1 Neighbourhood Care Networks Newsletter Welcome to the Neighbourhood Care Networks (NCN) Newsletter for ABuHB. This regular newsletter aims to keep you updated about how our NCNs are progressing their plans and working with partners to deliver community based services that meet the needs of the population served. A note from Dr. Liam Taylor, Deputy Medical Director It has been another busy year for our NCNs in Gwent where we have seen many innovative projects and programmes delivered through the leadership of NCNs. The networks have also offered real opportunities for re-design on a larger scale with the potential for Gwent-wide projects and further close working with partner organisations and the Health Board s own Secondary Care services to genuinely realise the benefits of having integrated health organisations in Gwent. We are now implementing our NCN Plans for 2017/20 whose key priorities have been informed by our Practice Development Plans. Popular areas for themes include Social Prescribing, tackling obesity and furthering the development of Health & Social Care Hubs. Dr. Liam Taylor, Deputy Medical Director NCNs have also been asked to choose either three of the National Clinical Priorities from the list below, or two areas and a local issue to address. Cancer COPD Dementia Pathway Liver Disease Improved Mental Health and Well-Being Following discussion at NCN meetings, it was agreed that the Liver Disease Pathway would be adopted by all 12 NCNs. 2 Putting NCN Plans into action what we ve achieved so far... Across Gwent Urology GP Advice Line The Urology Service provide an advice line to support Primary Care to care for your patients with urology problems. The service aims to respond to routine general medical advice queries via within 48 hours. Requests will be directed to the most appropriate health care professional. Please note that enquiries related to waiting times, or appointments will not be responded to. NCNs to play a key role in the Clinical Futures Level 1 programme The Partnerships & Networks Team met with Dr. Emily Warren, Interim Programme Lead for Clinical Futures Level 1 who will lead the work with our partners to transform services, to provide more care closer to people s homes. 3 The Cluster Development Project Team led by the Primary Care Hub will evaluate progress and look to further develop training resources to meet the needs of Primary Care clusters, which Dr. Andrew Gray noted are now the place to go and get things done...". Dr. Isolde Shore-Nye added the Confident Leaders programme also gave the opportunity to share learning and experience between clusters from across Wales. It is planned that the first 2 cohorts of the programme will merge to create an expanded network to continue opportunities to work across clusters and to develop new ideas and ways of working. Super 6 achieve IQT Silver Award! On Thursday 11 th May, 6 NCN Teams delivered their final project presentations for the Aneurin Bevan University Health Board accredited model for improvement programme at the NCN Improving Quality Together (IQT) Silver celebration event. NCN IQT Silver project teams ABCi delivered a tailored course for NCN teams over 5 half-day sessions every 8 weeks to equip them with the knowledge and skills in quality improvement to apply the Model for Improvement within their own services. The teams were given the skills to process map their problem, translating their knowledge into driver diagrams, undertaking several PDSA cycles, plotting their results onto run-charts to monitor their successes (and failures!) and above all promoted collaborative working across the NCNs. Michele Zeraschi reflected that her project has improved sharing of patient/ client information, improved relationships between staff, and improved understanding of each other s roles and the challenges each other faces. 4 The Partnerships & Networks Team would like to congratulate everyone who took part and extend their thanks to their ABCi team for their support! Launch of Gwent Mental Health Consortium Gwent Mental Health Consortium, comprising of 8 organisations within Gwent including Mind, Communities Mood Improvement Groups (CMIG), Growing Space, Inside Out and Hafal. launches on 20th July. Mental Health is an issue raised within many of the NCN plans. The service can support practices through their provision of counselling; advice, information and advocacy; training and community wellbeing. The new service addresses the inequity of mental health services within Gwent for those over 18 years of age by ensuring equal access to services, by professional or self-referral. The new Consortium: provides an opportunity for service providers to collaborate and share their skills enhances members main services, which will be retained at a local level, with a seamless integration of Gwent wide service available to all. The single point of access is controlled by a Common Database Co-ordinator. To ensure that referrals are