Session #3 Advancing Hospital to Home Connectivity: A Work in Progress Minnesota e-health Summit June 7,

Session Speakers Joe Dietlin Director of Business Development and Strategy, Pediatric Home Service Josh Larson Director of Operations Quality, Pediatric Home Service 2

Advancing Hospital to Home Connectivity: A Work in Progress

Learning Objectives Describe the current situation for Health Care Professionals (HCPs) prescribing home care equipment and orders for their pediatric patients with medical complexities. Review solutions applied to date in easing the administrative demand on HCPs. List the team members that participate in implementing these solutions. Identify requirements such as HIPAA, PHI, encryption in the secure exchange of this information. Discuss current limitations of technology.

About Pediatric Home Service 26 years transitioning children from hospital to home Respiratory, Infusion, Nursing, DME, Nutrition Support and Mental Health services 4,000 medically complex pediatric/adult patients 400 employees (50% clinicians) Tech savvy EHR use since 1991

PHS Interoperability Mission To improve quality of care, patient and provider satisfaction, and operational efficiency.

Why We Want Interoperability to Succeed Patients with complex conditions are rarely discharged from the healthcare system they are cared for in the hospital, clinic and home. More efficient communication can lead to better outcomes We can focus on the needs of the patient when operations are efficient

Provider Situation Overview

HCP Situation Overview Only about 15% of physicians used their EHR to exchange information with home health, long-term care, or behavioral health providers. - Journal of AHIMA - April 2016

Home Care Environment A 2014 MHCA survey of 51 home health agencies showed that 75% of our members do not have a qualified electronic health record. Interoperability is really the key goal for the health care industry.

Home Care Environment PHS EHR since 1991 Robust, affordable EHR? Qualified?

PHS Implemented Solutions E-signature Home care order templates Referral EHR access

PHS Implemented Solutions E-signature Home care order templates Referral EHR access

Implementation of Electronic Signature Secure, HIPAA compliant Sign anywhere 163 Physicians to date 5,000 documents signed to date

Return Time for Home Care Orders

Electronic Signatures work great! I think it is incredibly easy to use and leads to quicker turnaround. This is a great new service. Works awesome on my tablet, too, if I am not at my computer!

Not one solution fits all Provider partners need to have solution fit in to their EHR-specific workflow Each facility, including PHS, has security considerations to be addressed

PHS Implemented Solutions E-signature Home care order templates Referral EHR access

Home Care Order Template Situation Overview Provider to PHS communication was inefficient Multiple interactions where required to get appropriate and necessary documentation for detailed orders from prescriber to PHS

Home Care Order Template Solution Create EHR-specific order templates specific to home care Train frequent user / care manager / discharge planners on use of the template Create feedback loop for improvement

Home Care Order Template

Home Care Order Template Benefits Reduced documents to providers Less interactions required to get to required set of documentation Quicker administration of necessary care

Home Care Order Template Challenges Billing Code Changes Insurance Changes Human Factor EHR Administration / Data Entry Unnecessary customization

PHS Implemented Solutions E-signature Home care order templates Referral EHR access

Having the ability to see the patient's previous provider/hospital visits, historical laboratory tests, and medication history gives providers a more accurate, complete picture of the patient and their health status, which facilitates clinical decision making and improves patient outcomes. ~ from HIMSS website

Referral EHR Access Situation Overview Patients with complex conditions are rarely discharged from the healthcare system they are cared for in the hospital, clinic and home Each interaction with the system is not always communicated across the care team

Referral EHR Access Solution Gain read-only access to referral source EHR systems Train PHS clinicians to access appropriate information to aid in the care of shared patients

Referral EHR Access Benefits Reduced interactions with providers to understand patient situation and condition Increase in employee satisfaction and empowerment to do their jobs Quality of care improvement?

Referral EHR Access Challenges All EHRs are different HIPAA / new questions, scenarios

Ongoing Efforts MN ehealth Initiative HIMSS & AHIMA Tangible results We are excited about interoperability in whatever form it ends up taking.

Questions? Joe Dietlin Director of Business Development and Strategy Josh Larson Director of Operations Quality
