1 Torfaen Social Care and Housing Services SOCIAL WORK IN A HOSPITAL SETTING Supportive, Fair, Effective
2 Hospital Social Work Team Torfaen has a hospital-based social work team to support patients who live in the borough. This team is split over two sites: = The Royal Gwent Hospital team supports patients in the Royal Gwent and St Woolos hospitals (in Newport) = The County Hospital team supports patients in County Hospital, Nevill Hall Hospital and the surrounding hospitals Blaenavon, Blaina, Ebbw Vale, Aberbeeg and Tredegar 1 How can we help you? We will work closely with you, your carer or family and with health professionals to ensure that you are discharged safely from hospital and are able to return to normal life as quickly as possible. We will work with you to find out what you are able to do and what you find difficult. For example, we will see how you manage with everyday activities like washing and dressing, preparing hot drinks and meals and your mobility in your home. We will offer advice and/ or arrange appropriate support for you on discharge if you are eligible for help from social care services - your hospital social worker will explain more about this to you or you can read our leaflet Eligibility Criteria Getting the services you need. 2
3 If you were receiving services from us before you went into hospital but your care needs have increased, we may be able to arrange extra home care or social care support when you leave hospital. What if you are unable to return home? You will be assessed and if you are found to require a level of care that cannot be provided to you in your own home, your hospital social worker or community care worker will discuss your needs and wishes with you and advise you and your carer or family of the options available. One of the options may include residential or nursing home care. For more information see our leaflet Choosing a Care Home. What happens next? Before you leave hospital, your hospital social worker will discuss your assessment with you and your carer or family. Your hospital social worker will also give you, and any care agencies involved, a detailed care plan which will tell you exactly how your care needs are to be met and who will be responsible. Will you have to pay for care? Torfaen has a charging policy for any services received, which is based upon your ability to pay. You will be asked to fill in a financial assessment form with details of any benefits, income, savings, etc. you may have. This is not compulsory, however if you prefer not to disclose your financial details, you will be charged the maximum amount for services. Your hospital social worker or community care worker will be able to give you further information about this. 3 4

Your hospital social worker will provide you with a pack containing further information. Os yr oeddech yn derbyn gwasanaethau oddi wrthym cyn i chi fynd i mewn i r ysbyty ond bod eich anghenion gofal wedi cynyddu, efallai y gallwn drefnu gofal cartref ychwanegol neu gymorth gofal cymdeithasol pan fyddwch yn gadael yr ysbyty. Beth os na allwch ddychwelyd i ch cartref? Byddwch yn cael eich asesu ac os byddwn yn penderfynu bod angen gofal na ellir ei gynnig i chi yn eich cartref eich hun, bydd eich gweithiwr cymdeithasol ysbyty neu ch gweithiwr gofal cymunedol yn trafod eich anghenion a ch dymuniadau ac yn eich cynghori chi, eich gofalydd neu eich teulu ynghylch yr opsiynau sydd ar gael. Gallai un o r opsiynau gynnwys gofal mewn cartref preswyl neu gartref nyrsio. Am wybodaeth bellach gweler ein taflen Dewis Cartref Gofal. 3 Beth sy n digwydd nesaf?
