All candidates are required to submit their proposal as a recognized Bidder, by registering for an e-builder username and password. History and Background of The MetroHealth System The MetroHealth System (MetroHealth) is one of the largest, most comprehensive health care providers in Northeast Ohio, serving the medical needs of the Greater Cleveland community since More than 400 primary care and specialty care physicians practice within The MetroHealth System. Affiliated with Case Western Reserve University (CWRU) School of Medicine since 1914, MetroHealth is a center for medical research and education, with all active staff physicians holding CWRU faculty appointments. The System also includes an Out-patient Surgery Center and seventeen Community Health Centers (7,000 square feet - 60,000 square feet) which are conveniently located throughout Cuyahoga County providing a wide range of primary care and specialty services. The MetroHealth System is a County Hospital system, organized and operated under Chapter 339 of the Ohio Revised Code. II. Project Overview The MetroHealth projects include the renovation of an existing Class C operating room to a new Hybrid OR and the renovation upgrade of 19 existing Class C operating rooms in conjunction with a capital equipment purchase consisting of new surgery tables, light/equipment booms and electrical upgrades to the operation suites. Professional Design services have been acquired under a separate solicitation and the team of Bostwick Design serve as the Architect & Engineer of Record for the project. 2 III. Scope of Services MetroHealth is seeking statements of qualifications for construction management services as identified herein. The Construction Manager at Risk (CM) will provide construction management services under the terms and conditions of the proposed form of contract based on AIA Document A Standard Form of Agreement Between Owner and Construction Manager as Constructor where the basis of payment is the Cost of the Work Plus a Fee with a Guaranteed Maximum Price. The Owner- Construction Manager Agreement may be assigned in the future to a developer as selected by MetroHealth. A. Preconstruction Phase Services The Construction Manager will provide preconstruction phase services for the Project. The Construction Manager will assign a team to the Project who will be the same team that will be assigned during the construction phase to work cooperatively with the Owner and A/E team. The CM team will be responsible for performing the following duties including, but not limited to, cost estimating, value engineering, constructability review, scheduling, and preconstruction planning throughout the preconstruction phase. The CM shall provide recommendations with regard to accelerated or fast-track scheduling, procurement, and phased construction. During the development of the drawings and specifications, the partially completed documents shall be provided to the CM. The CM shall submit to the Owner and A/E their proposed Guaranteed Maximum Price (GMP) and its qualifications and assumptions based upon the documents. The Owner, A/E, and CM shall meet to reconcile any questions, discrepancies, or disagreements relating to the GMP and the qualifications and assumptions. This project will be open-book, including, but not limited to, bids on all trade packages, general conditions, and fees. The CM shall then submit to the Owner, for approval, the CM's proposed final GMP based upon the documents including the approved qualifications and assumptions. Contingent upon the Owner s approval of the final GMP, the parties will execute an amendment to the CM agreement establishing the GMP. The final negotiated GMP shall not exceed the project budget established for construction. If the proposed GMP exceeds the budget, then the Owner may terminate the agreement with the CM and seek other firms to complete the Project. B. Construction Phase Services The CM shall construct the Project pursuant to the construction documents and in accordance with the schedule requirements. The CM shall hold all subcontracts and shall be fully responsible for the means and methods of construction, project safety, project completion within the schedule agreed upon in the preconstruction phase, compliance with all applicable laws and regulations including monitoring compliance with all diversity participation, equal employment, and prevailing wage requirements, and submitting monthly reports of these activities to the Owner. Anticipated Construction Costs The estimate of probable construction cost for these project are: Hybrid OR - $830,000 OR Renovation/Upgrade - $1,399,000 VI. Community Business Enterprises Participation MetroHealth wants to ensure that minority, women-owned, historically underutilized and other disadvantaged business enterprises, as well as local small businesses and their employees (together known as Community Business Enterprises or CBEs ) have a fair and reasonable opportunity to participate in procurement activities. Contractors, suppliers, and service providers are expected to make a Good Faith Effort to efficiently maximize the voluntary CBE participation. It is understood and accepted that participation by CBE s is not possible for all elements of the project. MetroHealth is committed to facilitate and promote the participation of CBEs in all construction contracts, advance local economic revitalization, and achieve non-discrimination in the award and administration of the contracts. VII. Proposal Content To facilitate the review of the anticipated responses, each candidate firm shall submit its information in one bound volume divided into two tabbed sections. Tab One shall include the firm s information submitted on State of Ohio Statement of Qualifications form F , in accordance with the stated requirements. All references within form F to State projects or experiences shall herein be redefined to include all projects and clients. 4 Procurement tab, and choosing Statement of Qualifications under Procurement Forms. All contents included in Tab One shall be standard letter size, 8½ x11. All content pages shall be consecutively numbered. The proposer shall identify if it is currently certified as a Community Business Enterprise. 2. The proposer shall note whether or not its principal place of business is located in Cuyahoga County, Ohio 3. Explain your experience and approach to engaging other Community Business Enterprises? 