3 1 OVERVIEW SUMMARY AND DEFINITIONS 1.1 ORGANIZATIONAL OVERVIEW The Emergency and Health Services Commission (EHSC) is provided under the Emergency and Health Services Act and has the legislated mandate to provide British Columbians with access to pre-hospital emergency services, and non-emergency health and information services. The EHSC is responsible for the BC Ambulance Service (BCAS), BCBedline service (BCBL), BC Trauma Program and other programs. Created in 1974 the BCAS is tasked with the provision of public ambulance service across the Province. BCAS is the largest provider of emergency medical services in Canada and one of the largest in North America. BCAS serves over 4.4 million British Columbians and responds to calls for emergency 911 and inter-hospital transfer services across six health authorities covering 944,700 square kilometres. In 2010/11 BCAS paramedics responded by ground to 471,568 events 379,713 prehospital (911) events, and 91,855 inter-facility transfers. BCAS also transported an additional 7,732 patients by air ambulance. BCAS employs 3,994 individuals 3,594 paramedic and dispatch staff, 250 support staff and 150 excluded management staff. BCAS operates from 184 ambulance stations, five administration offices, and three dispatch centres. BCAS has a fleet of 540 vehicles, including 478 ambulances and 62 support vehicles and dedicated ambulance aircraft. For more information visit BCBedline ( provides assistance for physicians with the timely transfer of acute care patients to the appropriate level of care and facility. It coordinates the transfer of all critical care patients and authorizes all transfers out of the province or out of the country. BCBedline is transitioning to the Patient Transfer Network service. Trauma Services BC is one of the newest Provincial program under EHSC established to coordinate and integrate trauma services across all regions. The goal is to create an optimal, efficient and inclusive system for BC. The EHSC Medical Programs Branch oversees the medically related functions of the Commission. This includes, but is not limited to, development of medical policy, standards and protocols for pre-hospital care; clinical performance management (monitoring the delivery of, and recommending means to improve patient care); medical education; and, integrating cutting edge medical research into the process of care. 4 1.2 CONTEXT FOR THE REQUEST Currently, pre-hospital care for 911 ground response has no formal direct physician clinical support of a predictable, archive-able and structured nature. This gap in pre-hospital ground clinical support does not exist for critical care transports or infant transport team programs in the BCAS. This type of support for ground paramedic 911 responses represents the standard of care for most North American EMS systems. The first purpose of this project is to establish a system of clinical support provided through direct communication technology to on call physicians defined as Primary Response Physicians (PRPs). With patient transfers of an inter-facility nature further assistance is needed in facilitating sending/receiving physician decision making; in prioritizing and coordinating patient transfers; in assisting inter-facility patient transfer paramedics; and assisting sending physicians with direct clinical advice and support as needed, in a timely fashion. Thus, the second component of this project is to assist the Patient Transfer Network (PTN) in the construction of an Emergency Transport Physician (ETP) cohort to provide clinical support in the management of complex inter-facility transports as required. It is important to note that ETPs will only participate on inter-facility transfers where an issue has arisen, and only provide clinical support to sending physicians where specialist patient transfer procedures do not currently exist. 1.3 SUMMARY OF REQUIREMENTS The purpose of this RFQ is to identify practicing physicians interested in providing the following services: Emergency Transfer Physician (ETP), to provide clinical support and impact assessments of complex inter-facility patient transfers, where specialist patient transfer procedures do not currently exist Primary Response Physician (PRP), to provide emergency clinical support to paramedics engaged in complex pre-hospital care Current plans for the service involve providing the ETP and PRP roles from remote locations via mobile technology. Each physician will provide a specified number of shifts per month. Some shifts will be ETP focused and some PRP focused. The intent is not to mix roles on shifts, though ETPs will provide emergency clinical support to inter-facility patient transfer ground-based paramedics. 1.4 DEFINITIONS Throughout the RFQ, the following definitions apply: 1. BC Bid means the electronic tendering service maintained by the Province; 2. BC Bid Website means the website, or any replacement, maintained by BC Bid at 3. 5 5. Qualified Physician means a Respondent to this RFQ who has satisfied any and all conditions set by the EHSC; 6. Respondent means an individual or company that submits, or intends to submit, a Response; 7. Response means a submission received by the EHSC from a Respondent in reply to this RFQ; 8. RFQ or Request for Qualifications means the process described in this document; 9. should or desirable means a requirement having a significant degree of importance to the objectives of this RFQ. 6 2 REQUIREMENTS, SERVICES, TERM & MAXIMUM AMOUNT 2.1 MANDATORY PHYSICIAN REQUIREMENTS ETPs and PRPs must meet the following criteria: Be certified in Emergency Medicine. Certification in Emergency Medicine is defined as one or more of the following: CCFP(EM), CCFP and ABEM, FRCP(C). *Physicians who are currently pursuing certification in Emergency Medicine through one of the above mentioned programs will be considered Be duly licensed to practise Medicine in British Columbia for all age groups with active current clinical appointment in an Emergency Department. 