dealt with efficiently and signposted accordingly they have received Mental Health First Aid and Applied Suicide Intervention Skills Training (ASIST). Services are available five days a week and an answer phone service is available on weekends. Telephone (Single point of access for professional and self-referral) Independent Dental Advisor update Hannah Hutchinson is the conduit between dentists in the ABUHB area and the Health Board. Recognising the impact of oral health on themes within the NCNs, who she represents in a range of forums including the LDC, Dental QPS, IOHG, and MCN for Special Care Dentistry. Hannah Hutchison, Independent Dental Advisor Hannah is currently undertaking a workpiece focusing on Dementia Care Pathway with Dental Advisors and has met with Dental Advisors across other Health Boards in South East Wales. 5 Independent Optometry Advisor update Stephanie Campbell, Dr. Stuart Thomas (Caerphilly East NCN) and Hannah Hutchison (Independent Dental Advisor) are working towards training Clinicians in Caerphilly East NCN as the first Dementia Friendly NCN. Stephanie Campbell, Independent Optometry Advisor Working with all practices and the 'Dementia Friends' Association to offer basic training to all staff, Stephanie and the team are also arranging multidisciplinary training for clinicians and managers to explore how to make consultations as accessible as possible for those with Dementia. Blaenau Gwent Across the East... National Clinical Priorities agreed! Due to high prevalence within the locality, Blaenau Gwent East NCN have voted and chosen to undertake the National Clinical Priorities in Cancer, Liver Disease and COPD. Blaenau Gwent East Flu Plan Practices and District Nurses have met to discuss the implementation of a Blaenau Gwent East NCN Flu Plan for this forth coming influenza season. There will be a structured and coordinated approach to the vaccination of housebound patients and their Carers as well as specific sessions for the Care Home patients. This should reduce the number of times that District Nurses attend homes and also ensure that all patients are covered by the vaccination at the same time. The Practices will also be working closely with Community Pharmacies to promote the uptake of patients who are at risk. There are a number of Saturday clinics planned by the Practices to increase access to flu vaccinations. All of these initiatives will be publicised locally to encourage greater uptake. Brynmawr Primary Care Resource Centre Work has now begun on Primary Care Resource Centre in Brynmawr, where it was anticipated that there will be a year s build programme; this is still running to schedule. The anticipated Practical Completion date is the end of Brynmawr Primary Care Resource Centre Extensive work has been undertaken to develop appropriate plans to meet the needs of the local population, in line with the Health Board s Clinical Futures Strategy and the now Care Closer to Home Strategy. The Resource Centre will include a range of Primary Care and Community Care Services, which includes: General Medical Services Community Dental Services District Nursing Reproductive and Sexual Health Mental Health Services Diabetic Retinopathy Screening Pharmacy Page5

6 Across the West... National Clinical Priorities agreed! The Blaenau Gwent West NCN practices thoroughly debated the National Clinical Priorities and chose to undertake Cancer, Liver Disease and Improving Mental Health & Wellbeing. Across Caerphilly Caerphilly NCNs working in collaboration with Youth Services Caerphilly NCNs and Caerphilly Youth Services, are developing plans to help take forward the NCN agenda for young people, which includes: encouraging young people in the at risk group to have their flu vaccinations signposting to local smoking cessation support services, with selected youth workers willing to be trained as Smoking Champions signposting young people with low level mental health and wellbeing issues to local wellbeing services supporting Young Carers Caerphilly Youth Service comes into contact with 12,000 young people aged per year, they operate the county s LGBT group for young people and some Youth workers are trained in Mental Health First Aid and the Student Assistance Programme. NCN leads will be working with the service to set up mechanisms whereby Primary Care Practitioners can work with the Youth Worker Service to help young people access advice, support and information in their local area. Caerphilly Care for Carers Caerphilly NCNs are working with Caerphilly Carers Team to promote how to access their service, which can be via GP or Community Staff referral, and the invaluable help, support and information available to carers. To give carers piece of mind, a Carers Emergency Card, has been developed to assist emergency services and others to identify people as carers and raise the alert that they look after someone who cannot manage without help or assistance. They can then contact the number