5 Cancer Control and Prevention: Integrating Clinical and Public Health Efforts April 4, 2018 Grappone Center ~ Concord, NH Annual Meeting Objectives: Explain the difference between population health management and total population health and how these terms relate to cancer control and prevention. Identify access to care challenges faced by patients with cancer in NH. Identify strategies to reduce health disparities and inequities in cancer control and prevention. Describe how the social determinants of health affects prevention, screening, and treatment and discuss strategies to reduce their impact. More details to come!

6 Diabetes Today 2018 Whole Person Care: a coordinated and integrated approach to Diabetes Management Learning Objectives at the conclusion of this program participants will be able to: Discuss updates and guidelines to the basal insulins, sglt-2is, GLP-1RAs and DPP4s, and the most recent CVOTs (cardiovascular outcome trials) Identify treatment options for the inpatient and outpatient settings. Identify resources for the low income diabetes patient to get what they need. 7 May 4, 2018 UpReach Therapeutic Equestrian Center Goffstown, NH Target Audience: Medical Providers, Community Health Workers, Nurses and Medical assistants who provide care to individuals who have experienced various forms of trauma. Learning Objectives: 1. Providers will have increased knowledge of the effects of childhood trauma as it relates to physical, mental and emotional health. 2. Providers will be able to look at their own health care setting and identify possible procedures, policies, or physical structures that could trigger patients. 3. Providers will be able to describe three interventions or skills that they can use to decrease the trauma response in patients. Program is free. More details to come!

10 Stanford s Chronic Disease Self-Management Program: Better Choices, Better Health Do you have patients with a chronic disease? Are you looking for resources to help them take better care of themselves? The NH CDSMP Network is offering Stanford s Chronic Disease Self-Management Program: Better Choices, Better Health as a resource to providers and residents of New Hampshire. The program gives patients with chronic diseases and the people who care for them an opportunity to learn how to put life back into their lives.
