All new patients receive details as they register, but as the ethnic background of the practice is very low, we more than match this with the patient group. With regards to gender and age, we always strive to enrol younger members to the group, despite the lack of interest in these age groups, and the doctors encourage all patients to come forward. Female group members are more difficult in the younger ages as many are working parents. We always ask the group members to suggest dates and times of meetings to fit in with their availability, and will have members that are just virtual members if this is more convenient for them. NO There are no specific characteristics. 2. Review of patient feedback Outline the sources of feedback that were reviewed during the year: GP Access Survey 2015, HealthwatchOnline access survey, Inhouse patient survey, feedback from patient group meetings at the local George Eliot Hospital, Family and Friends Scheme, handwritten comments book kept in the Waiting Room. 5 rl/:kj 3. Action plan priority areas and implementation Priority area 1 Description of priority area: Room temperatures within the Surgery, particularly the doctors and nurses rooms. These can be cold in Winter and hot in Summer. What actions were taken to address the priority? Research into a replacement boiler and/or air conditioning viability. Result of actions and impact on patients and carers (including how publicised): It was decided to prioritise the purchase of a new boiler, and after collecting estimates and looking at options, this was commissioned from a local reputable Company. This was installed at the end of last year, and has made a considerable improvement. AHW PPG Annex C Reporting Template

7 ~ Priority area 2 Description of priority area: Can patients who have conditions such as Diabetes and Epilepsy have identify wear/jewellery to advertise their condition in the case of collapse/emergency treatment? What actions were taken to address the priority? Various companies, both local and online were reviewed for availability of these items. Price was looked at and ease of ordering. Result of actions and impact on patients and carers (including how publicised): The Doctors now have a supply of outlets and reputable online companies who can supply these to patients. The doctors hold a tear off order pad in their room to give to patients, and the practice nurse has made the availability of these an item on all diabetic checks. The doctors will also discuss this with all their suitable patients, eg epileptics. AHW PPG Annex C Reporting Template

9 rl7"m Priority area 3 Description of priority area: Worries reported back from the Patient Group re the planned release of 70% of diabetic patients back to Primary Care from Secondary Care. There were concerns over any risks involved to patients as members of the Group are diabetics, and concerns re the training in Primary Care needed to do this. Patients requesting some sort of action to plan for this happening. What actions were taken to address the priority? Both doctors talked to the Group re the risks, said the Doctors and Nurses would ideally benefit from some additional training, and that it would increase the surgery workload, not hugely, but it was manageable. It was stated that local representatives from drug companies recognise that this Surgery's treatment of diabetic patients is excellent in comparison to some that they visit" but there may be some additional training re insulin initiation that could be offered via these companies. The Practice Manager to research this, and to initiate contact with companies to offer training on insulin initiation. The Patient Participation Group members who attend external hospital meetings will also take forward this to the CCG to request their provision of further training. Result of actions and impact on patients and carers (including how publicised): The Practice Manager has now initiated contact with a trainer who came in to see the Practice Manager, Dr Motala and Practice Nurse in February 2015 to discuss study courses for insulin initiation for doctors and nurses. This has now been arranged and will be attended by at least one doctor and the practice nurse, starting on Wednesday 25 th March AHW PPG Annex C Reporting Template

11 Progress on previous years ~ If you have participated in this scheme for more than one year, outline progress made on issues raised in the previous year(s): Patients seem to be aware of the Extended Hours late surgery and its day and times. This has been advertised in the Surgery and by the receptionist over the phone, and most patients are aware. Increased number of telephone consultations now offered every day with the Doctor as requested by patients. Patients requesting to know more about the scope of the Practice Nurse's appointments and the different appointments offered with her. This has dvertised in the waiting area for all to see. Patients request appointments on the day whenever they call- all patients who call on the day are either given an appointment or telephoned Back by the GP. AHW PPG Annex C Reporting Template