on the back of the card so that someone can assist the person that is being cared for. The Team also organise various activities and events for carers to acknowledge their hard work and to give them a break from their caring role. 7 Across the East... National Clinical Priorities agreed! Caerphilly East NCN has chosen, via a voting process within the NCNs, to undertake the National Clinical Priorities in COPD, Dementia and Liver Disease. Caerphilly East will be funding some multidisciplinary dementia training to specifically improve the knowledge of practitioners and provide the opportunity to integrate research findings on the dementia/ gold-standard approach from professional bodies. Across the South... National Clinical Priorities agreed! Caerphilly South NCN practices have voted and agreed to undertake the National Clinical Priorities in Improving Mental Health & Wellbeing, Liver Disease and a local priority of Atrial Fibrillation. Across Monmouthshire National Clinical Priorities agreed! Following discussion amongst the North and South NCNs at M1, the following national clinical priority pathway areas were agreed: Cancer COPD Liver Disease The next step is to undertake baseline audits to inform peer review sessions at each of the remaining NCN meetings in year 1. There are also plans for the final report to include a pan-monmouthshire perspective. Across the North... Welcome to Molly Morgan, NCN funded Community Dietician Molly Morgan, Obesity Specialist Dietitian, is providing maternity cover for the NCN funded Community Dietician post, which aims to expand and enhance obesity services in North Monmouthshire. In addition to regular Dietetic Weight Management clinics, Molly offers an Antenatal Obesity clinic and a Diabetes Prevention Programme for eligible patients. Priorities agreed! Monmouthshire South have agreed the following priorities for : Local priority: Ongoing development of South Monmouthshire Hubs In order to support the Care Closer to Home strategy and in alignment with Public Health s Integrated Wellbeing Network and Monmouthshire County Council s approach to Place-based working, the NCN agreed to continue to support the Page7

8 development of South Monmouthshire hubs at M1. Population need priority: Tackling maternal and childhood obesity The NCN is in the process of developing a 3 year action plan with partners. In terms of priorities the NCN has agreed, in partnership with the Monmouthshire National Exercise Referral Scheme (NERS) and local Midwifery teams, to fund an antenatal Pathway to Pregnancy scheme, due to commence later this year. The programme fits with the population level priority of tackling obesity and will include aqua and dryside exercise classes. Also linked to this theme, the NCN will fund a multi-agency childhood obesity workshop which is also planned for later this year, in order to map current service availability and consider any gaps. Update from Tom Willgoss, NCN Practice-based Pharmacist Tom Willgoss is operating medication review clinics at Grayhill and Mount Pleasant Surgeries, in addition to supporting the Wye Dean Surgery team with Polypharmacy medication reviews. He is also regularly involved in the following pieces of work: Supporting GP workload by actioning discharge summaries from Secondary Care Tom Willgoss Actioning medication queries that would otherwise go to GPs NCN Funded Practicebased Pharmacist Cost-saving work reducing prescribing expenditure Visiting Nursing and Residential Homes to complete medication reviews on residents Domiciliary medication reviews Driving and developing the use of Repeat Dispensing in practices Prescribing audit work (QOF and CEPP) Pulmonary Rehabilitation presentations on the role of the community pharmacist to avoid reliance on seeing a GP thomas.willgoss@wales.nhs.uk Across Newport Health Check Living Well, Living Longer Will Beer (centre), Newport East NCN Lead and Public Health Consultant and the Living Well Living Longer Team The ABUHB Living Well, Living Longer Programme will be bringing its Health Check Programme to Newport East and West from July for the next few months. The programme invites residents, aged between 40 and 64, who are registered with a GP practice and not currently experiencing any Cardio Vascular health issues, or on any relevant medication or disease register, to receive an NHS health check, or a midlife MOT. Health Checks will take place at one of the local community venues being used across Newport. Individuals will then supported to identify lifestyle changes to lower their identified risks and signposted to local services such as Stop Smoking Wales, National Exercise Referral Scheme (NERS), Adult Weight Management Services and the Gwent Drug & Alcohol Service (GDAS). This is a Welsh Government Tackling Poverty priority and is being delivered by Aneurin Bevan University Health Board and Public Health Wales. Page8

