3 Advancing Hospital to Home Connectivity: A Work in Progress
4 Learning Objectives Describe the current situation for Health Care Professionals (HCPs) prescribing home care equipment and orders for their pediatric patients with medical complexities. Review solutions applied to date in easing the administrative demand on HCPs. List the team members that participate in implementing these solutions. Identify requirements such as HIPAA, PHI, encryption in the secure exchange of this information. Discuss current limitations of technology.

10 Home Care Environment A 2014 MHCA survey of 51 home health agencies showed that 75% of our members do not have a qualified electronic health record. Interoperability is really the key goal for the health care industry.
