April 2017, a.m. The medication regime and its administration is a central process for hospitals. However, in the advancing digitisation of the facilities this cannot be separated from other medical processes especially against the backdrop of drug therapy safety. After all, the medication does not only affect the pharmacy or the station. It already takes effect in the patient intake and works until the patient s discharge and beyond. The service centre management and even endoscopy or radiology are also affected. With that in mind it is obvious that the medication regime must be viewed holistically and should be mapped seamlessly within the hospital information system. "With ORBIS Medication, a management system for the entire medication process, Agfa HealthCare allows this to happen from the master data system in the pharmacy to the doctor's prescription to the nursing documentation of the medication delivery via work lists and the temperature curve," explains Christoph Brandes, Head of Country Solution Management DACH (Germany, Austria and Switzerland) for ORBIS. Further functions are also implemented including integration into the doctor's letter, complete and structured archiving of the relevant documents as well as assessments of the medication. "Through the use of ORBIS Medication, both physicians and nurses gain an accurate overview of the drug therapy of a patient. The past, the current and the planned administration and dosage are shown in ORBIS Medication. This eases documentation and helps to avoid mistakes," says Brandes.

For this purpose, products from the company MMI are integrated via webservice in ORBIS, which double checks drug risks. "An essential part of ORBIS Medication is a function which can crucially support medical decisions in drug administration by, for example, displaying warnings on drug interactions, contraindications or dosage instructions," Brandes explains. In the same way, the underlying scientific database monitors contraindications based on the diagnoses being made to ensure that the medication regime matches the patient's diagnoses. As well as an overview of the current medication on the ward, the pharmacist's workplace is also able to communicate directly with the medical service by pharmacological recommendations. Thus, ORBIS Medication makes a process-controlling function available to the pharmacist. April 2017, a.m. The invoicing of services is as complex for hospitals as it is vital after all, appropriate registration and coding of the services provided is the basis for corresponding remuneration. With ORBIS Assisted Coding, Agfa HealthCare provides users with valuable work support. The rules and procedures can be itemised directly into the hospital information system (HIS), which helps to later improve clinical decisions and establish processes. "ORBIS Assisted Coding is based on a semantic text analysis, so it identifies relevant text passages from ORBIS-case records. The program analyses content from all important documents doctor's letters, surgical reports, histology and other documents and then suggests ICD and OPS codes to the DRG controller, which after selection help to determine the DRG and thus can be used to increase revenue," said Christoph Brandes, Head of Country Solution Management DACH (Germany, Austria and Switzerland) for ORBIS, explaining how the solution works. "True to our philosophy of integration, the user works with ORBIS the whole time in the standardised environment of ORBIS Assisted Coding. No external application is launched, but rather all procedures are mapped for the user in ORBIS. The interface thus allows for loss-free processing of textual and structured information in the background," says Brandes. This makes it a very powerful assistance program, which in turn makes life easier for the DRG controller. "You can imagine what it is like when examining a paper-based file," says Brandes. "The person responsible for the DRG marks relevant parts of the documentation for coding in the

6 conhit PRESS INFORMATION: ORBIS ASSISTED CODING Page 2/2 file and assesses this later." ORBIS Assisted Coding automatically takes over this timeconsuming process. It can focus entirely on determining the right DRG. "ORBIS transfers data in the background via a specially developed interface to a semantic rules engine of the partner 3M. Based on the transferred ORBIS content, this application determines suggestions for the relevant diagnostic and procedure codes. In addition to this, the system asks intelligent questions to improve coding and can speed up an MDK process by combining codes with the corresponding text sections. April 2017, a.m. Doctors and nurses do not want to be bound to a permanent workstation in order to retrieve information about patients and their treatment. ORBIS ME!, the mobile version of the hospital information system by Agfa HealthCare, gives them that freedom. Thanks to the continuous syncronising between the mobile terminal and the database, all data remain up to date on all workstations the solution resulted in a paradigm shift with the change towards open HTML5 technology, and the application now runs on all tablet computers, as well as other mobile devices such as notebooks. "Our customers favour the independence of the mobile operating system," says Christoph Brandes, Head of Country Solution Management DACH (Germany, Austria and Switzerland) for ORBIS. This independence has very practical advantages for health care institutions: They are free to decide which devices to purchase. Different generations of mobile devices with different operating systems can all be used at the same time. "We also noted that our strategy of keeping mobile functionality identical to the desktop PC in terms of display and functional operation has been well received by our users," adds Brandes. The goal: to make data more mobile, and to make it available exactly where it is needed by the patient's side. ORBIS ME! can be seamlessly integrated into the hospital-wide information system and makes all patient information available at all times online and offline on mobile devices. With personalised favourites list, access history, or the