9 Multi-agency Mental Health model for under 18 s goes live in Newport! The development of the Primary Care Mental Health Support Service single point of access (PCMHSS SPA) fits well with NCN priorities and was approved by the North, East and West Newport NCNs who proposed Newport as a trial site. CYP and family attends GP consultation Mild-moderate emotional/behavioral need identified. Hopes of support are discussed and documented. GP informs CYP and family that they will refer to multiagency panel. Provide information leaflet. GP gets consent from CYP/family to refer to multi-agency project. The GP notes who is giving verbal consent and this is included in referral documentation. GP writes a referral letter or completes a referral form. This is sent direct to Families First Newport Every Tuesday morning Referrals discussed in Families First panel meeting in collaboration with PCMHSS CYP. Referral needs are matched and allocated to relevant strands/services Service/Strand allocated will contact GP/referrer to advise of panel meeting outcome. Service/Strand will carry out assessment and if needed, intervention. GP will be informed of involvement following assessment and at discharge. All GP referrals regarding children and young people (under 18) experiencing mild-moderate emotional, mental health and behavioural difficulties are received by Families First Newport via the SPA. The initiative aims to ensure that children and young people are seen by the right person, at the right time: To provide effective early intervention With good communication between different agencies leading to reduced duplication More effective sharing of skills and enhanced partnership working. Newport North and East NCNs will be undertaking their National Clinical Audits in 2017/18 on a collaborative basis. They have chosen Mental Health (Early Intervention), Liver Disease and a local priority of Frailty. As with the first NCN meeting of the year which addressed Mental Health, meetings 2 and 3 will be held on a joint basis, with partners and other agencies invited to attend a workshop to strengthen integration and improve partnership working. The outcome of these workshops will be to develop and agree local implementation plans for all three priority areas. Progress will be reviewed against these plans throughout 2017/18 and reported back through the NCNs. Page9

10 North & East Newport NCNs join forces strengthen Mental Health partnership working for children and young people across Newport North and East Newport NCNs held a joint meeting on 13 th July 2017 at the Christchurch Centre, Newport to address delivery requirements for the 2017/18 QOF National Clinical Priorities. The key role that GP Practices and wider NCN Partners have in the early recognition, diagnosis and treatment of mental health problems in children and young people was discussed. Attendees at the Improved Mental Health & Wellbeing Over 50 multi-agency and Third Sector colleagues attended the event. An action plan to address the issues identified will be developed based on following the four main themes: 1. Improved communication 2. Reduced bounce backs between services 3. Development of a single point of access 4. Parenting responsibilities / managing parental expectation. Across the West... Priorities agreed! Newport West NCN have chosen to undertake COPD, Liver Disease and a local priority of Frailty as their National Clinical Priorities for 2017/18. The NCN will be working collaboratively with Newport North and East NCNs for the Liver and Frailty priorities. GP Practices have been asked to collate their baseline information in preparation for the first meeting on 20 th July 2017 and will agree the next steps and a timetable to complete the process over the forthcoming year. Across Torfaen Care Closer to Home turning our strategy into action The second Care Closer to Home Workshop took place in Torfaen on 13 th July, with a focus on turning our strategy into action. A wide range of partners attended and took part in workshops which explored: What would success look like if we were working in an integrated fashion What challenges would we face How could we overcome these challenges, and What support would we need from our leaders and organisations Attendees at the Care Closer to Home Workshop The discussions were aided by a presentation on the Monmouthshire experience how their teams got to a place where they are working in an integrated fashion. A staff member portrayed a service user during a role play who was asked questions by the audience about their experience of current working arrangements this was quite powerful and set the tone for the workshops. Workshops will be held across Gwent over the coming months. Page10

