4 Program Background, Philosophy and Principles Dignity Health Community Grants Program is conducted each year by all member hospitals Priority health needs are established by the triennial Community Health Needs Assessment. The grants program is one of the ways in which Dignity Health hospitals advance their mission to serve those in need, build healthier communities, and enhance advocacy and social justice efforts 4

5 Program Background, Philosophy and Principles Dignity Health seeks to partner with other nonprofit organizations who are working to improve the health status and quality of life of the communities we serve and who share our five core values: Dignity Respecting the inherent value and worth of each person Collaboration Working together with people who support common values and vision to achieve shared goals Justice Advocating for social change and acting in ways that promote respect for all persons and demonstrate compassion for our sisters and brothers who are powerless Stewardship Cultivating the resources entrusted to us to promote healing and wholeness Excellence Exceeding expectations through teamwork and innovation 5

6 Program Background, Philosophy and Principles In keeping with our strategic priorities for community benefit, Dignity Health expects programs to integrate one or more of our following principles: Focus on disproportionate unmet health-related needs; Emphasize primary prevention and address underlying causes of health problems; Contribute to a seamless continuum of care; Build community capacity; and Emphasize collaborative governance 6

8 Collaboration This year s program remains focused on the collaborative concept Goals include: Developing strategic partnerships between community-based organizations and Dignity Health hospitals that leverage resources and address priority health issues Innovative partnerships that have a direct, positive and lasting impact on the health of disadvantaged individuals and families in our community 8

9 Collaboration GSSA awarded more than $860,000 in community grants to eleven collaborative proposals across Sacramento, Yolo and Nevada counties for FY

10 Priority Health Needs This year s priorities were identified in the 2016 Community Health Needs Assessments (CHNA). Access to Behavioral Health Services: Includes access to mental health and substance abuse prevention and treatment services. Active Living and Healthy Eating: Affects health behaviors (e.g., fruit and vegetable consumption), associated health outcomes (e.g., diabetes) and aspects of the physical environment/living conditions (e.g., food deserts). Access to High Quality Health Care Services: Includes access to primary and specialty care, dental care and maternal and infant care. Additionally, this category includes health education and literacy, continuity of care, care coordination and patient navigation including linguistically and culturally competent services. Disease Prevention, Management and Treatment: Includes disease prevention and/or management programs that improve health status. Safe, Crime and Violence Free Communities: This encompasses programs that improve safety from violence and crime, including violent crime, property crimes, domestic violence and human trafficking. Basic Needs: Includes food security, affordable housing, economic security (e.g., employment, benefits) and education (e.g., reading proficiency and graduation rates). 10

12 Community Grants Criteria Funding up to $100,000 per collaborative proposal Proposals below $20,000 per collaborative will not be considered Proposals must identify the type of change expected; how and over what time period progress will be measured Proposals must include a project sustainability plan after funding cycle is completed Strict adherence to grant guidelines is expected 12

13 Community Grants Requirements To be eligible for funding in this year s grant cycle, organizations must: Be a 501(c)3 organization (e.g., community health centers, FQHCs, nongovernment run FQHC look-a-likes, faith-based centers, family resource centers, other community-based organizations and faith-based organizations) Work in collaboration with a minimum of 3 organizations. Program/Project responds to two or more of the stated priority health needs Programs must show direct linkage to one or more of the Dignity Health hospitals located within Sacramento County Programs must provide services within Sacramento, El Dorado and Placer Counties NO individual LOIs will be accepted Partner organizations must submit ONE joint Letter of Intent with one organization identified as the Fiscal Agent 13

14 Partnership Requirements Must establish a meaningful and viable partnership with strategic alignment as evidenced by: A mechanism for shared decision-making and authority Partners serve the same community Partners have compatible missions Minimum of 3 organizations that each bring an area of expertise Clearly define the specific role and services each organization brings to the partnership Collaborations should be specific about: How/when/where services will be provided within the context of the partnership How patient/client services will be coordinated, monitored and tracked How the partnership will measure outcomes collectively 14