8 conhit PRESS INFORMATION: ORBIS ME! Page 2/2 selection of a specific department or station, users are given flexible options to facilitate their access to patient information. ORBIS ME! provides access to the patients' records across stations, including the acquisition of vital signs and files. Photos can be attached to the patient's file and dictations can be assigned to newly created forms. In the current version it is possible, for example, to send requests to the radiology department or the laboratory. "The cumulative laboratory findings are displayed in the same way as they were with the ORBIS desktop solution, and all this directly on the mobile device," explains Brandes. Those who use ORBIS Medication can prescribe drugs directly through ORBIS ME! and have an overview of all medications in list format or as a graph. In addition, the nursing staff can confirm when medication has been administered while they are by the patient's bed. 9 conhit CUSTOMER CASE Konrad-Zuse-Platz 1-3 D Bonn Head of Marketing Communication T martina.goetz@agfa.com Medication process management Klinikum Lüneburg continues to speed up digitalisation further with ORBIS Medication Digitalisation always means change not only of procedures and processes, but also in the minds of employees. The introduction of ORBIS Medication, including the PDOK, PPRO and KURV modules of Agfa HealthCare, took over the patient documentation at the Klinikum Lüneburg and reduced the patient paper records to a minimum. This change is described by Gabriela Renter, Head of Division for Patient and Process Management, as a milestone for all occupational groups involved in patient care. "The procedures have changed positively for all participants. However, it takes some time of familiarization until the new processes take hold." Before the digitalisation, nurses took care of their patients, for instance, during the morning shift, using the information from the paper chart and entered all treatment results in there. "Afterwards, the paper charts were on the move all day, either with the doctor who had to write the doctor's letter, in a diagnostic department or in the operating theatre for further treatment," explains Patrick Evel, the deputy director of nursing. "Care services were documented shortly before or shortly after shift transfer." Since 2016, the Klinikum Lüneburg has expanded its ORBIS KIS by the modules PDOK, PPRO, KURV and Medication. "Today, all nurses document their own services at the medical trolley directly in the hospital information system (KIS). There a nurse can then also access results and other information on her patients," says Korinna El-Cheikh, site manager at the clinic, responsible for IT.

10 conhit CUSTOMER CASE: ORBIS MEDICATION AT KLINIKUM LÜNEBURG Page 2/5 New processes with digital drug management At the Klinikum Lüneburg medicine is prescribed exclusively by the doctor and then put on the stations by employees of the pharmacy. "The four-eyes principle between the doctor and the pharmaceutical-technical assistant (PTA), therefore, is granted, which ensures better patient and medication safety," says Patrick Evel. ORBIS Medication is now used successfully in almost all inpatients. However, the way to get there was not easy, which was, among others, due to the archiving. "Per prescription and periodically for drug administration, the module generates a new PDF file," describes KIS coordinator Marc Papenberg the situation. "In case of a longer period of inpatient care, the patient documentation is likely to surpass 1,000 pages. In order to ensure a clear and resource-preserving archiving, an adaptation of the previously valid archiving logic was necessary. In cooperation with Agfa HealthCare, the archiving was optimized. Today, for the abovementioned modules chronological and/or active substance-prepared summaries are created on a few pages, which are archived in a revision-safe manner in the Enterprise Content Management System HYDMedia after the patient has been released. And how was the collaboration with the IT provider? "We have grown together very well," says Gabriela Renter, "and we are now convinced that we have chosen the right partner. A close cooperation and certain flexibility were essential in this pilot project. "We always had a dedicated contact at Agfa HealthCare at our disposal," emphasizes Korinna El-Cheikh. Good preparation, smooth introduction The mutual introduction of the care modules, the chart and ORBIS Medication also required flexibility from both sides. In order to prepare and accompany these, the clinic completely released one employee from the nursing service and one from the occupational management from duty. The core work group around Gabriela Renter, Patrick Evel and Marc Papenberg at Medication supported by the head of the pharmacy, Manfred Kühne continuously involved all the occupational groups of the Klinikum Lüneburg at the single steps. "When we introduce software, we think from the existing processes. These should be