11 Direct Access Physiotherapy Service rated as excellent by patients The Musculoskeletal Direct Access Physiotherapy Service has been live in Torfaen Since 3 rd April The service operates from 09:00am 11:30am at the Cedar Unit, County Hospital, Pontypool. The service operates a drop in clinic on a first come first served basis to provide assessment of patients presenting with a musculoskeletal problem. Demand has increased steadily since the introduction of the service with 315 patients receiving a musculoskeletal assessment between April 2017 and mid July Following assessment, a joint decision is made between the physiotherapist and the patient regarding their treatment options. Results so far indicate that: 13.8% of patients are discharged to self-manage 68.6% of patients are placed on the physiotherapy routine waiting list 15.9% of patients are offered an urgent appointment within the physiotherapy service 1.8% of patients are signposted back to the GP o 0.7% are referred back to the GP for further investigation of their condition or related condition o 1.1% for onward referral to a consultant led secondary care service Over 53% of patients who have attended the clinic have completed a questionnaire about their experience. Patients have given very positive feedback with 89.5% of patients rating the service as excellent and a further 9.8% rating the service as good. Childhood Obesity Following the success of last year s Childhood Obesity Workshop, a second workshop, funded by North and South Torfaen NCNs was held on 8 th June Linking with Fit for Future Generations and Every Child Has The Best Start In Life we explored what is currently being undertaken to create safe, active communities and ways to develop this area. We also planned how to deliver more effective weight management services for children, young people and families in Torfaen. Torfaen NCNs have agreed to continue with obesity as a local priority, which will be progressed by the Management Team. Across the North... National Clinical Priorities agreed! As with other NCNs, Torfaen North NCN have agreed to develop an Integrated Wellbeing model supported by Public Health colleagues. In line with the GP contract (Quality Outcomes Framework) the NCN continues to strive to support GP Practice sustainability through dedicated funding streams for Direct Access Physiotherapy, Social Prescribing and a Practice Based Pharmacist. The NCN also supported to re-implementation of a Practice Manager Forum which allows sharing of ideas, best practice and provides direct access to the NCN Leads. The NCN also agreed to take forward Liver Disease, Cancers and COPD as National Clinical Priorities. Page11

12 Update from Carl Peacock, NCN Practice-based Pharmacist Carl Peacock works across 6 GP practices, providing an invaluable service in North Torfaen leading and delivering a specialised pharmacy service to GP practices to improve the quality and effectiveness of medicines management within GP practices. Carl has recently gained his Independent Prescriber status and provides patient focussed, prudent healthcare, reviewing polypharmacy and promoting medicines safety. Carl Peacock, NCN Funded Practicebased Pharmacist Across the South... National Clinical Priorities agreed! Torfaen South NCN have also agreed to take forward Liver Disease, Cancers and COPD as National Clinical Priorities and will discuss relevant action at M1 in August The NCN has also agreed to adopt the development of a Torfaen-wide Integrated Wellbeing model with the help of Public Health colleagues, underpinned by the role of the Torfaen Social Prescribers. NCN Action Plan The draft 3 year NCN action plan will be shared with NCN members prior to M1 (August 2017) in order to finalise relevant action and themes. The NCN is obliged to sign-off the plan by the QOF deadline of 31 st July Update from Lisa Pottenger, NCN Practice-based Pharmacist Lisa Pottenger works across 7 GP Practices and is a core member of the practice team, providing a significant cost saving to the individual surgeries and NCN. She works effectively with other health and social care professionals to deliver services to patients, who are positive about the valuable service she provides. Lisa Pottenger, NCN Funded Practice-based Pharmacist Lisa will be using her Independent Prescriber in Type 2 Diabetes qualification to commence routine diabetes reviews in care home and housebound settings, and will continue to: Carry out medication and polypharmacy medication reviews, in nursing homes, patients homes and within the GP surgery. Support the practices to achieve CEPP/QOF initiatives and support all surgeries with the Repeat Dispensing project. De-prescribe medication, gradually withdrawn where appropriate as medication is unnecessary/reduce tablet burden. Medication dosages have been reduced in light of renal function. Provide educational training sessions to practices - such as advanced inhaler technique training and regarding drug alerts such as warning of DKA and lower limb amputations with SGLT-2 inhibitors Conduct multiple drug safety searches and changed numerous patients therapy such as Simvastatin in combination with Amlodipine and SGLT-2 with reduced renal function or foot complications. lisa.pottenger@wales.nhs.uk Page12