2.2 DESIRABLE PHYSICIAN REQUIREMENTS ETP Knowledge Required Orientation and on-going education will be provided for the following knowledge areas, though physicians with significant knowledge in these areas will be given preference: Knowledge of emergency service provision in British Columbia, including the EMS system and patient transfer processes in particular Knowledge of specialist procedures in British Columbia Knowledge of EHSC Services EHSC Paramedic Treatment Guidelines and Protocols EHSC Service Level Agreement and Performance Guidelines EHSC Policies (including Code of Conduct) BCAS/EHSC Systems & Processes BCAS fleet status and capabilities Knowledge of BC Healthcare Services Specialized units (Stroke, Trauma, etc) - Scope Of Practice, Hours of operation, Contact Numbers Hospitals and Other Facilities Capabilities, Hours of operation, Contact Numbers Knowledge of BC Transport Services - Capabilities, Restrictions, Contact Numbers and Hours of Operation Education and orientation will be provided for the following knowledge areas: EHSC EPOS Physician Guidelines EHSC PTN Processes PRP Knowledge Required Orientation and on-going education will be provided for the following knowledge areas, though physicians with significant knowledge in these areas will be given preference: EPOS RFQ v3.5 Page 6 of 18

8 2.3 SERVICES ETP & PRP Service Provision Current plans for the service involve providing the ETP and PRP roles from remote locations via mobile technology. Each physician will provide a specified number of shifts per month. Some shifts will be ETP focused and some PRP focused. The intent is not to mix roles on shifts, though ETPs will provide emergency clinical support to inter-facility non-ccp patient transfer paramedics if required. Both ETP and PRP services will require the following: Physicians on shift to be available and able to meet response time targets, and not engaged in other activities that might reduce that availability Documentation of every case that the physician provides advice for ETP Services It is expected that with most patient transfers, the ETP will not normally be called. ETPs will also NOT be engaged where existing specialist procedures exist. ETPs will provide the following services: Provide clinical assistance to Sending and/or Receiving Physicians involved in a patient transfer. Provide clinical support to inter-facility ground-based paramedics involved in a patient transfer. Provide clinical support to CTNs (Clinical Transfer Nurses) in determining the clinical impact of various resources, routing decisions, and priorities allocated to a patient transfer plan. The CTN will: Confirm the need for a high acuity transfer Triage cases (if required) and determine urgency and priorities of high priority transfers Confirm care required for transfer including special medical resources Facilitate and confirm appropriate destination of patient(s) Provide clinical assistance to Dispatch Officers in the PTCC (Patient Transfer Coordination Centre), where resource allocation or prioritisation conflicts exist. Provide support to the PTCC if solicited regarding determination of appropriate transport vehicle (ground, rotor, fixed wing) Provide medical guidance and assistance to physicians if solicited where patients are requiring care prior to transport. Provide clinical support in order to facilitate the preparation of the patient for transport including working (if required) with the CCP transport team, CC Transport Advisor or other specialists. 9 departments are better informed and up-to-date. Participate in, and provide medical expertise for, quality reviews in partnership with QSRMA, EHSC, health authorities and PTN in the context of providing EPOS services. Report to, and assist, the Medical Director for EPOS services on EPOS clinical standards, practices, protocols and processes. Engage in education, reviews, meetings and conference calls as requested by the PMD, including attendance in person at a bi-annual EPOS physician conference or meeting. The framework for the ETP role is as follows: Calls are expected to last from 10 to 30 minutes but may be longer, or require on-going assistance during the patient transfer, depending upon case load. Calls should be treated as high priority calls and will require rapid response times PRP Services PRPs will provide the following services: Provide emergency clinical support to paramedics involved in pre-hospital situations Provide liaison between hospitals and BCAS Paramedics to help resolve patient care issues and interactions Assist BCAS in meeting its continuing medical educational and quality improvement goals Provide medical input or subject matter expertise input with respect to policy or procedural development or educational content such that decision making departments are better informed and up-to-date. Participate in, and provide medical expertise for, quality reviews in partnership with QSRMA, EHSC, health authorities and PTN in the context of providing EPOS services. Report to, and assist, the Medical Director for EPOS services on EPOS clinical standards, practices, protocols and processes Engage in education, reviews, meetings and conference calls as requested by the Medical Director, including attendance in person at a bi-annual EPOS physician conference or meeting. The framework for the PRP role is as follows: Calls are expected to last between 2 and 5 minutes Calls should be treated as emergency calls and will require immediate response times EPOS RFQ v3.5 Page 9 of 18