4. Judgments: Provide a list of all convictions or fines assessed against the Candidate firm or any of its officers or directors for violations of State or Federal Law. Section I Authorized Representative Tab Two (optional) shall include supplemental information not already provided in Tab One which is conducive to highlighting the firm s expertise relevant to the proposed work with MetroHealth. The contents included in Tab Two may be of any page size not to exceed ledger size, 11 x17. All content pages shall be consecutively numbered. VIII. Selection Criteria The following criteria will be used by the Owner to evaluate each Candidate: 1. Responsiveness to this RFQ. Completeness of qualifications in responding to the information requested and in the designated format. 2. Current and past experiences of the proposed firm and its identified personnel on projects of similar size, scope and complexity. 3. Qualifications (including technical training and education) and relevant experience of the key personnel. 4. Ability of the firm in terms of its workload and the availability of qualified personnel, company resources, and facilities to perform the required services competently and expeditiously. This includes the availability of the key personnel to commence active engagement on the project. 5. Demonstrated knowledge of the firm s ability to manage the project schedule and team activities in a timely manner. January 30, 2017 The MetroHealth System Page 4 of 6

5 6. The firm s understanding of the strategies to complete the project s objectives as completely and effectively as possible. 7. Relevant project and client references and evidence of successfully executing similar projects for the firm and designated project personnel, including sub consultants, for projects of a similar nature. 8. Any other relevant criteria as determined by the Owner. This RFQ sets forth the intent of MetroHealth as to the procedure and criteria by which the construction management firm will be selected and the proposed terms of the agreement between MetroHealth and the successful Candidate. Except as required by statute, MetroHealth reserves the right, in its sole discretion, to modify this procedure and criteria until an agreement between MetroHealth and a successful Construction Manager is executed. MetroHealth reserves the right, in its sole discretion, to have any Candidate firm clarify or supplement its submittal, including making such request through direct contact with the Candidate firm. MetroHealth may hold discussions with individual firms to explore further the firm s statement of qualifications. Based on the identified criteria, MetroHealth will request pricing from the top three CMr candidates in the form of a pricing and technical proposal. Any attempt to influence any member of MetroHealth with regard to this RFQ prior to the selection/decision will be grounds for disqualification. MetroHealth will do business only with firms which provide for Equal Employment Opportunity pursuant to Section of the Ohio Revised Code. MetroHealth reserves the right to assign this contract to a developer. In submitting its Statement of Qualifications, the Candidate firm agrees that it will accept the future assignment of its contract to a developer selected by MetroHealth. IX. Submission Instructions Submit one (1) electronic copy via e-builder bidding module for the project prior to deadline stated. Submit two (2) hard copies directly to: MetroHealth Medical Center Center for Campus Transformation 3525 West 25 th Street Cleveland, OH Electronic submittals should be combined into one PDF file. The file name shall include the project number listed on the RFQ and the Candidate firm s name. Facsimile or ed copies of the submittal will not be accepted. 6 Plainly marked on the outside of the package shall be MetroHealth Hybrid Operating Room (OR) CM and OR Renovation Upgrade CM Each candidate is individually responsible to ensure that its submittal meets the delivery requirements to the designated delivery destination at the designated time. Candidates shall not rely upon the use of FedEx, UPS, or any other package or mail delivery service that will only provide a drop service at the loading dock or mail room. X. Other Instructions Notice of Intent to Respond Upon receipt, review and consideration of this RFQ, each candidate intending to submit an RFQ response shall indicate Will Bid in the e-builder bidding module. The Intent to Respond shall be received no later than the date and time noted previously in this RFQ. Candidate firms are not to rely on any oral instructions or answers. Oral responses affecting the information provided by the Candidate firm in its submittal will not be binding on MetroHealth, its employees, agents, or representatives. 1. No access to the site is permitted. 2. No visits will be allowed. 3. MetroHealth reserves the right to reject any or all RFQ responses, to select any response as the basis for negotiation of a contract, and to exercise its discretion and apply its judgment with respect to any aspect of this request, the evaluation of responses, and the negotiation and award of any contract. 4. MetroHealth reserves the right to reject any, part of any, or all submittals received and to waive informality in any submittal. The construction management firms whose submittals are not accepted will be notified after a binding contractual agreement between MetroHealth and the successful construction management firm exists, or if MetroHealth rejects all submittals. 5. By submission of a RFQ response, Consultant agrees to preserve the confidentiality of information of the Owner used by the consultant or to which it shall be granted access for purposes of preparing the response or in performance of work pursuant to any contract resulting from the response. Such information regarding the Owner shall be protected from disclosure by the consultant and access thereto, limited to the consultant or those having a need therefore in performance of services to the Owner. Such information may not be disclosed to any other persons, or entities whatsoever without prior written consent of the Owner or unless such information shall have become otherwise generally available, without fault of the consultant. 6. MetroHealth assumes no responsibility, and no liability, for costs incurred in the preparation or submission of any RFQ response. 7. If provided by the Owner, any provided documents are complimentary. They are provided for use in preparing a response for the requested service only. MetroHealth cannot guarantee their accuracy.
