic acne, psoriasis, atopic dermatitis or eczema; arthritis; seizure disorders, severe motion sickness susceptibility, alcohol or drug abuse; pregnancy. APPLICATION REQUIREMENTS The following documentation is required to be submitted along with the application and medical history: Social Security Card * Birth Certificate and/or unexpired Passport* * A photocopy is acceptable provided the original or certified copy is available during Tarleton Transition Week check-in. AROTC SCHOLARSHIPS US Army offers two-, three-, and four-year awarded to students based on merit. Pays 100% tuition and all mandatory fees, provides $1200/year for books, and provides a monthly tax-free stipend throughout the school year. Special consideration to Cadets pursuing degrees in nursing, science, technology, engineering, and math. AROTC COMMISSIONING Upon successful completion of the AROTC Program and baccalaureate or graduate degree, a Cadet is commissioned a Second Lieutenant in the U.S. Army. In some instances, active service can be delayed by students continuing in post-baccalaureate degree programs. Cadets can serve in the Army National Guard, Army Reserve, or compete for Active Duty depending on the scholarship program. Normal active duty service obligation is four years and ARNG/USAR service obligation is six years. 3

4 4 AIR FORCE ROTC APPLICANTS If you are interested in applying to be a part of the AFROTC program through Tarleton State University, please review the application information below. If you have any questions regarding the application process, contact the AFROTC recruiting officer at (817) or with questions. AEROSPACE STUDIES PROGRAM REQUIREMENTS A four-year program that enables Cadets to take advantage of four years of aerospace studies courses. Each semester, for the first two years, Cadets take a one-credit hour academic class and a one-credit hour Pass/No-Credit Laboratory (LLab). The first two years collectively are referred to as the General Military Course (GMC). Upon successful completion of the GMC and an ensuing four-week Air Force paid field training course, qualified and selected students may elect to enroll in the final two years, referred to as the Professional Officer Course (POC). Each semester in the POC, students take a three-credit hour academic class and a one-credit hour Pass/No-Credit LLab. AFROTC uniforms and textbooks are issued by the unit. ELIGIBILITY REQUIREMENTS Be a full-time student (12 semester hours or more) Be a U.S. Citizen Be in good physical condition/health Have good moral character Be no older than 34 years upon commissioning AEROSPACE STUDIES Enrollment in the General Military Course (first two years) is voluntary for eligible students and does not obligate non-scholarship students for further military service. The Professional Officer Course (last two years) is also voluntary but competitive. Because the Professional Officer Course leads to a commission in the U.S. Air Force, those selected to continue training will incur military obligation. AFROTC BENEFITS As Air Force ROTC cadets, students are entitled to selective benefits. Social and co-curricular activities, together with leadership and academic training, are all part of Air Force ROTC. Contracted Cadets receive a nontaxable subsistence allowance each month during the school year. The detachment sponsors a Civil Air Patrol where cadets can obtain front-seat and back-seat flying time in Cessna aircraft. Drill team, honor guard and Arnold Air Honor Society are just a few social outlets for the Cadets. Summer opportunities for Cadets can include a paid visit to a military installation for two weeks, freefall parachuting, combat survival training, flight nurse shadowing and Cadet training assistant duty at field training. AFROTC SCHOLARSHIPS Air Force ROTC offers scholarships that vary in length of award and amount based on academic major and applicant qualifications. All awarded scholarships pay a stipend for textbooks and fees, plus a monthly, nontaxable, stipend during the school year. Scholarship awards are based on specific academic majors related to the needs of the U.S. Air Force. These scholarship opportunities for in-college students are determined at the national level by Air Force ROTC and are subsequently administered by the detachment/department of Aerospace Studies. Scholarship applicants are selected using the whole-person concept, which includes objective factors (i.e., GPA, standardized test scores (SAT/ACT), and physical fitness test) and subjective factors (i.e., personal evaluations). Students who are enrolled in Air Force ROTC generally improve their scholarship selection opportunity. In addition to meeting the general qualifications mentioned above, scholarship applicants must be at least 17 years of age when the scholarship is activated and must be less than 31 years of age as of the end of their commissioning year. Because the scholarship program varies according to budget and needs of the Air Force, interested applicants should contact the Department of Aerospace Studies at (817) or specifics. 