7 Outcomes to be achieved by the end of Year 3 S1 1. Participate in the care of 2-3 patients holistically; manage all aspects of the patient s needs/ problems. 2. Perform assessment, planning, implementation and evaluation when caring for the patient. 3. Document findings of assessment in the patient in the patient s records (including charts). 4. Provide health education and/ or instructions to patient and family. 5. Assist in-charge in managing 2-3 patients 6. Present one case study outlining (verbal bedside presentation). 6.1 Patient s current issues / needs and the causes. 6.2 Interventions that reduced and solved these issues / needs. 6.3 Evaluation of the interventions and treatment 6.4 Presentation include: Patient s primary diagnosis, Able to relate pathophysiology, medication and investigation. Able to link issues based on patient s assessment Covers the listed categories

8 Operating Theatre Setting Year 3 S1 (Learning Outcomes to be achieved at the end of the placement) 1. Participate in the nursing management of client at the OT reception. 2. Participate in the care of client during anaesthesia (general, regional and local). 3. Participate in the management of the client as scrub and/ or circulating nurse. 4. Observe the roles of the reception, scrub, circulating, AU and recovery room nurse within the operating theatre complex. 5. Participate in the nursing management of client in the recovery room/ PACU. 6. Write a reflection on the clinical experience during the c linical posting. 7. Share the reflection during the debriefing session.

9 Peace Haven Nursing Home 1. Shift duty: 8 4, Mon - Fri 2. Simei - nearest MRT. Still need to take SBS 5 to reach the home Tampines MRT - SBS 37 as well as SBS 2 Alight at Japanese School and walk to 9 Upper Changi Road North 3. Not within reach of any food courts - need to take a bus. Lunch is catered by their kitchen - not halal certified. However the students can purchase a meal coupon $3 from finance. Meal - main course, soup with dessert The muslim students will have 'pack a lunch' 4. The students will have to share a locker per RLA

10 Gerontology Setting Year 3 S1 (Learning Outcomes to be achieved at the end of the placement) 1. Participate in the care of 2-3 patients holistically; managing all aspects of the patient s needs/ problems. 1.1 Use valid and reliable assessment tools to guide nursing care for older adults. 1.2 Plan and implement patient-centred care for older adults with physical and cognitive needs (with emphasis on preventing risk and promoting quality and safety e.g., falls, medication mismanagement, pressure ulcers). 1.3 Use resources and programmes to promote functional, physical and mental wellness. 1.4 Monitor and evaluate outcomes of care. 2. Document findings of assessment and care in the patient s care records (including charts).

Discuss mistreatment (physical, mental, or financial abuse, and/or self-neglect) in older adults and the preventive measures at family and community level (including national policies). 4. Participate in inter-professional teams by understanding the roles of other members, in particular those of older adults, families and other healthcare professionals. 5. Discuss measures to manage an environment suitable for older adults/ residents with cognitive impairments. 6. Provide health education and/ or instructions to the patient and family/ caregivers. 7. Discuss ways to promote dignity in the care of the older adults. 8. Explore issues related to end of life care of elderly. 9. Assist in-charge in managing 2-3 patients. 10. Write a reflection on the clinical experience.

12 Hospice Care Setting end Year 3 S1 (Learning Outcomes to be achieved at the end of the placement) 1. Practise basic communication skills. 2. Observe management of symptoms like pain, nausea and vomiting, constipation and symptom distress. 3. Participate in interdisciplinary care of patients. 4. Provide the following nursing interventions: 4.1 oral hygiene. 4.2 meeting nutritional needs. 4.3 skin care (e.g., positioning, hygiene, moisturizing) 4.4 care of patients with pressure ulcers and other wounds. 4.5 bowel care and stoma care. 4.6 care of patients with drains and catheters. 4.7 care of patients with intravenous therapy / syringe driver. 4.8 care of patients on oxygen therapy.

13 Hospice Care Setting end Year 3 S1 (Learning Outcomes to be achieved at the end of the placement) 5. Explore psychosocial and spiritual issues commonly encountered at end of life. 6. Explore ethical and legal issues in end-of life care including advanced care planning. 7. Recognize own self care needs and team support. 8. Gain knowledge of hospice and home care eligibility, services and administration. 9. Write a reflection on the clinical experience. 10. Present a reflection of clinical experience during the debriefing session.

14 Emergency Care Setting Year 3 S1 (Learning Outcomes to be achieved at the end of the placement) 1. Participate in the assessment of patient s airway, breathing and circulation status. 2. Perform equipment checks such as air viva function test & defibrillator check. 3. Participate in triage, emergency resuscitation. 4. Work in collaboration with healthcare team in the care of patients with any the following conditions: - medical emergencies. - trauma emergencies. - poisoning 5. Write a reflection on the clinical experience during the clinical posting. 6. Share the reflection during the debriefing session.