10 2.4 TERM The Term of the Contract will be for one (1) year with an option to extend for additional three (3) 1-year Terms at the sole discretion of EHSC. 2.5 MAXIMUM AMOUNT The estimated maximum amount of the Contract is $65,000 per year based on an estimated time commitment of 450 hours per year provided at the following compensation rates: 1. Hours Daytime hours: 07:00 19:00 Evening hours: 19:00 23:00 Night-time hours: 23:00 07:00 Note that physicians are expected to provide 15 minutes after their shift to transition any calls from one shift to the next at no extra compensation. 2. Shifts 3. Types of Pay Physicians will provide an equal number of Daytime and Night Shifts. Shifts will either be ETP or PRP shifts; the type of shift the physician will provide will be dependent on call volumes (to be determined) and as required by the Medical Director. Physicians will provide both ETP and PRP shifts Shifts will be 12 hours + 15 unpaid transition minutes A Night Shift = Evening hours + Night hours Physicians providing a Night Shift may NOT provide availability for Evening hours only, unless requested by the Medical Director. Daytime On-Call Pay: $30 per hour for being available to receive calls. Evening On-Call Pay: $30 per hour for being available to receive calls. Night-time On-Call Pay: $40 per hour for being available to receive calls. Daytime Call-Received Pay: If a call is received, the physician will be paid one (1) hour of normal BCMA specialist physician rates (currently $ /hour). Subsequent calls during that hour will not incur additional compensation. The hour will be deemed to be concluded when sixty (60) minutes has passed with no calls received, whereupon On-Call-Pay will resume. Night-Time Call Received Pay: If a call is received, the physician will be paid normal BCMA specialist physician rates (currently $ /hour) for each full hour remaining on that shift, with the minutes remaining in the hour the call is received counting as one full hour. 4. Night Shift Payment Night Shift will be paid according to the following formula: For Evening hours: Daytime Shift payment rates For Night-time hours: Night-On-Call-Pay + Night-time Call-Received-Pay 6. Expenses necessarily incurred as a result of providing the Services will be reimbursed on the same basis as the EHSC pays its Group II employees when they are on travel status. 12 3 RESPONSE 3.1 RESPONSE REQUIREMENTS Responses must be provided according to the following requirements. If interviews are conducted, the interview will be evaluated and scored out of 100 making the total evaluation score of 200. 13 3.3.1 Response Review Committee Responses will be reviewed by a small group of EHSC staff, including at least one practising physician Review and Selection The response review committee will check Responses against the mandatory criteria. Responses not meeting all mandatory criteria will be rejected without further consideration. Responses meeting the mandatory criteria will then be assessed and scored against the desirable criteria and, where applicable, the interview criteria Enquiries All enquiries related to this RFQ, including any requests for information and clarification, are to be directed, in writing, to the RFQ Contact Office identified on the cover page. 18 APPENDIX D - RFQ TERMS AND CONDITIONS Respondent s Expenses Respondents are solely responsible for their own expenses in preparing a Response and for subsequent negotiations with the EHSC, if any. The EHSC will not be liable to any Respondent for any claims, whether for costs or damages incurred by the Respondent in preparing the Response, loss of anticipated profit in connection with any final Contract, or any other matter whatsoever. Acceptance of Responses This RFQ is not an agreement to purchase services. The EHSC is not bound to enter into a Contract with any Qualified Physician. Responses will be assessed in light of the qualification review criteria. The EHSC will be under no obligation to receive further information, whether written or oral, from any Respondent. Definition of Contract Notice in writing to a Respondent that it has been identified as a Qualified Physician will neither constitute a Contract nor give the Respondent any legal or equitable rights or privileges relative to the service requirements set out in this RFQ. Only if a Qualified Physician and the EHSC enter into a subsequent full written Contract will a Respondent acquire any legal or equitable rights or privileges. Modification of Terms The EHSC reserves the right to modify the terms of this RFQ at any time in its sole discretion. This includes the right to cancel this RFQ at any time without entering into a Contract. Ownership of Responses All documents, including Responses, submitted to the EHSC become the property of the EHSC. They will be received and held in confidence by the EHSC, subject to the provisions of the Freedom of Information and Protection of Privacy Act. Confidentiality of Information Information pertaining to the EHSC obtained by the Respondent as a result of participation in this RFQ is confidential and must not be disclosed without written authorization from the EHSC. Collection and Use of Personal Information Respondents are solely responsible for familiarizing themselves and ensuring that they comply with the laws applicable to the collection and dissemination of information, including resumes and other personal information concerning employees and employees of any subcontractors. If this RFQ requires Respondents to provide the EHSC with personal information of employees who have been included as resources in response to this RFQ, Respondents will ensure that they have obtained written consent from each of those employees before forwarding such personal information to the EHSC. Such written consents are to specify that the personal information may be forwarded to the EHSC for the purposes of responding to this RFQ and use by the EHSC for the purposes set out in the RFQ. The EHSC may, at any time, request the original consents or copies of the original consents from Respondents, and upon such request being made. Respondents will immediately supply such originals or copies to EHSC.