5 5 MARINE PLC PROGRAM APPLICANTS If you are interested in applying to be a part of the Marine Platoon Leaders Class (PLC) program through Tarleton State University, please review the application information below. If you have any questions regarding the application process, contact the Marine PLC recruiting officer at (817) or CPT Michael A. Bressler at michael.bressler@marines.usmc.mil. MARINE PLC PROGRAM The Marine PLC program is a Marine officer program for college students attending or planning to attend an accredited college or university. It is not ROTC, but MLSC and LDRS classes at Tarleton along with the leadership oriented courses that the Corps implements would be beneficial to student-cadets as future officers. Upon successful completion of all academic requirements at Tarleton and Officer Candidates School, a Cadet is commissioned as a Second Lieutenant in the United States Marine Corps. All PLC training takes place in the summer. For freshmen and sophomores, PLC consists of two paid six-week internship sessions at the Marine Corps Officer Candidate School in Quantico, VA. Juniors will attend a 10-week internship session. ELIGIBILITY REQUIREMENTS Minimum of 1000 SAT (math/reading only) or a 22 on ACT Be a U.S. Citizen Be in good physical condition/health Have good moral character MARINE PLC FINANCIAL BENEFITS Financial Assistance: Cadets can receive financial assistance of $3, annually. That is $1, for each semester. Cadets can earn this assistance for up to three years for a total of $9, This is offered in exchange for an additional six months of active duty service. Tuition Assistance Program (MCTAP): Cadets can receive annual reimbursable expenses of up to $5, per academic year for a total of three years. The maximum amount that a member will be reimbursed is $15, over a period of three years. This is offered in exchange for an additional year of active duty service. MARINE PLC COMMISSIONING A Cadet who completes all requirements of the PLC Program and accepts a commission in the Marine Corps is obligated to serve the period of time specified in their service agreement. Currently, this obligation is 4 years for non-aviation officers. The obligation for aviation officers is 6 years after completion of flight school. A student participating in the PLC Program incurs no service obligation until they actually accept a commission upon college graduation. Recipients of financial assistance incur a financial obligation to reimburse the government if they dos not accept their commission. 5

IMPORTANT NOTES: 1. The initial outlay for uniforms is approximately $800 to $850. (estimated). Uniforms are University specific and no substitutions are allowed. Please complete and return the Cadet Uniform Rental Agreement (see page 11). 2. On-campus housing is required for new Cadets. Failure to secure on-campus housing could prevent a new student from entering the Corps of Cadets. The housing application can be found at tarleton.edu/housing. 3. The Cadet lifestyle, to include Physical Training, is designed to safely challenge Cadets. Examples of typical activities are listed below and are detailed in Texan Corps of Cadets regulations. WEEKDAYS: 0545 Rise lights out Su-Th (may study later if desired). Physical Training conducted (M-F). Mandatory study hall three to four days a week for underclassmen M-F characterized by minimum interaction with upperclassmen, with study encouraged and naps or individual free time allowed. Physical Training (PT) is designed to attain and maintain an acceptable level of fitness for each Cadet. Physically able Cadets are required to pass the Corps Physical Fitness Test (PFT), involving the execution of pushups, sit ups, and a 2 mile run. In addition, Cadets that have a military contract/scholarship are required to pass the physical requirements for their branch of service, which in some cases is more difficult than the Corps PFT. Cadets are also expected to meet and maintain a height/weight (or alternatively a body fat content percentage) standard. New Cadets have the entire academic year to attain the standards for Ht/Wt and PFT. Unit organized activities are designed to enhance Espirit de Corps, as such, the goal is for all members to participate. Activities include close order drill, outfit runs, crossfit workouts, 3 to 5 mile ability group runs, organized sports, and obstacle/stamina course events. Cadets must be physically fit or willing to work toward becoming fit. I certify that I have reviewed the list of typical activities and feel that I (my child) am (is) mentally and physically healthy and are (is) therefore capable of undertaking these activities. I also agree that any medical concern as noted by the physician on page 2 (reverse) can be disclosed to individuals within the Tarleton State University Commandant s Staff and the Corps of Cadets organization responsible for my (my child s) health and wellbeing. Requirements for Minor in Studies: 1. Membership in the Texan Corps of Cadets & 18 hours of University-recognized leadership coursework in one or a combination of the three tracks described above. 2. All Freshman and Sophomore Cadets must register in Army or Air Force ROTC courses and labs MLSC /CRN (WED) or AEST 1100 (THU), respectively. Sophomore Cadets may request an exception to enroll in LDRS courses in lieu of ROTC courses through the Office of the Commandant. 3. Aerospace Studies classes and labs are taught at TCU campus in Ft. Worth on Thursday afternoons from 1:00-6:00 pm. Transportation is provided by the Corps of Cadets. 4. Satisfactory completion of Progression within the Texan Corps of Cadets. Progression is defined as a minimum of four semesters of participation in the Texan Corps of Cadets, including: completion of at least two Corps leadership positions, participation in weekly physical training, and passing the ROTC or Corps Physical Fitness test annually, unless waived by the Commandant/Department Head. 5. Satisfactory completion of the summer training or internship and evaluation conducted by the Dept. of Military Science or Dept. of and Strategic Studies. 6. A minimum minor grade point average of 2.5 in all courses taken to fulfill the minor. 7. A minimum of six hours of course work at the 3000-level or above must be taken to fulfill the minor requirement.