At the same time, though, we are looking at how the processes can also be adapted to the software. With a project of this size, it also always leads to the reconsideration and optimization of existing processes," explains Gabriela Renter. Owing to the good preparation, the rollout in the clinic went as planned. At least one station could be changed per month. "I don t know any clinic that has managed to connect 13 stations with four central medical documentation modules within 365 days," says Patrick Evel proudly. "This requires a high level of commitment and much energy from all stakeholders." Due to the early involvement of the individual specialist divisions, it was possible to incorporate and implement individual requirements. "It was also a great advantage that the company management supported the project at all times," adds Marc Papenberg. What were the challenges for the introduction of the chart for instance? Before the introduction, it was determined that the view of the digital fever chart is identically structured in all specialty departments. "Considering the large number of interdisciplinary patient occupancy, it is unacceptable for employees to work with different views," says Patrick Evel. This requires the widest possible standardization of processes. "In the interdisciplinary project group, it was, therefore, determined in advance which sections of the fever chart are to be displayed in which order. It was also talked about uniform procedures on the stations. However, the latter fails in practice from time to time, so that in the real clinic we occasionally had to find a compromise between the individual requirements of the respective specialty departments. Better, faster and more transparent processes In terms of drug management, the consistent specification works very well. Patients who are coming to a planned surgery are first of all received by a PTA in the pre-admission consultation hours. She creates a drug history and enters it into the medication record of ORBIS Medication. Medication that is not part of the house list is entered as free text. Then the treating doctor controls the medication during the patient history. If the patient appears to be admitted to the station, a pharmacist substitutes the medication according to the

On the stations, the drugs are supplied by a PTA at a fixed time. If a patient is hospitalized via the emergency department, the PTA already stores the medication during the day at the medication record as well, transfers it to the hospital medication and the treating physician releases it with his signature after verification. In the night service the medical service takes over the intake of the medication. "Since we have established this process, we have been able to significantly reduce the time factor for the drug entry and relieve the medical service. The nursing staff also benefits from this approach, emphasizes Gabriela Rentner. According to consensus view, with ORBIS Medication, the quality of the medication management has also improved considerably. "Any nurse or physician can track changes immediately, regardless of the location in the clinic. Doctors no longer have to go to the station to prescribe or release new drugs. They are supported by an automatic AMTS interaction check, which provides indications for interactions between medicine and contraindications according to the recorded patient diagnoses," stresses Gabriela Renter the benefits of the software. The medication software also facilitates the work in the pharmacy, "We are able to implement much more quickly and better medicines of our patients, which are not included in the house list," says chief pharmacist Manfred Kühne. "The pharmaceutical consultation for doctors has also become more effective, there is a closer exchange in terms of medication and substitute products. And it all works better and faster than at analogue times with paper." Gabriela Renter also draws a positive conclusion, "ORBIS Medication is a good and worthwhile investment in patient and drug therapies safety at the Klinikum Lüneburg." The clinic will continue along this path. "For the next step, we would also like to get support from ORBIS Medication for outpatients. We deem a consistent process as important and it simply starts in the outpatient clinic, describes Korinna El-Cheikh the further planning.