11 Tarleton State University Cadet Uniform Warehouse Traditions South Residence Hall Stephenville, TX CADET UNIFORM RENTAL AGREEMENT I acknowledge receiving the Corps of Cadets uniform items. I understand all items issued to me are the property of Tarleton State University (except those noted in paragraph 4 below) and shall remain the property of Tarleton State University. 2. I agree to assume full responsibility for the care, maintenance, and storage of all items issued to me. I understand that I may not loan, exchange, sell, or otherwise transfer any issued items to any other individual, nor may I try to return other items claiming them as my own. I further understand that I am responsible for the safe storage of the uniforms during the summer months. RfiD bearing uniforms must not be tampered with nor tags removed from the garments. 3. I agree to promptly and personally return all required items, DRY CLEANED, FREE FROM STAINS AND PRESSED, to the Cadet Uniform Warehouse (CUW) upon my separation from the Corps. Cadets must turn in their uniforms within 10 school days of leaving the Corps. Leaving the Corps is characterized as, but not limited to (1) initiating outprocessing by picking up resignation paperwork; (2) unauthorized absence from the Corps; (3) taking a leave of absence from the Corps, such as study abroad, co-op program, or health issue, or (4) completing the senior year in the Corps of Cadets. Cadets withdrawing from the Corps or Tarleton State University must clear their uniform accounts by either a complete turn-in or payment for items that do not meet the exchange requirements listed on this document. Cadets planning to return to the Corps will be re-issued uniforms when they return to active status. 4. If I decide to leave the Corps, I have the option of handling the uniform cleaning myself, or having the cleaning billed to my student account through the CUW. I understand that I will be charged for any issued item not returned or that is damaged beyond normal wear and tear. Normal wear and tear would include (but not be limited to) frayed pockets or obvious worn spots on the garments. I also understand that there are items (headgear, boots, shoes, brass items, gloves, & belts) once worn, are not returnable and I will not receive a credit should I leave the Corps. Should I not return the uniforms as prescribed, I understand that a charge will be added to my student account. The billing will not be removed, the uniforms will not be accepted by the CUW, and the debt must be paid. If the uniforms are not returned by the deadline, the garments are the property of the signed student and the bill must be paid. 5. I further acknowledge that I will be billed for issued uniform items at the beginning of each academic year. Should I leave the Corps prior to being billed for a given year, I will still be charged for the non-returnable uniform parts. This Cadet uniform rental agreement is between the Cadet Uniform Warehouse, a department within the Office of the Commandant for the Corps of Cadets at Tarleton State University, a component of the Texas A&M University System, an agency of the State of Texas, and the individual student. Students (parents and/or guardians) are urged to carefully read this contract. When this agreement for newly assigned students, or agreement renewal form for returning students is completed and returned to Cadet Uniform Warehouse, Traditions South Residence Hall, Tarleton State University, Stephenville, TX 76401, or is completed electronically and transmitted to Cadet Uniform Warehouse via electronic means, it becomes a binding agreement between the student (his/her parent or guardian if the student is under 18 years of age) and Tarleton State University.
